Pada awal 2020, dunia dikejutkan dengan mewabahnya pneumonia baru yang bermula dari Wuhan, Provinsi Hubei yang kemudian menyebar dengan cepat ke lebih dari 190 negara dan teritori. Wabah ini diberi nama coronavirus disease 2019 (COVID-19) yang disebabkan oleh Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2). Penyebaran penyakit ini telah memberikan dampak luas secara sosial dan ekonomi. Masih banyak kontroversi seputar penyakit ini, termasuk dalam aspek penegakkan diagnosis, tata laksana, hingga pencegahan. Oleh karena itu, kami melakukan telaah terhadap studi-studi terkait COVID-19 yang telah banyak dipublikasikan sejak awal 2020 lalu sampai dengan akhir Maret 2020.
ABSTRAKPendahuluan. Insiden infeksi jamur invasif dilaporkan semakin meningkat di berbagai negara. Candida merupakan genus jamur yang paling sering menyebabkan infeksi jamur invasif. Penelitian ini bertujuan untuk mengetahui prevalensi, karakteristik klinis, dan pola sebaran spesies jamur pada pasien kandidiasis invasif (KI) dewasa.Metode. Penelitian ini merupakan studi retrospektif, menggunakan desain potong lintang, berdasarkan data sekunder pasien sepsis yang dirawat di Rumah Sakit dr. Cipto Mangunkusumo (RSCM) Jakarta sejak Januari 2012 sampai Juni 2014. Dilakukan pemilihan pasien KI berdasarkan kriteria EORTC/MSG 2008. Selanjutnya, dilakukan pencatatan data demografis, data klinis dan penunjang, diagnosis, spesies penyebab, jenis obat antifungi dan antibiotik yang diberikan, luaran klinik, serta masa rawat.Hasil. Prevalensi KI di RSCM adalah 12,3%. Dari 91 pasien, didapatkan 35 pasien termasuk kategori proven, 31 pasien probable dan 25 pasien possible. Manifestasi klinik KI yang paling sering adalah kandidemia, dengan penyebab utama Candida albicans. Rerata usia pasien 47,9 tahun, didominasi oleh pasien medis yang dirawat di ruang rawat biasa, nonneutropenia, dan menderita syok sepsis. Kebanyakan pasien menderita keganasan yang seringkali disertai infeksi paru, sedangkan piranti medik yang paling sering digunakan adalah kateter urin. Umumnya, pasien mendapat antibiotik Sefalosporin generasi tiga, sedangkan antifungi yang paling sering digunakan adalah flukonazol. Sebanyak 44% pasien KI tidak mendapatkan pengobatan antifungi sistemik. Tingkat mortalitas pasien KI didapatkan sebesar 64.8% dan median masa rawat total adalah 27 hari.Simpulan. Prevalensi kandidiasis invasif sebesar 12,3% dengan mortalitas yang cukup tinggi dan C. albicans merupakan spesies yang paling sering ditemukan.
Pendahuluan. Ketepatan pemberian antibiotik dapat meningkatkan keberhasilan terapi pada pneumonia komunitas dan mencegah timbulnya resistensi bakteri terhadap berbagai macam antibiotik. Community-acquired pneumonia (CAP) masih banyak ditemukan di Indonesia dengan angka insiden 2,4%. Tujuan penelitian ini adalah untuk menilai ketepatan pemilihan antibiotik pada CAP dengan menggunakan alur Gyssens yang diberikan berdasarkan panduan IDSA-ATS 2007, serta hubungannya dengan respons klinis pasien.Metode. Penelitian ini merupakan penelitian kohort retrospektif dengan menggunakan data sekunder dari data penelitian sebelumnya. Penelitian dilaksanakan pada tiga rumah sakit di Jakarta yaitu RS Cipto Mangunkusumo, RSUD Budhi Asih dan RSPAD Gatot Subroto selama periode bulan September 2016 hingga November 2017.Hasil. Pada penelitian ini, dari 151 pasien yang mendapat antibiotik pertama yang diberikan secara monoterapi, pemberian yang tepat dengan kategori 0 dan 1 didapatkan sebanyak 38 pasien (25,2%). Untuk pemberian antibiotik yang tidak tepat (kategori II-V) didapatkan sebanyak 113 pasien (74,8%) yang sebagian besar karena pemberian yang terlalu singkat (kategori III-b), yaitu pada 51 pasien (33,8%). Untuk antibiotik kedua yang diberikan secara kombinasi, pemberian yang tepat didapatkan pada 15 pasien (9,9%). Uji hubungan antara ketepatan pemberian antibiotik pertama dengan perbaikan klinis pada CAP mendapatkan nilai RR=0,970 (interval kepercayaan [IK] 95%: 0,80 – 1,16); nilai p=0,738. Sementara pada antibiotik kedua didapatkan nilai RR=1,194 (IK 95%: 0,648 – 2,20); nilai p=1,00. Dengan demikian, tidak didapatkan adanya hubungan yang bermakna antara ketepatan pemberian kedua antibiotik tersebut pada CAP yang dinilai berdasarkan alur Gyssens dengan perbaikan klinis pasien.Simpulan. Tidak didapatkan adanya hubungan yang bermakna antara ketepatan pemberian antibiotik pada CAP yang dinilai berdasarkan alur Gyssens dengan perbaikan klinis pasien. Kata Kunci: Alur Gyssens, CAP, Perbaikan klinis, Terapi antibiotik Correlation between Proper Antibiotics Administration to Community Acquired Pneumonia Patients Based on Alur Gyssens with Clinical ResponseIntroduction. The proper of antibiotics treatment will increase the improvement of the patient with community-acquired pneumonia (CAP) and also counteract antimicrobial resistance. Community-Acquired Pneumonia is commonly found in Indonesia with an incidence of 2.4%. This study aimed to determine the proper of antibiotic selection for CAP based on IDSA-ATS 2007 guidelines with Gyssens method and evaluate the relationship between the proper of antibiotic treatment with the clinical response of patients. Methods. A retrospective cohort study was conducted with secondary data from the previous study. Study was conducted in three hospitals in Jakarta, namely Cipto Mangunkusumo General Hospital, Budhi Asih Hospital, and Gatot Subroto Hospital between September 2016 and November 2017.Results. From 151 patients, the appropriate of the first antibiotic treatment, including category 0 and 1 was found in 38 patients (25.2%), while for inappropriate one (category II-V) was in 113 patients (74.8%) mainly due too short antibiotic treatment. For the second antibiotic given to 54 patients, appropriate treatment was found in 15 patients (9.9%). There was no correlation between the accuracy of both antibiotics treatment in CAP based on Gyssens method with clinical response of the patients (first antibiotic treatment: RR=0,970 (95% CI: 0,80 – 1,16), p=0,738; second antibiotic treatment: RR=1,194 (95% CI: 0,648 – 2,20), p=1,00).Conclusion. There was no correlation between the accuracy of antibiotics treatment in CAP based on Gyssens method with clinical response of the patients.
