Introdução: A osteonecrose dos maxilares associada ao uso de medicamentos é uma doença que se dá pelo uso de terapias com drogas antirreabsortivas, como bisfosfonatos e Denosumab, ou antiangiogênicos. Embora a manifestação desta doença não tenha alta incidência, sua ocorrência gera um impacto significativo na vida do paciente, levando à sintomatologia dolorosa, com supuração e odor fétido. Apesar dos vários tratamentos empregados, não existe um consenso acerca da maior eficácia de um tratamento frente a outro. Assim, este trabalho objetiva demonstrar as diferentes modalidades de tratamento e os benefícios aliados a cada uma. Materiais e Métodos: Buscas feitas na base de dados PubMed/MEDLINE sobre o tema Osteonecrose dos Maxilares Associada ao Uso de Medicamentos, aliado a operadores boleanos, durante os meses Maio, Junho e Julho de 2020. Resultados: A pesquisa resultou em 143 artigos e, após a aplicação dos critérios de elegibilidade, 49 foram selecionados (23 estudos prospectivos, 17 retrospectivos e 9 séries de caso), englobando 2617 pacientes e 2848 lesões. Os tratamentos foram divididos em 5 modalidades (terapias adjuvantes, tratamentos cirúrgicos agressivos, terapias teciduais, tratamento assistido por medicamentos e terapias cirúrgicas conservadoras).Conclusão: Vários tratamentos são propostos e parece claro que as cirurgias agressivas são as terapias com melhores resultados, sobretudo quando aliadas a uma terapia tecidual ou adjuvante. As cirurgias conservadoras apresentam papel importante, principalmente, em pacientes portadores de condições sistêmicas que contraindiquem as cirurgias maiores.
Palavras-chaveObjetivos: Descrever um caso clínico de tumor de células gigantes em mandíbula tratado por curetagem. Relato de caso: Paciente de 9 anos, sexo feminino, leucoderma, compareceu ao serviço de CTBMF do hospital de Clínicas de Uberlândia, queixando-se de aumento de volume em região de sínfise mandibular, assintomática. A Tomografia evidenciou uma lesão expansiva envolvendo os elementos 34 ao 43, com deslocamentos dos adjacentes. Resultados: Punção aspiratória negativa para líquido e a biópsia incisional com lesão Central de Células Gigantes. A abordagem conservadora para reduzir o tamanho da lesão, foi realizada com um protocolo de 8 aplicações intralesionais quinzenalmente de 20 mg/ml de triancinolona hexacetonida diluída. Ao final das aplicações, a paciente permaneceu ausente por 9 meses, retornando com um aumento considerável da lesão, que agora se estendia dos dentes 36 até o 45 com grande expansão vestíbulo-lingual. Conclusão: Diante do caráter agressivo da lesão e da falta de respostas às injeções intralesionais do corticóide, a paciente foi levada para curetagem da lesão em ambiente hospitalar e instalação de placa de reconstrução mandibular. A paciente apresentou boa cicatrização, melhora estética e aguarda o fim do seu desenvolvimento esquelético para reconstrução óssea na região operada. Sem recidiva em 5 anos de acompanhamento.Células Tumorais, células gigantes, mandíbula.
O enfisema subcutâneo é uma complicação incomum, porém séria que pode ser proveniente de uma cirurgia odontológica. Essa condição é caracterizada pela injeção forçada do ar no tecido conjuntivo frouxo abaixo da derme ou mucosa que tem como característica clinica há uma sensação de crepitação ao toque. Embora o quadro seja benigno e autolimitado, há um risco de progressão para consequências mais graves, entretanto, na maioria dos casos o enfisema subcutâneo é reabsorvido espontaneamente sem complicações e sem abordagem cirúrgica ao fim de alguns dias. O objetivo deste trabalho é apresentar um extenso enfisema subcutâneo, abrangendo desde espaço supraorbitário direito até espaço visceral do pescoço, mediastino superior e espaço perigoso, com extensão contralateral para os espaços submandibular e sublingual esquerdo, decorrente de utilização de turbina de alta rotação durante realização de biopsia incisional em mandíbula, bem como as terapias medicamentosas e de suportes utilizadas para conduzir o presente caso.
