Systémy zdravotníctva vo vyspelých krajinách v súčasnosti sú a v najbližších dekádach aj budú z pohľadu verejných fi nancií vystavené najmä dopadom procesov starnutia populácie, a to v podmienkach prekonávania dôsledkov dlhovej krízy. V súvislosti s novými populačnými trendmi vo vyspelých krajinách, sprevádzanými najmä poklesom pôrodnosti, predlžovaním veku dožitia a celkovým starnutím populácie, sa preto pozornosť politikov ale aj vedeckých kruhov stále intenzívnejšie orientuje na tvorbu vízií a nových politík, ktoré by refl ektovali najmä niektoré nové nosné výzvy.Napr.: aký bude vývoj a štruktúra dopytu po zdravotnej starostlivosti -najmä zo strany rastúcej kohorty post produktívneho obyvateľstva? Aký bude vývoj dopytu po kombinovanej sociálno-zdravotnej starostlivosti pre seniorov? Ako zabezpečiť adekvátne kapacity zo strany poskytovateľov zdravotnej a kombinovanej sociálno--zdravotnej starostlivosti pre seniorov? Ako sa vyššie uvedené trendy prejavia na rovnováhe/nerovnováhe príjmov a výdavkov zdravotného systému.Systém zdravotníctva v Slovenskej republike (ďalej len SR) je vystavený neustá-lym reformám, resp. korekciám, najmä v dôsledku jeho permanentne sa obnovujú-cej nerovnováhy, ale aj v dôsledku absencie politického konsenzu z hľadiska jeho základného konceptu.1 Je to tak v oblasti inštitucionálnej, v oblastiach vlastníckych foriem poskytovateľov zdravotnej starostlivosti, systému zdravotného poistenia, ako aj všeobecne v oblasti fi nancovania.Príspevok sa venuje najmä fi škálnej ne/rovnováhe vo verejnej časti systému zdravotníctva a prognóze bilancie v podmienkach SR. Vychádza z analýzy teoretických prístupov a súčasného stavu poznatkov o systémoch zdravotníctva so zameraním sa na ich fi škálnu udržateľnosť. Analýza systému zdravotníctva na Slovensku a následné použitie vybraných simulačných modelov poskytuje v závere alternatívne prognózy bilancie príjmov a výdavkov systému na najbližších dvadsať rokov. Na očakávané populačné zmeny reagujú prognózy alternatívnymi politikami na strane príjmov tak,
