Cada día encontramos con mayor frecuencia el término inteligencia artificial en todos los escenarios de la medicina incluyendo la neumología, siendo una necesidad el conocimiento en el tema y el acercamiento del profesional de salud al uso de estas herramientas. La aplicabilidad de la inteligencia artificial en neumología tuvo sus inicios en la interpretación de pruebas de función pulmonar y en ámbitos de diagnóstico. Sin embargo, rápidamente vemos la importancia del uso en soporte de decisiones, monitoreo y vigilancia de desenlaces en la práctica diaria y en el campo de investigación, modelos de predicción clínicos, solo para mencionar algunos. Este artículo es una revisión narrativa del papel de la inteligencia artificial en algunas de las patologías más frecuentemente vistas en nuestra práctica diaria. Describimos el uso de inteligencia artificial en el diagnóstico clínico, funcional e imagenológico de la EPOC y el asma así como en el monitoreo remoto de los pacientes, los avances en la interpretación de imágenes y de patología de las enfermedades intersticiales con el uso de Aprendizaje Automatizado (AA) y Aprendizaje Profundo (AP), la aplicación en tamización de hipertensión pulmonar a partir de pruebas diferentes al cateterismo cardiaco derecho, y finalmente la amplia gama de aplicaciones en medicina del sueño, en la que el avance es asombroso.
Background The allergic phenotype is responsible for more than 50% of severe asthma cases. In a stepwise approach, add-on treatments such as anti-IgE are used for severe allergic asthma (SAA). This study was aimed to describe the real-world effectiveness of omalizumab in adult and pediatric patients with SAA in Colombia. Methods This was an observational, non-interventional, retrospective study. Data from patients with SAA that received at least one month of treatment with omalizumab was obtained from medical records at eight sites in Colombia. Time-zero (t − 0) was defined as the date of initiation of omalizumab, and data was gathered for a 12-month period before t − 0 and a 12-month period after t − 0. Clinical outcomes, including exacerbations, were assessed at 6 and 12 months. Effectiveness of omalizumab was evaluated in terms of the reduction of the risk of exacerbations (annualized rate). Results We included 143 patients with SAA. There was a decrease of 72.4% of the annualized rate of clinically significant asthma exacerbations during the year after omalizumab (from 1.74 before to 0.48 after) with a substantial reduction of the risk of exacerbations by 56.7% (RR [95% CI] 0.43 [0.30–0.63] p < 0,001). Conclusion The use of omalizumab in Colombia as a treatment for SAA notably reduced the risk of clinically significant exacerbations. This study is the first to evaluate omalizumab real-life effectiveness in pediatric and adult patients in the country.
Introducción. El diagnóstico de la enfermedad pulmonar intersticial difusa (EPID) requiere un enfoque multidisciplinar y, en ocasiones, de una biopsia pulmonar. Objetivo. Describir las características sociodemográficas y clínicas, y los hallazgos radiológicos e histológicos de pacientes con EPID que requirieron biopsia pulmonar luego de no lograrse un diagnóstico de EPID por la junta multidisciplinar (neumología, radiología y patología) de un centro de referencia en enfermedades respiratorias en Bucaramanga, Colombia. Materiales y métodos. Estudio transversal. Se revisaron las historias clínicas de 56 pacientes atendidos en el Instituto Neumológico del Oriente y que fueron remitidos a biopsia pulmonar entre 2015 y 2019. Se analizaron variables demográficas y clínicas, calculando medidas de tendencia central y de dispersión para su respectiva caracterización. Se realizó un análisis bivariado mediante test exacto de Fisher para determinar si existían diferencias en la distribución de las variables sociodemográficas y clínicas de acuerdo con los patrones radiológicos y el diagnóstico histológico definitivo. Resultados. La mediana de edad fue 67 años (RIC: 59-72), 55.4% fueron hombres. 43 pacientes presentaron patrón radiológico inconsistente con neumonía intersticial usual (NIU); 4, patrón de posible NIU y, 9, patrón de NIU. Los diagnósticos histológicos más frecuentes fueron neumonitis por hipersensibilidad (NH) (32%), neumonía intersticial no específica (NINE) (17.8%) y NIU (19.6%). Conclusión. La principal razón para realizar biopsia pulmonar en la población de estudio fue la presencia de un patrón radiológico inconsistente con NIU, siendo NH el principal diagnóstico histopatológico. Este es el primer trabajo que caracteriza a pacientes con EPID del oriente colombiano llevados a biopsia pulmonar, lo que representa un importante aporte al conocimiento de la epidemiología de esta enfermedad en Colombia.
Introducción: La fibrosis pulmonar idiopática (FPI) es una enfermedad pulmonar intersticial (EPID) de mal pronóstico, considerada huérfana en Colombia. Un diagnóstico correcto tiene implicaciones para el paciente y los costos de atención. Los grupos de discusión multidisciplinaria (GDM) se consideran el estándar de oro en el diagnóstico. No hay estudios previos en Colombia de la experiencia de un GDM. Objetivos: Evaluar el impacto de un GDM en una institución de 4° nivel en Bogotá en cambio de diagnóstico de pacientes con EPID y la concordancia entre el diagnóstico inicial y el diagnóstico final de FPI. Materiales y métodos: Pacientes con EPID evaluados entre 2015-2018 por el GDM conformado por neumólogos, radiólogo, patólogo y reumatólogos. Criterios ATS/ERS/JRS/ALAT de diagnóstico de FPI. Descripción del cambio en el diagnóstico y concordancia entre el diagnóstico inicial y del GDM en FPI. Resultados: De 165 pacientes con EPID se cambió el diagnóstico en 35,2%. En 77,3% con diagnóstico inicial de FPI y en 6,7% con diagnóstico inicial diferente a FPI se confirmó FPI por el GDM. Al descartar FPI, los principales diagnósticos fueron neumonitis de hipersensibilidad en fase crónica (29,4%) y neumonía intersticial no específica (23,5%). El índice kappa entre el diagnóstico inicial y final de FPI fue 0,71 (0,60-0,82). Conclusiones: El GDM en EPID tuvo un importante impacto clínico demostrado por un alto porcentaje de cambió del diagnóstico de remisión. Se descartó el diagnóstico inicial de FPI en un porcentaje significativo de pacientes y se ratificó en un grupo menor sin esta sospecha clínica inicial

El síndrome de apnea obstructiva del sueño es un trastorno caracterizado por eventos repetitivos de obstrucción total o parcial de la faringe que se presentan al dormir. Es un problema de salud pública por las complicaciones que genera y por la gran cantidad de personas a las que afecta. La polisomnografía como estándar de referencia para el diagnóstico es costosa, poco disponible y no ha resuelto la necesidad de un diagnóstico masivo para el síndrome de apnea obstructiva del sueño. Por este motivo, la poligrafía respiratoria ha surgido como una alternativa diagnóstica útil, válida y accesible. Existen diferentes tipos de polígrafos, pero los más comunes son aquéllos que definen los eventos respiratorios usando señales de flujo respiratorio, esfuerzo y pulsioximetría. El objetivo de este documento es describir las reglas de calificación manual para polígrafos respiratorios basados en la señal de flujo y/o esfuerzo (que no incorporan evaluación de estadíos de sueño ni alertamientos) para el diagnóstico del síndrome de apnea obstructiva del sueño en adultos, de acuerdo con la versión 2.
