Zwei einander entgegengesetzte Richtungen des menschlichen Denkens haben wesentlich zur Vertiefung der ~rztlichen Diagnostik beige~ragen: Auf der einen Seite das Bestreben nach immer feinerer Differenzierung klinisch a.hnlicher Krankheitsbilder, das zur allmi~h-lichen Trennung zwischen Masern und Scharlach, zwischen Typhus und Meningitis ffihrte; auf der anderen Seite das Suchen nach mOglichst umfassenden klinischen, pathologisch.anatomischen oder i~tio-logischen Einheiten, dem z.B. die Erkenntnis der genetischen ZusammengehSrigkeit der mannigfaltigen Krankheitsbilder tuberkulSsen oder luetischen Ursprungs zu danken ist. Neben unseh~tzbaren Fortschritten sind aber auch manche Irrtfimer und Rfickschritte in der Gesehichte der Medizin diesen an sich fruchtbaren Tendenzen entsprungen, n~mlich immer dann, wenn sie im Enthusiasmus neuer Entdeckungen sinnlos iibersteigert wurden.Eine solehe Welle kritikloser Ubertreibung in dem Streben nach ~tiologiseher Vereinheitlichung scheint heute den cerebralen Erkrankungen des friihen Kindesalters zu drohen. W~hrend der jahrzehntelangen Arbeit an der K|~rung der unter dem Sammelbegriff der cerebralen L~hmungen des friihen Kindesalters zusammengefafiten, anatomisch und ~tiologiseh so mannigfaltigen Krankheitsbilder hat bald die eine, bald die andere Richtung die 0berhand gewonnen. Es sei nur an die Aera yon Stri~mpell erinnert, der alle Symptome auf eine akute Polioencephalitis zuriickfiihren wollte, oder an die Zeit naeh den grundlegenden Arbeiten yon Little, Mac~utt und Kundrat, als das damals neu entdeckte Geburtstrauma ungebiihrlich in den Vordergrund geriiekt wurde. Sehon Freud warnt 1897 in seiner kritisehen Monographie vor solehen Ubertreibungen. Er schi~tzt die traumatische _&tiologie auf 50--60%; daneben seien vasculiire und entziindliche Ur. saehen sowie kongenitale Entwicklungshemmungen zum Teil noch unklarer Genese zu beriicksichtigen.
