A Doença de Fahr ocorre devido depósitos de cálcio nos núcleos da base, centro sinovial e núcleos denteados do cerebelo bilateralmente, identificado facilmente por uma tomografia computadorizada (TC). Ela apresenta uma clínica diversificada que vai desde alterações comportamentais a distúrbio dos movimentos, da fala, paresias, cefaleia, sinais piramidais e convulsões. Devido quadro clínico inespecífico, essa patologia pode ser facilmente confundida com outras enfermidades. A paciente descrita no relato foi diagnosticada com Doença de Fahr durante sua internação, de forma que a sintomatologia poderia ser equivocadamente associada à essa identificação. Porém a suspeita de um quadro infeccioso, devido os exames laboratoriais e presença de um abscesso em região perianal, direcionou a pesquisar por outras causas associadas, sendo determinado neste ato a meningite bacteriana.
