Between March 1994 and July 2001, 178 patients underwent 248 functional neurosurgical procedures performed by the same team at the University of California at Los Angeles. The procedures included pallidotomy (122 patients), thalamotomy (19 patients), and implantation of deep brain stimulation electrodes in the subthalamic nucleus (36 patients), globus pallidus internus (17 patients), and ventralis intermedius nucleus (54 patients). One hundred forty-four procedures involved macroelectrode stimulation and 104 involved MER. Groups were analyzed according to the presence of arterial hypertension, MER or macroelectrode stimulation use, and occurrence of hemorrhage. Nineteen patients with arterial hypertension underwent 28 surgical procedures. Five cases of hemorrhage (2.02%) occurred. One patient presented with hemiparesis and dysphasia but no surgery was required. The incidence of hemorrhage in patients in whom MER was performed was 2.9%, whereas the incidence in patients in whom MER was not used was 1.4% (p = 0.6529). Bleeding occurred in 10.71% of patients with hypertension and 0.91% of those who were nonhypertensive (p = 0.0111). Among the 104 patients in whom MER was performed, 12 had hypertension. Bleeding occurred in two (16.67%) of these 12 patients. An increased incidence of bleeding in hypertensive patients who underwent MER (p = 0.034) was noticed when compared with nonhypertensive patients who underwent MER. A higher number of electrode passes through the parenchyma was observed when MER was used (p = 0.0001). A positive trend between the occurrence of hemorrhage and multiple passes was noticed. Conclusions. Based on the data the authors suggest that a higher incidence of hemorrhage occurs in hypertensive patients, and a higher incidence as well in hypertensive patients who underwent MER rather than macroeletrode stimulation. Special attention should be given to MER use in hypertensive patients and particular attention should be made to multiple passes.
Dedicated linear accelerator-based stereotactic radiosurgery that uses a 5-mm collimator to deliver 90 Gy to the nerve root entry zone is a safe and effective method for the treatment of essential trigeminal neuralgia. Care should be taken to limit brainstem volume included in the 50% isodose line in the treatment plan to avoid facial numbness.
This is the first study in which techniques of hypothalamic DBS in humans have been applied in freely moving nonhuman primates. Future studies can now be conducted to determine whether VMH DBS can change hypothalamic responsivity to endocrine signals associated with adiposity for long-term modulation of food intake.
ObjectThe authors report the evolution of linear accelerator (LINAC)–based radiosurgery in the treatment of patients with intracranial meningiomas. They describe the technical aspects as well as clinical and radiological outcomes.MethodsThe authors performed a retrospective review of 161 patients harboring 194 intracranial meningiomas treated with various types of stereotactic irradiation at their institution between May 1991 and July 2002. Clinical and radiological follow-up data (mean follow-up period 32.5 months, range 6–125 months) were obtained in 128 patients (79.5%) with 156 meningiomas (80.4%). There were 88 women and 40 men whose mean age was 57.2 years (range 18–87 years). Stereotactic irradiation was the primary treatment in 44 patients, and 84 patients underwent resection prior to radiosurgery. Stereotactic radiosurgery (SRS) was used to treat 79 lesions and fractionated stereotactic radiotherapy (SRT) was used to treat 77. The mean dose for SRS was 1567 cGy (range 1200–2285 cGy) prescribed to a mean isodose line of 66.6% (range 50–90%). Stereotactic radiotherapy was delivered using a mean dose of 4839 cGy (range 2380–5400 cGy), prescribed to a mean isodose line of 89% (range 50–90%).The mean follow-up periods were 40 and 24 months in SRS- and SRT-treated patients, respectively. Tumor control was achieved in 58 SRT-treated benign meningiomas (90%) and in 70 SRT-treated lesions (97.2%). In patients with atypical meningiomas a considerably poorer prognosis was seen. Clinical improvement or stabilization of symptoms was observed in the majority of patients. Symptomatic complications were limited to four patients (5%) treated with SRS and four (5.2%) treated with SRT.ConclusionsStereotactic irradiation techniques have changed the neurosurgical approach to intracranial meningiomas. Either SRS or SRT delivered as a primary treatment in selected cases of skull base lesions or as an adjuvant after conservative resection has improved the management of these complex intracranial tumors.
Lymphocytic hypophysitis is a rare inflammatory disorder of the pituitary gland. Standard therapy consists of transsphenoidal resection or oral administration of corticosteroid medications. Two patients with symptomatic lymphocytic hypophysitis, which recurred after standard therapy, were treated with low-dose stereotactic radiotherapy. On imaging studies both lesions demonstrated a response to radiation and each patient experienced relief of symptoms. There has been no adverse sequela of the radiation treatment. The authors conclude that stereotactic radiotherapy represents an effective, noninvasive treatment option for patients with lymphocytic hypophysitis, particularly if the disease is recurrent after surgery or resistant to corticosteroid medications.
