This article aims to contribute to the debate on the SUS regionalization policy and the establishment of health regions in Brazil. Understanding them require to recognize the dichotomy between public health and individual health -which marks the history of Brazilian public health -and identify the different rationalities that lead this process. Such rationalities allow not only to consider the legacy of municipalization in the current regionalization process, as well as to establish links between the two fields of fundamental knowledge to the debate, epidemiology and geography. Clinical epidemiology, privileging individual health, gives basis to a healthcare model that prioritizes the optimization of resources. The recognition of health in its broader concept, in the social epidemiology, bases an attention model aimed at social determinants. With geography, functional regions can be formulated, based on Christaller's theory, or lablachianas regions which recognize the social loco / regional structure, allowing intervention in determining or conditioning the way of illness and death of populations.
RESUMO Este artigo tem como objetivo compreender os efeitos que o financiamento das ações e dos serviços públicos vinculado às redes temáticas de atenção à saúde tem sobre as transferências de recursos federais para o Fundo Municipal de Saúde no interior do Bloco de financiamento da Média e Alta Complexidade (MAC), no sentido de identificar em que medida a implementação dessas redes restringiu a autonomia dos entes municipais e quais foram prioritárias com relação ao volume de recursos efetivamente transferidos. De forma geral, indaga-se se as transferências vinculadas às redes temáticas significaram um acréscimo aos recursos do Bloco MAC, mantendo assim preservada a parcela de uso mais autônomo dos entes municipais.
O estudo proposto nesta tese aborda o processo de regionalização do SUS a partir de duas vertentes dinamizadoras. Uma orientada pela diretriz da descentralização políticoadministrativa e outra pela diretriz de regionalização e hierarquização da rede de saúde com foco na integração de atividades e serviços em regiões. Tal processo pode ter como resultado a mitigação ou o aprofundamento das desigualdades regionais. O processo de regionalização assim considerado foi analisado segundo duas dimensões do desenvolvimento capitalista, a territorial e a da saúde pública. Em relação à dimensão territorial, a divisão interregional do trabalho foi considerada elementar para a diferenciação das regiões segundo suas funções na organização da produção. É nessa perspectiva que recorremos à rede urbana como síntese da divisão interregional do trabalho. Em relação à dimensão da saúde pública, o norteamento para a compreensão da sua inserção no desenvolvimento capitalista foi dado por dois conceitos dicotômicos de saúde, saúde como ausência de doença e saúde como bem-estar social. É por meio deles que se torna possível identificar as diferentes funções sociais da saúde pública e, a partir daí, desvelar seu papel no aprofundamento do movimento desigual da acumulação do capital ou, ao contrário, como elemento de transformação social. Devido à importância do financiamento na elucidação do processo de regionalização, a tese evidenciou as transferências de recursos da esfera federal para os governos municípios no âmbito do bloco de financiamento da Média e Alta Complexidade Ambulatorial e Hospitalar (Bloco MAC) com foco nas estratégias vinculadas às redes temáticas. Foram observadas as transformações na concentração/desconcentração territorial dos recursos do Fundo Nacional de Saúde (FNS) para os Fundos Municipais de Saúde (FMS) paulistas no período 2009-2014, considerando as condições de urbanização das 63 Regiões de Saúde do estado de São Paulo (Metropolitana, Alta urbanização, Média urbanização e Baixa urbanização) propostas pela tese. A análise assim fundamentada tornou possível verificar que o processo recente de regionalização do SUS no estado de São Paulo, dinamizado pelo financiamento das ações e estratégias vinculadas às redes temáticas, aprofunda as características de distribuição espacial desigual no território paulista.
Objectives: to analyze the relation between cesarean section rates in SUS childbirth care establishments in São Paulo State and urbanization conditions, according to Robson group classification system. Methods: Sistema de Informação sobre Nascidos Vivos (Live Births Information System) and Cadastro Nacional de Estabelecimentos de Saúde (National Registry of Health Establishments) 2016 databases were analyzed. The studied outcome was cesarean section rates in the establishments, grouped by administration type (public or nonprofit entities) and urbanization condition. Results: the cesarean section rate in SUS childbirth care establishments was 50.5%, ranging from 41.1% in metropolitan regions up to 75.2% in the low urbanized regions. Cesarean section rates in public administration establishments (38.2%) were significantly lower than the nonprofit administration maternity hospitals (62.3%). Robson groups 5 and 2 contributed mostly to the cesarean section global rate (36.6% and 21.5%, respectively). Conclusions: The less urbanized regions showed significantly higher cesarean section rates than the metropolitan and highly urbanized regions. Cesarean section rates of public administration establishments were significantly lower than the nonprofit administration establishments. However, when separated by urbanization condition its difference was only observed in the metropolitan regions.
Objetivo: Sistematizar as principais características das portarias que orientam a transferências de recursos para a rede de cuidado obstétrico e neonatal (Rede Cegonha), a Rede de Atenção à Urgência (RAU), a Rede de Atenção Psicossocial (RAPS) e a Rede de Atenção à Pessoa com Deficiências (Viver sem limites). Métodos: As portarias foram analisadas a partir de três eixos estruturais: i) perfil assistencial dos elementos financiados, sendo observados os componentes e os equipamentos, insumos e ações; ii) o montante de recursos financeiros disponível para cada um dos elementos financiados, discriminado em investimentos e custeio; e iii) o comprometimento de cada um dos entes federativos no financiamento e execução do recurso. Resultados: A análise permite evidenciar três principais aspectos no repasse de recursos financeiros federais para os entes subnacionais: i) preponderância de recursos para o cuidado especializado, cuja configuração produtiva exige economia de escala, tendendo a se concentrar em municípios que apresentam maior centralidade; ii) indução ao comprometimento de recursos, financeiros ou não, dos entes subnacionais; e iii) grande fragmentação nos repasses financeiros. Conclusão: O financiamento das redes temáticas, além de favorecer o modelo médico assistencial por meio da transferência de recurso prioritariamente para a atenção à saúde voltada à média e alta complexidade, parece tender às regiões privilegiadas.
