Les débats de société envahissants dans les suites de «l’affaire Weinstein» ont permis de réinterroger les places et positions des hommes et femmes dans la société. Si certains en ont pointé un changement radical pouvant faire basculer les rapports entre les sexes, il pourrait s’agir de réactualisations de la capacité à pouvoir naviguer de façon fluide entre le masculin et le féminin psychique, c’est-à-dire composer avec la bisexualité psychique pour pouvoir assumer son propre sexe. L’émergence de troubles psychiatriques, tout particulièrement à l’adolescence, connaît des déterminants sociaux, qui dans le cas de l’anorexie mentale masculine se particularisent par l’entrave à cette navigation interne, ayant pour résultats des conflits psychiques touchant aux identifications sexuelles. Chez le garçon souffrant d’anorexie mentale, l’homosexualité est plus fréquente que dans la population générale. Nous avons souhaité dans cet article mettre en lumière les rapports entre l’anorexie mentale masculine et l’homosexualité, à partir des dialogues internes qui président à l’aménagement de la bisexualité psychique. Pour cela, nous avons pris pour étude le film Blood Brothers, de Robert Mulligan sorti en 1987, qui sur fond d’anorexie cherche à montrer les difficultés adolescentes à intégrer ces différentes composantes.
Au décours du confinement de mars 2020 lié à la Covid -19, nous avons pu observer une patientèle composée de jeunes souffrant de troubles des conduites alimentaires ayant décompensé pendant la période suspendue du confinement. Nous avons souhaité, après avoir décrit la présentation clinique de ces troubles qui nous a semblé singulière, nous interroger sur les mécanismes psychopathologiques ayant pu être impliqués dans ces décompensations inédites.
Les progrès de la science offrent aujourd’hui la possibilité de changer de sexe. Les souffrances liées au genre sont aujourd’hui reconnues sous le terme de dysphorie de genre et les récentes études montrent l’intérêt d’une prise en charge précoce et multidisciplinaire des adolescents en souffrant, soulignant ainsi l’importance de l’écoute nécessaire à fournir par les psychiatres. Nous avons souhaité dans cet article reprendre les dernières recommandations internationales des standards de soins de la World Professional Association for Transgender Health (WPATH) datant de 2011 afin de guider nos réflexions quant aux questions posées par les jeunes que nous accueillons en consultation concernant la sensation de ne pas appartenir au bon genre. L’étude aura pour but de faire entrer en résonnance et en questions nos positionnements, notamment identito-identificatoires, en tant que soignant exerçant dans un cadre de consultation non spécialisée pour dysphorie de genre, avec ceux des jeunes et de leurs familles dans ces cas singuliers.
