Several studies have established that systemic sclerosis patients have a reduced exercise capacity when compared to healthy individuals. It is relevant to evaluate whether aerobic exercise in systemic sclerosis patients is a safe and effective intervention to improve aerobic capacity. Seven patients without pulmonary impairment and seven healthy controls were enrolled in an 8-week program consisting of moderate intensity aerobic exercise. Patients and controls had a significant improvement in peak oxygen consumption (19.72+/-3.51 vs. 22.27+/-2.53 and 22.94+/-4.70 vs. 24.55+/-3.00, respectively, p=0.006), but difference between groups was not statistically significant (p=0.149). This finding was reinforced by the fact that at the end of the study both groups were able to perform a significantly higher exercise intensity when compared to baseline, as measured by peak blood lactate (1.43+/-0.51 vs. 1.84+/-0.33 and 1.11+/-0.45 vs. 1.59+/-0.25, respectively, p=0.01). Patients improved the peak exercise oxygen saturation comparing to the baseline (84.14+/-9.86 vs. 90.29+/-5.09, p=0.048). Rodnan score was similar before and after the intervention (15.84+/-7.84 vs.12.71+/-4.31, p=0.0855). Digital ulcers and Raynaud's phenomenon remained stable. Our data support the notion that improving aerobic capacity is a feasible goal in systemic sclerosis management. The long term benefit of this intervention needs to be determined in large prospective studies.
SSc patients without pulmonary impairment have reduced exercise capacity. Abnormal vascular response to exercise may account for this finding, as the vascular system is one of the major target organs in this pathological condition.
INTRODUÇÃO: Fibromialgia é uma síndrome crônica, caracterizada por dor músculo-esquelética generalizada. A possibilidade de atenuação dos sintomas com a atividade física abriu novas perspectivas para o tratamento desta doença. OBJETIVO: Avaliar o efeito de um programa de condicionamento físico sobre a capacidade funcional, dor e qualidade de vida de pacientes com fibromialgia. MÉTODOS: Adotado o desenho de coorte para avaliar 18 mulheres, média de 46,4 ± 5,8 anos de idade, com a síndrome em média de 10,6 ± 5,7 anos, submetidas a um ano de condicionamento físico supervisionado, predominantemente aeróbio. No início do estudo e trimestralmente foram realizados: teste de esforço cardiopulmonar para determinação da capacidade funcional; avaliação da intensidade de dor empregando a escala analógica visual; contagem dos pontos dolorosos e determinação do limiar de dor à pressão com o uso do algômetro de pressão; aplicação do questionário de qualidade de vida Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36) traduzido e adaptado para a população brasileira. RESULTADOS: A capacidade funcional melhorou a partir do terceiro mês (p < 0,05), o limiar de dor aumentou a partir do sexto mês (p < 0,05), houve diminuição da dor pós-esforço (p < 0,05) e do número de pontos sensíveis (p < 0,05) no nono mês. A intensidade de dor diminuiu no 12º mês (p < 0,05). Com exceção do item "estado geral de saúde" (p > 0,05), os demais domínios do questionário de qualidade de vida melhoraram em diferentes períodos do estudo (p < 0,05). CONCLUSÃO: As pacientes com fibromialgia submetidas ao programa de condicionamento físico supervisionado apresentaram melhora da capacidade funcional, da dor e da qualidade de vida.
Dentre as várias sequelas causadas pelo acidente vascular encefálico (AVE), destaca-se o comprometimento motor como a hemiplegia e a hemiparesia. A recuperação das sequelas neurológicas pode ocorrer de maneira espontânea, porém parte da recuperação depende de estímulo motor. Isto posto, o exercício físico é um método importante para a reabilitação e promoção da saúde em indivíduos que sofreram AVE. Objetivo: Verificar os resultados obtidos na força muscular global e em equilíbrio dinâmico, em indivíduos com hemiplegia pós AVE, que participaram de um programa de exercícios físicos. Métodos: Participaram do estudo 29 indivíduos, com média de idade de 57 anos. Foram analisados, retrospectivamente, dados dos prontuários de pacientes com diagnóstico de hemiplegia após AVE do Instituto de Medicina Física e Reabilitação do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo – IMREA HCFMUSP, unidade Lapa, que participaram de programa de exercícios no serviço de Condicionamento Físico da instituição no período de setembro de 2011 a julho de 2013. Resultados: Observou-se aumento significativo em força muscular (p<0,05) em todas as musculaturas envolvidas no teste de 10 RM. O maior ganho de força foi no grupo dos isquiotibiais (65,85%) e a musculatura com menor ganho de força foi tríceps braquial, com 31,34%. A média total de ganho de força foi de 45,20%. O tempo de realização dos testes Timed Up and Go (TUG) e Teste de Sentar e Levantar (TSL) foi menor ao término do programa, o que significa que os pacientes melhoraram a capacidade de realizar as mesmas funções avaliadas inicialmente. Conclusão: Este estudo mostrou que o treinamento resistido é muito importante para as pessoas com sequelas de hemiplegia pós AVE, por melhorar a capacidade funcional como o equilíbrio dinâmico, além de contribuir em suas atividades cotidianas com o aumento da força muscular global.
