There is a waid gap between supply and demand of medical specialists at the national and regional levels, as a whole representing approximately 45% of the current offer, regardless of the estimation method.
RESUMENLa participación del inversor privado en la prestación de servicios de salud públicos, en la figura denominada "asociación público-privada" (APP) data del siglo pasado, tanto en América Latina como en Europa, en el Perú se publicó en el 2008 la normativa para APP en infraestructura, mantenimiento y prestación de servicios en general, pero es a finales del 2013 cuando se inicia a implementar APP para servicios de salud. En Colombia se dieron cuenta de que esta modalidad les resulta muy costosa; en Chile no regularon desde el inicio y hoy es difícil regular al privado; Costa Rica nunca entregó todo el poder de decisión al privado, siempre mantuvo la responsabilidad de brindar el servicio de salud a su población y mantiene la salud como un derecho. En el Perú, en esta etapa de implementación de APP para servicios de salud, no se tiene en cuenta las recomendaciones de otras experiencias como: la transparencia, participación de todos los involucrados, desarrollo de legislación específica, entre otros. ABSTRACTPrivate investor participation in the provision of public health care services (called "public-private partnership" or PPP) dates from the last century, both in Latin America and Europe. In Peru, legislation for PPPs was published in 2008 in terms of infrastructure, maintenance and service provisions in general; but it was at the end of 2013 when PPP began to be implemented for health services. In Colombia, it was realized that this model was very costly. In Chile, the private sector was not regulated from the beginning and today it is difficult to regulate. Costa Rica never gave full decisional power to private sector; the responsibility for providing health services to its population and maintaining health as a right has always been maintained. In Peru, at this stage of PPPs implementation for health services, other experiences are not taken into account such as: transparency, participation of all stakeholders, development of specific legislation, among others.
Objetivos. Caracterizar la oferta, la demanda y la brecha de médicos especialistas en establecimientos del Ministerio de Salud del Perú a nivel nacional, regional y por tipo de especialidad. Materiales y métodos. Estudio descriptivo, observacional a través del cual se calculó la oferta de médicos especialistas utilizando fuentes secundarias del MINSA. El análisis de la demanda de médicos especialistas se basó en dos metodologías: necesidades de especialistas según la norma de categorización de los establecimientos de salud y según el perfil epidemiológico y demográfico. La diferencia aritmética entre la demanda calculada y la oferta fue el procedimiento utilizado para calcular la brecha de médicos especialistas. Resultados. El MINSA, a nivel nacional, tiene una oferta total de 6074 médicos especialistas de los cuales el 61,5% pertenecen a las especialidades clínicas, 33,2% a especialidades quirúrgicas, 4,9% a especialidades de apoyo al diagnóstico y tratamiento y 0,4% a las especialidades de salud pública. Según la norma de categorización existe una demanda total de 11 176 médicos especialistas y según el perfil epidemiológico y demográfico, de 11 738. Las brechas estimadas a nivel nacional son similares en ambos métodos, aunque difieren ampliamente en las regiones y por tipo de especialidad. A nivel de región, las brechas son mayores en Loreto, Piura, Puno y Madre de Dios cuando se usa la valoración del déficit en forma relativa a la oferta. En cuanto a especialidad, la brecha es mayor en las cuatro especialidades básicas: ginecología-obstetricia, pediatría, medicina interna y cirugía general. Conclusiones. Existe una gran brecha entre la oferta y la demanda de médicos especialistas a nivel nacional y regional que, de forma agregada, representa aproximadamente el 45% de la oferta actual, independientemente del método de estimación empleado.

La promulgación de la Ley 30885 de Redes Integradas de Salud, marca un hito en el esfuerzo del país de lograr resolver las necesidades de atención de los ciudadanos en una red y es la apuesta del sector salud para mejorar el desempeño del sistema sanitario. Para ello se plantean elementos clave para la organización y funcionamiento: Identificación de población y familias, asignación de responsabilidad del cuidado, fortalecimiento de las puertas de entrada al sistema de salud, estratificación de riesgos individuales y familiares, coordinación para facilitar el acceso y utilización de servicios de salud, continuidad y complementariedad del cuidado, y gobernanza en la red. Esta nueva propuesta política implica ajustes en la red de servicios de salud: establecimientos de salud (clínicas de familia) de 12 y 24 horas para el cuidado individual, familiar, manejo de los determinantes de la salud y hospitales para complementar el cuidado. El modelo de cuidado será más complejo y de avance difícil si no está unido a ajustes en el financiamiento, aseguramiento y en la gestión del cambio cultural de los trabajadores, usuarios del sistema y del sistema de gobernanza en salud. ABSTRACTPeru's Ministry of Health focuses on achieving the fourfold goal for its health system: seeking to improve the health of the population, a better user experience when using health services, better use of public resources being invested during the last decade, and better job satisfaction for healthcare workers. The enactment of Law 30885 on Integrated Health Networks marks a milestone in the country's effort to meet the care needs of citizens in a network and is the health sector´s proposal to improve the performance of the healthcare system. To this end, key elements for organization and functioning are proposed: identification of the population and families, assignment of liability for care, strengthening of the entry gates to the health system, stratification of individual and family risks, coordination to facilitate access to and use of health services, continuity and complementarity of care, and network governance. This new proposal involves adjustments to the health service network: 12-and 24-hour health facilities (family clinics) for individual care, family care, management of health determinants, and hospitals to supplement care.
