AIM: To determine whether Houttuynia cordata Thunb (HCT) can increase the survival of the retinal ganglion cells (RGCs) and inhibit microglia activation following retinal ischemia-reperfusion (RIR) injury. METHODS: Rat model of RIR was induced by transient elevation of the intraocular pressure (IOP). HCT was orally administered for 2d before the performance of retinal RIR model and once a day for the next 14d. After 14d of RIR injury, the rats were sacrificed for further analysis. Survival RGCs were stained with haematoxylin and eosin (H&E). Apoptosis of RGCs was detected by TUNEL staining. Retinal function was examined by flash-electroretinography (F-ERG). Retinal microglia were labeled using Iba-1, one specific marker for microglia. The mRNA expression levels of inducible nitric oxide synthase (iNOS), tumor necrosis factor alpha (TNF-α), and interleukin 1 beta (IL-1β) were assessed by quantitative real time reverse transcription polymerase chain reaction (qRT-PCR). RESULTS: Systemic HCT treatment significantly reduced RGCs death by H&E staining and exhibited an anti-apoptotic effect as assessed by TUNEL staining at day 14 after RIR injury. HCT greatly improved the retinal function as examined by F-ERG. The number of activated microglia significantly increased after RIR injury, which was significantly attenuated by HCT treatment. Besides, RIR injury induced a strong upregulation of pro-inflammatory genes TNFα, iNOS and IL-1β mRNAs at day 14 post injury, which was suppressed by HCT. CONCLUSION: Neuroprotective effects of HCT encourage the survival of RGCs through inhibiting microglia activation due to RIR injury. Together these results support the use of HCT as promising therapy for the ischemic events of the retina diseases.
Objectives To evaluate skeletal muscle mass in patients with both type 2 diabetes mellitus (T2DM) and concomitant lower extremity arterial disease (LEAD) and determine the contribution of skeletal muscle mass to macrovascular diseases. Methods In total, 112 patients with T2DM were divided into the T2DM and T2DM + LEAD groups. Hepatic function, renal function, uric acid, blood glucose, and glycated hemoglobin (HbA1C) were measured. Dual-energy X-ray absorptiometry was used to measure visceral fat area and skeletal muscle mass index (SMI). Results Waist-to-hip ratio, uric acid, and body fat percentage were significantly higher in the T2DM+LEAD group than in the T2DM group; SMI was significantly lower in the T2DM+LEAD group than in the T2DM group. There were no significant differences in albumin, creatinine, fasting blood glucose, HbA1C, or blood lipids. Uric acid, SMI, and body fat percentage were significantly positively correlated with T2DM and concomitant LEAD. Logistic regression analyses suggested that SMI is an independent risk factor for LEAD in T2DM (odds ratio = 1.517; 95% confidence interval: 1.082–2.126). Conclusions Skeletal muscle mass is lower in patients with T2DM and concomitant LEAD than in patients with T2DM who do not exhibit LEAD. SMI is an important risk factor for LEAD.
The Chinese government has issued the policy of promulgating the clinical use of antibacterial drugs since 2011. Prophylactic antibiotic use is a challenging problem among young children with intussusception after successful air enema reduction. There were limited data regarding the clinical value of prophylactic antibiotics for intussusception with low-risk infections. A retrospective non-randomized comparative study was conducted among 188 young children with intussusception after successful air enema reduction between January 1, 2011 and December 30, 2013. Among these children, 51 received prophylactic antibiotics and 137 did not receive antibiotics. Our results showed that there were no significant differences in age, gender, weight, admission period, reduction interval, axillary temperature, leukocytes, neutrophils, lymphocytes, monocytes, mesenteric lymph nodes and complications between groups (P > 0.05). The national policy had significantly improved clinical use of antibiotic for young children with low-risk intussusception (OR < 0.001, P < 0.001). Inpatients days were longer for children used antibiotics than those who did not (median, 27.0 hours vs 21.0 hours, P = 0.003). Prophylactic antibiotics appeared to be of little value after the successful air enema reduction of intussusception in young children with low-risk infection. Policy intervention is effective for antibiotic use in China.