Terapi antibiotika awal dan tepat merupakan faktor yang penting untuk kesintasan pasien sehingga diperlukan pemeriksaan yang cepat dan akurat untuk deteksi adanya bakteri di sirkulasi. PCT merupakan biomarker yang paling sering dipelajari dan rutin digunakan dalam praktik klinik dan rekomendasi saat ini di beberapa negara. Kadar PCT serum meningkat pada sepsis. Kadar PCT normal di bawah 0,5 ng/mL dan kadar PCT > 2 ng/mL memiliki risiko tinggi untuk sepsis. PCT lebih unggul daripada CRP untuk diagnosis dan prognosis sepsis pada pasien kritis tetapi penggunaannya harus tetap diiringi dengan penilaian secara klinis. Hal ini terutama penting pada awal infeksi atau pasien dengan infeksi fokal dan pasien pembedahan. PCT mungkin lebih baik untuk menyingkirkan diagnosis sepsis daripada untuk diagnosis sepsis itu sendiri pada pasien kritis terutama jika dilakukan pemeriksaan PCT serial. Pemeriksaan PCT juga dapat digunakan untuk membantu dalam penggunaan antibiotika. Pemeriksaan PCT dapat digunakan untuk menghindari penggunaan antibiotika yang tidak diperlukan pada pasien kritis dengan gejala SIRS tanpa infeksi; walaupun demikian, emeriksaan PCT tetap harus diinterpretasikan sesuai dengan temuan klinis dan parameter laboratoris lainnya.
ABSTRAKPendahuluan. Sepsis Berat merupakan masalah kesehatan dengan tingkat mortalitas yang tinggi serta insiden yang terus meningkat. Bersihan laktat menggambarkan kinetika metabolisme anaerob pasien sepsis berat dan merupakan parameter yang potensial untuk mengevaluasi kondisi penyakit dan intervensi pengobatan yang didapat pasien. Namun demikian, hubungan antara bersihan laktat terhadap terjadinya kematian pasien sepsis berat belum diketahui. Berdasarkan hal tersebut, penelitian ini dilakukan untuk mengetahui pengaruh bersihan laktat terhadap kesintasan pasien dengan sepsis berat, serta faktor-faktor perancu yang mempengaruhi hubungan tersebut.Metode. Penelitian ini merupakan penelitian kohort prospektif yang dilaksanakan di Unit Gawat Darurat dan ruang perawatan Rumah Sakit dr. Cipto Mangunkusumo (RSCM) pada bulan Maret-Mei 2011. Pasien termasuk dalam kelompok bersihan laktat tinggi bila terdapat perbedaan kadar laktat ≥10% dalam 6 jam pertama pengobatan, sedangkan perbedaan <10% termasuk ke dalam kelompok bersihan laktat rendah. Selanjutnya, dilakukan observasi terhadap terjadinya kematian dalam 10 hari pertama perawatan pada kedua kelompok. Data yang terkumpul selanjutnya dianalisis mengunakan uji statistik log-rank test, serta dicari nilai hazard ratio dengan menggunakan uji cox regression model. Selanjutnya, dilakukan analisis variabel perancu dengan menggunakan uji cox regression.Hasil. Laju kesintasan kelompok bersihan laktat tinggi dan rendah masing-masing sebesar 60,0% dan 26,7% (p=0,004). Median kesintasan yaitu 3 hari pada bersihan laktat rendah, sedangkan kematian tidak mencapai 50% pada bersihan laktat tinggi. Interkuartil I kedua kelompok berturut-turut sebesar 1 dan 4 hari. Dari analisis didapatkan hazard ratio sebesar 2,87 (IK 95%; 1,41-5,83). Pada analisis multivariat keberadaan syok sepsis, skor SOFA, penggunaan vasopresor/inotropik, transfusi dan cairan resusitasi, tidak ada yang mengubah nilai hazard ratio >10%. Oleh karena itu, tidak ada yang memenuhi syarat untuk disebut sebagai perancu.Simpulan. Pasien bersihan laktat tinggi memiliki kesintasan lebih tinggi dibandingkan pasien bersihan laktat rendah dan hubungan tersebut tidak dipengaruhi perancu.
