O estudo teve como objetivo analisar a incidência de sífilis congênita no Brasil, entre 2011 e 2021, relacionando com o número de consultas de pré-natal realizadas pelas gestantes e o nível de escolaridade destas. Trata-se de um estudo observacional e descritivo, de base populacional e sem coleta de material biológico, com dados obtidos pelo Boletim Epidemiológico do Ministério da Saúde. Foram observados 201.572 casos de sífilis congênita em território nacional, com aumento anual, porém a partir de 2019 houve redução da quantidade de diagnósticos. Além disso, 160.780 gestantes com filhos que apresentaram sífilis congênita realizaram pré-natal com o número de consultas preconizadas pelo Ministério da Saúde. Em relação ao número de gestantes que tinham o diagnóstico de sífilis na gestação, 23,5% apresentavam ensino médio incompleto e apenas 0,9% com ensino superior completo, podendo-se, portanto, observar elevada prevalência em população com baixa escolaridade. Dessa forma, é possível avaliar a importância de agir sobre alguns fatores de risco para transmissão vertical da sífilis relacionados à escolaridade, como campanhas de conscientização.
The risk of infection is bigger in cases of active disease and in use of polypharmacy with immunosuppressive drugs. Exposure to immunosuppression is more relevant than the type of disease 1 . Discussing the impact of COVID-19 on rheumatologic patients is essential, given the dysregulation of the immune system and associated comorbidities 1,2 . Despite the increased risk of infection from the use of corticosteroids in moderate to high doses, the American Society of Rheumatology guidelines does not recommend abrupt discontinuation of treatment for rheumatologic diseases during a corticosteroid COVID-191. This study seeks to understand the impact of corticosteroid therapy on the emergence of flu-like symptoms during the SARS-CoV-2 pandemic.
METHODSThe cohort study was conducted by means of electronic interviews with patients and controls in the Brazilian Federal District in the first half of 2020, in which self-reported influenza symptoms were investigated following the Brazilian Ministry of Health criteria. this study was part of the Mario Pinotti II project from the Brazilian Society of Rheumatology. The data collected were analyzed using descriptive statistics. The association of influenza data and corticotherapy use was done by means of chi-squared or Fisher's exact test, and the normality of the data was tested with the Kolmogorov-Smirnov test.
O transtorno por uso de substâncias (TUS) corresponde a um padrão em que os pacientes usam determinada substância e têm prejuízos em várias esferas das vidas, sendo uma problemática evidente de morbimortalidade em muitos países. Nesse contexto, a ibogaína vem sendo estudada para o tratamento do TUS. Tal função medicamentosa está interligada à capacidade de gerar múltiplas interações com neurotransmissores associados à adicção. Assim, apesar de ser uma substância utilizada em diversos âmbitos há séculos, é essencial o estudo adequado da ibogaína para verdadeiro entendimento da droga e de seus possíveis efeitos no corpo humano. Dessa forma, esse estudo foi baseado em publicações de 2017 a 2022 encontrados no Pubmed, Google Acadêmico e Scielo e realizada revisão bibliográfica. As pesquisas evidenciaram que o uso de ibogaína para o tratamento do uso nocivo de substâncias pode ser intervenção eficaz, reduzindo sintomas de abstinência e do desejo de uso. No entanto, outros estudos relataram complicações graves, inclusive mortes, que parecem ter relação com os efeitos neurotóxicos e cardiotóxicos. A ausência de efeitos da ibogaína no TUS pode estar relacionada a fatores como a equivalência entre as doses terapêuticas não ser bem conhecida, a falta de grupos controle e placebo nos estudos e, ainda, o fato de que muitos dos tratamentos foram realizados em contextos não médicos. Além disso, tratam-se de estudos com reduzido número de amostras, evidenciando limitação para compreensão dos benefícios existentes.
