Metadonas (Methadone, Dolophine) -sintetinis miu (µ 1 ir µ 2 ar OP 3 ) opioidinių receptorių agonistas. Metadonas sintezuotas Vokietijoje II Pasaulinio karo metu, kai sutriko natūralių opioidų gamyba. Metadonas yra ir veiksmingas analgetinis vaistas, ir preparatas, kuris slopina kitų opioidų nutraukimo simptomus (1-4). Todėl literatūroje minimos metadono vartojimo indikacijos (3, 5-11).Stipraus neuropatinio ir vėžinio skausmo malši-nimas, kai analgetikai arba pirmos eilės opioidai (pvz., morfinas) neužtikrina adekvačios skausmo kontrolės (12-17). Lėtinis (ne onkologinės ligos sukeliamas) skausmas blogina žmonių funkcinį pajėgumą ir gyvenimo kokybę (15). Tokiam skausmui malšinti metadonas vis plačiau vartojamas nuo 1990 m. (17), ypač per pastarąjį dešimtmetį (15). Metadono vartojimas JAV skausmui malšinti nuo 1998 iki 2006 m. išaugo beveik aštuonis kartus, t. y. nuo 531 000 atvejų iki 4,1 mln (17).Priklausomybės nuo opioidų gydymas -tai yra pakeičiamasis palaikomasis ilgamečių aguonų ekstrakto (i/v) ir heroino vartotojų gydymas (11,12,17,18). Ši gydymo metadonu programa (angl. MMTP -Methadone Maintenance Treatment Program) pasaulyje vykdoma nuo 1960 m. Daugiau nei prieš 10 metų ji įdiegta ir Lietuvoje. Nors Vokietijoje programa pradėta vėliau nei kaimyninėse šalyse, besigydančiųjų metadonu skaičius per pastaruosius penkerius metus pasiekė per 70 000, t. y. gydosi trečdalis ar net pusė ligonių, turinčių priklausomybę opioidams (18). Pakaitiniu gydymu metadonu siekiama: laipsniškai gerinti sergančiųjų priklausomybe nuo opioidų somatinę ir psichinę būklę; geriau organizuoti užsikrėtimo ŽIV, hepatitų B, C prevenciją, veiksmingiau gydyti gretutines ligas ir komplikacijas; gerinti sergančiųjų socialinę adaptaciją ir integraciją į visuomenę; mažinti narkotikų vartotojų nusikalstamumą bei mirties riziką nuo atsitiktinio heroino perdozavimo.Heroino vartotojams ankstyvos mirties rizika 6-20 kartų didesnė nei to paties amžiaus ir lyties žmonių, neturinčių priklausomybės. Vien Prancūzijoje metadono programa kasmetinį 500 mirčių nuo heroino skaičių sumažino iki 100 (1). Gydant metadonu, yra atsistatymo galimybė EEG normaliam smegenų aktyvumui temporalinėse srityse (5). FarmakodinamikaMetadonas yra visų opioidinių receptorių agonistas, nors dažniau minimas tik jo agonistinis poveikis miu (µ) receptoriams (lentelė). Tikslus metadono poveikio opioidiniams receptoriams profilis dar nepakankamai ištirtas (3,9,12,19,20). Raumenų sutraiškymas – tai galūnių ar kūno raumenų pažeidimas (trauminė rabdomiolizė). Sisteminės raumenų sutraiškymo pasekmės: rabdomiolizė, elektrolitų kiekio, šarmų ir rūgščių pusiausvyros pokyčiai, hipovolemija, ūminis inkstų nepakankamumas. The objective of this study was to identify case characteristics and clinical course of the disease in patients hospitalized with 2009 pandemic influenza A (H1N1) infection during the first wave of the pandemic and to identify risk factors associated with the complicated course of illness. Material and methods. A retrospective study of adult cases of the laboratory-confirmed 2009 pandemic influenza A (H1N1) virus admitted to three hospitals in Kaunas between November 1, 2009, and March 15, 2010, was carried out. The main outcome measures were clinical characteristics, risk factors for complicated disease, treatment, and clinical course of the disease. Results. The study enrolled 121 of the 125 patients hospitalized due to 2009 pandemic influenza A (H1N1) virus infection. The median age was 31 years (range, 18–83); 5% of the patients were aged more than 65 years. Pregnant and postpartum women comprised 26% of all hospitalized cases. Nearly half (49.5%) of those who underwent chest radiography had findings consistent with pneumonia, which was bilateral in one-third of cases. The risk to have pandemic influenza complicated by pneumonia increased significantly with one-day delay from symptom onset to antiviral treatment (OR, 2.241; 95% CI, 1.354–3.710). More than half (57%) of the patients received antiviral treatment. In 45% of the treated patients, antiviral drugs were administered within 48 hours from symptom onset. Intensive care was required in 7.4% of the cases. The overall mortality was 5% (6/121). The median age of the patients who died was 43.5 years (range, 23–62); 4 patients had been previously healthy, 1 patient suffered from chronic lympholeukemia, and 1 patient was a pregnant woman. Conclusion. The 2009 pandemic influenza A (H1N1) caused considerable morbidity in a significant proportion of hospitalized adults. The main risk factor associated with the complicated course of illness was delayed antiviral treatment.
