El quiste odontogénico calcificante (QOC) es raro y representa <1% de todos los quistes odontogénicos, pertenece a las lesiones conocidas como “tumores de células fantasma”, estos son una familia de lesiones que varían en presentación clínica de quiste o neoplasia sólida, en su comportamiento biológico de benigno a localmente agresivo o metastásico, esta diversidad ha generado extenso debate sobre la clasificación, terminología, manejo y pronóstico de estas patologías. Presentamos el caso clínico de un paciente masculino de 17 años de edad con diagnóstico de quiste odontogénico calcificante asociado a odontoma complejo situado en la región posterior del maxilar, en la literatura existen reportados solo 27 casos de QOC en esa zona. Fue tratado mediante enucleación quirúrgica, curetaje, rotación de almohadilla grasa bucal y colocación de malla de titanio para reconstruir la pared anterior del seno maxilar. El control postoperatorio clínico y radiográfico a 12 meses no muestra recidiva de la lesión además exhibe un excelente resultado funcional y estético, esto nos permite concluir que el tratamiento fue adecuado. Realizamos una revisión de la literatura en Science Direct, PubMed y Biblioteca Cochrane, con objeto de informar las características clínicas, histopatológicas, radiográficas y el tratamiento de estas lesiones.
Introducción: los defectos del cráneo y las anomalías del hueso craneofacial que requieren reconstrucción son comunes en una variedad de procedimientos neuroquirúrgicos. Después de una craniectomía o de fracturas craneofaciales posteriores a traumatismos cráneoencefálicos, los pacientes pueden desarrollar defectos cosméticos importantes. Algunos de estos son la depresión de la piel y un defecto de hundimiento que lleva a una apariencia asimétrica de la cabeza, sin dejar de lado las repercusiones físicas, neurológicas y psicológicas que estas lesiones conllevan. La reconstrucción craneofacial y la craneoplastía tienen una larga historia, pero las nuevas técnicas quirúrgicas y una multitud de opciones de materiales han impulsado recientemente el avance en esta área. Los implantes de polimetilmetacrilato (PMMA) han demostrado ser estables, biocompatibles, no conductores, radiotransparentes y de bajo costo. Es así que se pueden colocar y modificar fácilmente, con lo que se elimina la morbilidad del sitio donante. Presentación del caso: en este artículo presentamos un caso de craneoplastía de defecto frontal, posterior a traumatismo, cuya reconstrucción fue realizada mediante una prótesis de polimetilmetacrilato (PMMA) en el Hospital General Xoco de la Cuidad de México. Introducción: el reflejo trigémino cardíaco es una respuesta fisiológica parasimpática repentina, que ocasiona alteraciones cardiovasculares importantes durante la estimulación nerviosa en el trayecto del V par craneal, generalmente ocurre al realizar procedimientos quirúrgicos en cirugía oral y maxilofacial, neurocirugía, oftalmología y dermatología. La artroscopia de la articulación temporomandibular ha sido tradicionalmente considerada un procedimiento quirúrgico seguro, mínimamente invasivo, aun así, esta técnica no está exenta de complicaciones neuro y cardiovasculares. Objetivo: presentar un caso clínico que durante una artroscopia de la articulación temporomandibular en la fase de instrumentación del espacio articular superior, y que, bajo monitoreo continuo del anestesiólogo, reportó una bradicardia súbita. Conclusión: el diagnóstico intraoperatorio de reflejo trigémino cardíaco, fue descrito por el servicio de anestesiología recibiendo un tratamiento farmacológico temprano para esta condición, lo que permitió obtener una adecuada evolución trans y postoperatoria.La artroscopia de la articulación temporomandibular ha sido tradicionalmente considerada un procedimiento quirúrgico seguro, mínimamente invasivo, aun así, esta técnica no está exenta de complicaciones.El objetivo de este artículo es presentar un caso clínico que durante una artroscopia de la articulación temporomandibular en la fase de instrumentación del espacio articular superior sufrió una bradicardia súbita, con un diagnóstico intraoperatorio de reflejo trigémino cardíaco, recibiendo un tratamiento temprano por parte del servicio de anestesiología de esta condición, obteniendo una adecuada evolución trans y postoperatoria.
