Immediate electronic transfer of a preliminary radiology report results in a small but important number of adverse outcomes; however, if a final edited report follows within 24 hours and referring physicians are called whenever the preliminary report contains erroneous information, the benefits of rapid information transmission may outweigh the additional risks.

Ziel: Bei prim?rem Engstand im Wechselgebiss kann ausreichend Platz f?r die permanenten Z?hne geschaffen werden, durch deren Durchbruchssteuerung mittels fr?hzeitiger Extraktionen von Milchz?hnen und anschlie?ender Extraktion von bleibenden Z?hnen.
Dieser Artikel stellt eine Orientierungsstudie zur ?berpr?fung der Modifikation zur klassischen Serienextraktion nach 10-j?hriger klinischer Anwendung vor.
Patienten und Methodik: 3 Patientengruppen mit jeweils 50 Probanden wurden gebildet und untersucht: Bei den Patienten der Gruppe 1 (G1) kamen initial die Milcheckz?hne im Unterkiefer zur Extraktion. Bei der Gruppe 2 (G2) wurden initial die ersten Milchmolaren im unteren Zahnbogen extrahiert. Die dritte Gruppe (G3) fungierte als Kontrollgruppe ohne Extraktionen. Zur Kontrolle dienten OPAN-R?ntgenbilder, einmal zum Zeitpunkt t1 (jeweils vor Beginn der durchgef?hrten Extraktionen) und zum Zeitpunkt t2 (durchschnittlichen 2,5 Jahre post extractionem). F?r die Kontrollgruppe ohne Extraktionen wurden zeitlich ?quivalente OPAN-Aufnahmen zur Beurteilung verwendet. Ausgewertet wurden die relativen Lagever?nderungen der permanenten Eckzahn- und Pr?molarenkeime.
F?r die sich anschlie?ende Orientierungsstudie zur Pr?fung einer Behandlungsoptimierung und deren statistische Eruierung der Anwendungsh?ufigkeit bez?glich der vorgenommenen Extraktionsmodifikation, kamen 63 Probanden mit der Indikation zur Extraktion von ersten Pr?molaren bei prim?rem Engstand zur Auswertung.
Ergebnisse: In ?ber 90% der Extraktionsf?lle wurde die modifizierte Serienextraktion angewandt. Die Vorteile, wie die Umkehrung der nat?rlichen Durchbruchssequenz von Eckzahn und Pr?molaren im Unterkiefer als Folge des ge?nderten Extraktionsvorgehens, wirkten sich positiv auf die Eruption des ersten Pr?molaren aus.
Schlussfolgerungen: Die Modifikation der Serienextraktion im Unterkiefer erster Milchmolaren statt Milcheckz?hnen f?hrt zu einer ?nderung des Eruptionsverhaltens des bleibenden Eckzahnes und des ersten Pr?molaren. Das L?ckenmanagement im Extraktionsfall ist klinisch besser zu handhaben und die Dauer der Multibandbehandlungszeit verk?rzt sich signifikant. Die vorgestellte Modifikation der Serienextraktion kann als Alternative der klassischen Serienextraktion vorgezogen werden.
An interdisciplinary task force at the authors' institution developed a design for a new film library that placed heavy emphasis on service to its customers. The task force established locations and staffing for "satellite" inpatient film libraries organized by clinical specialties. Inpatient films were not permitted to be taken from the radiology department but were available for 24-hour viewing. Film locations were recorded in the radiology information system. Reorganization led to important improvements in film library operations and in the clinical staff's opinion of film library service. Objective measures of performance, such as the fraction of requested films available for conferences, showed significant improvement (P less than .001). Closer working relationships developed between film librarians, radiologists, and their clinical colleagues. Film library personnel were recruited more easily and stayed on the job longer. Several hundred thousand film-tracking transactions per year were recorded. The authors conclude that decentralization of certain film library activities improved many critical aspects of performance.
