Polymyxins are antibiotics developed in the 1950s. Polymyxin-induced neurotoxicity has been often described in medical literature. The same cannot be said of nephrotoxicity or tubulopathy in particular. This report describes the case of a patient prescribed polymyxin B to treat a surgical wound infection, which led to significant increases in fractional excretion of calcium, magnesium, and potassium and subsequent persistent decreases in the levels of these ions, with serious consequences for the patient. Severe hypocalcemia, hypomagnesemia, and hypokalemia may occur during treatment with polymyxin. Calcium, magnesium and potassium serum levels must be monitored during treatment to prevent life-threatening conditions.
RESUMO
A ocorrência de Injuria Renal Aguda (IRA) em pós-operatório de cirurgia cardíaca é evento com impacto em mortalidade e morbidade. A incidência pode passar os 30% e quando métodos de diálise são necessários à mortalidade ultrapassa os 60%. A prevenção de disfunção renal é mais eficaz do que o tratamento. Conhecer a população de risco é importante na intensificação dos cuidados. O objetivo do trabalho foi conhecer o perfil dos pacientes evoluindo com IRA após a realização de cirurgia cardíaca em pós-operatório mediato. Analisando o prontuário de 84 pacientes, a incidência de IRA foi de 29,7%. Em sua maioria eram do sexo masculino (76%), hipertensos (80%), fizeram uso de circulação extracorpórea (76%), possuíam uma doença renal crônica classe 3a, com Taxa de Filtração Glomerular média de 56 mL/min/1.73m². Esses pacientes apresentaram menos diurese do que os pacientes que não cursaram com disfunção renal e tinham uma tendência a um balanço hídrico menos negativo. O uso de inotrópicos por mais de 48 horas não foi um bom preditor para IRA. Injuria renal aguda é evento grave e comum em pós-operatório de cirurgia cardíaca. A prevenção e monitorização deve ser rotina nos cuidados pré e pós-cirurgicos, intensificada em pacientes mais idosos e com disfunção renal prévia.
