Interstitial or cornual implantation of the blastocyst is rare, accounting for 2% to 3% of ectopic pregnancies, being considered not viable. The important complications of interstitial pregnancy are uterine rupture and massive bleeding, which usually occur before 12 weeks of pregnancy. The authors report a case of a 36-year-old woman with complaints of transvaginal bleeding and abdominal pain associated with amenorrhea for seven weeks and positive betahuman chorionic gonadotropin (HCG). Transvaginal ultrasound and exploratory laparotomy were performed, confirming the diagnosis of interstitial ectopic pregnancy. The patient underwent a salpingectomy and cornual resection on the left, evolving with clinical improvement and hospital discharge.
INTRODUÇÃO: A pré-eclâmpsia (PE) é uma desordem complexa que se caracteriza por hipertensão e proteinúria após a 20ª semana de gestação, e causa a maior morbidade neonatal e mortalidade perinatal. Até agora, a patogênese exata da PE ainda não é totalmente compreendida, e intensos esforços de pesquisa estão focados na PE para elucidar os mecanismos fisiopatológicos. Estudos apontam que, fatores genéticos estão relacionados à fisiopatologia dessa síndrome. OBJETIVOS: Este estudo objetiva apresentar através de uma revisão de literatura, a relação entre o polimorfismo genético e patogênese da PE, bem como a atuação de alguns genes envolvidos no desenvolvimento da PE. MÉTODOS: Para tanto, realizou-se uma busca nas bases de dados Medline/Pubmed, Lilacs, Biblioteca virtual em saúde (BVS) e Scielo, mediante os descritores: Pré- eclâmpsia; Polimorfismo; e Fatores genéticos, bem como seus correspondentes em inglês. Foram selecionados 14 artigos para compor essa revisão, quem compõe o acervo dos últimos 11 anos. RESULTADOS: De fato, na PE existe uma infinidade de causas mal definidas, incluindo componentes genéticos, que implicam em fatores etiológicos, como má adaptação imune, isquemia placentária e aumento do estresse oxidativo. Ademais, muitos estudos confirmaram a presença de genes desregulados em pacientes grávidas com PE. Entretanto, a função e o mecanismo desses genes estão sendo revelados gradualmente. Dessa forma, uma maior compreensão dos fatores etiológicos da PE é fundamental para melhorar o tratamento e a prevenção. CONCLUSÃO: A pesquisa sobre genes de suscetibilidade à PE precisa envolver um grande número de amostras de diferentes regiões e etnias para melhor compreender o papel dos fatores genéticos na ocorrência e desenvolvimento da PE. O número crescente de pacientes diagnosticadas com PE e com a necessidade de um diagnóstico mais precoce e efetivo bem como uma intervenção apropriada, o entendimento e conhecimento da patogênese dessa alteração é de grande importância.

Objetivou-se descrever as estratégias e formas de análise, planejamento e monitoramento do Comitê de Prevenção de Mortalidade Materna, Perinatal e Infantil para a prevenção do óbito materno, infantil e fetal no município de Sobral, Ceará. Trata-se de um estudo qualitativo de sistematização da experiência vivenciada, sob a perspectiva da observação participante. O Comitê teve um total de 643 óbitos infantis investigados de 2006 a 2021, e essa avaliação dos óbitos trouxe evidências das causas e problemas no acesso e/ou na qualidade da assistência no pré-natal ou no parto à mãe e à criança, reduzindo assim a Taxa de Mortalidade Infantil. A vigilância do óbito realizada de forma sistemática e integrada aos diferentes setores permite a construção de indicadores para o monitoramento. A experiência vivenciada evidenciou as atividades organizacionais do Comitê quanto à evitabilidade dos óbitos maternos, infantis e fetais. Os dados aqui apontados corroboram a necessidade de ampliar o olhar para a atenção materno-infantil, buscando alternativas que viabilizem uma assistência cada vez mais qualificada à gestante, puérpera e criança.
