Objetivo: Comparar el abordaje quirúrgico transoral y el transcervical para estiloidectomía en síndrome de Eagle. Método: Presentación de casos de síndrome de Eagle, análisis de las ventajas y desventajas de los abordajes quirúrgicos transoral y transcervical, y revisión de la literatura. Resultados: Se presentan 8 casos de pacientes de sexo femenino entre 42 y 60 años con dolor cervical como síntoma principal. Fueron diagnosticadas con palpación de la fosa amigdalina, y con radiografía simple y tomografía con reconstrucción tridimensional. A 4 pacientes se les realizó estiloidectomía utilizando abordaje transoral sin amigdalectomía. En los otros casos se utilizó el abordaje transcervical, realizando procedimiento bilateral en 2 pacientes. Se realizó estiloidectomía unilateral izquierda en 6 casos. No hubo complicaciones intraoperatorias ni posoperatorias. Una paciente con estiloidectomía bilateral persistió con dolor cervical que mejoró al tratamiento con inyección local de esteroides. Conclusiones: Se recomienda la técnica transoral por la facilidad en el abordaje, menor tiempo quirúrgico, sin complicaciones infecciosas ni hemorrágicas, menor riesgo de lesión carotidea interna y de ramas marginales del nervio facial y sin cicatrices evidentes.
