Objetivo: relatar um caso de linfonodomegalia axilar metastática como apresentação de melanoma oculto diagnosticado em uma paciente no Hospital Universitário Walter Cantidio (HUWC-UFC) em 2017. Metodologia: resgatou-se o prontuário no Serviço de Arquivos Médicos do HUWC, onde a paciente foi atendida em ambulatório de Cirurgia Oncológica e realizada cirurgia de ressecção de tumor em axila. Iniciou-se pesquisa bibliográfica da literatura nacional e internacional, abrangendo relatos de caso, estudo de casos, artigos de revisão escritos nas línguas inglesa, portuguesa e espanhola. Resultados: paciente, 60 anos, sexo feminino, chegou ao ambulatório de cirurgia oncológica do HUWC com linfonodomegalia axilar direita, dolorosa, diâmetro de aproximadamente 10 cm, com 6 meses de evolução. Traz resultado de histopatológico de março 2017 mostrando linfadenectomia axilar metastática com células em anel de sinete. Imuno-histoquímica sem alterações que direcionasse o diagnóstico. Pesquisada e descartada neoplasia gástrica. Iniciou-se quimioterapia para neoplasia de mama sem resposta adequada. Solicitado revisão de imuno-histoquímica que agora demonstra positividade para Mel-CAM. Realizada exérese de linfonodomegalia axilar direita com margens cirúrgicas. Conclusões: temos uma frequência de 3,2% de melanoma metastático com tumor primário oculto sendo relevante e necessário relatos de casos para constante discussão deste tema.
