Niña de 7 meses de edad, sin antecedentes de importancia, que consulta en el Servicio de Urgencia por un cuadro caracterizado por tos y fi ebre de hasta 37,9 °C de cuatro días de duración. Al examen, se observa decaída, temperatura rectal: 38,6 °C, frecuencia respiratoria: 28 ciclos/min. El examen pulmonar muestra roncus y sibilancias bilaterales. El examen físico es por lo demás normal. Se sospecha una bronquiolitis aguda, pero se solicita radiografía (Rx) de tórax anteroposterior (AP) y lateral (Figura 1) para descartar neumonía.
