El número de amputados en el mundo, incluida Colombia, es de más de diez millones de personas, siendo la amputación por debajo de rodilla, o amputación transtibial, bastante recurrente. Debido a los cambios fisiológicos que se presentan en los amputados transtibiales, la mecánica de la marcha se ve alterada y puede llegar a afectar significativamente la movilidad del sujeto. Con el fin de conocer el comportamiento dinámico de los amputados transtibiales, y generar conocimiento para entender la conducta biomecánica de ellos y así ayudar en el proceso de rehabilitación física de estos sujetos, se propuso estudiar la marcha protésica desde simulaciones generadas de un modelo computacional. Para ello se construyó un modelo de la marcha humana haciendo uso del intercambio de energía que se presenta en este movimiento, empleando el análisis de Lagrange. También se obtuvo el modelado matemático (diagrama de bloques) de una prótesis transtibial activa a partir del diseño en 3D realizado en el software Solidworks®; este modelo se procesó en Matlab® con el fin de emular el uso de la prótesis. En el modelo de la marcha normal obtenido se reemplazó una de las piernas por el modelo de la prótesis para observar la marcha protésica. Los resultados del modelo tanto en valores de los rangos articulares como en parámetros espacio-temporales se encuentran acordes con lo observado en otros estudios. El análisis cinemático del comportamiento del modelo muestra que la prótesis proporciona soporte al cuerpo y permite la marcha en ausencia del segmento corporal, ya que los datos arrojados por el modelo del patrón de la marcha obtenido concuerdan con los datos existentes en literatura. Esta propuesta de simulación es una ayuda para asegurar el aprendizaje del estudiante y del médico respecto al comportamiento biomecánico de la marcha protésica, puesto que en los últimos años se ha visto y comprobado la utilidad del uso generalizado de simulaciones en el ámbito médico.
La falta de información objetiva en la alineación de prótesis es un fenómeno recurrente en el área de prótesis. Al presentarse una inadecuada ubicación angular del encaje de una prótesis transtibial los parámetros biomecánicos de rangos articulares, distribución de fuerza en los pies y ubicación del Centro de Presión (COP) en los amputados se ven afectados, además influye en la comodidad del amputado en posición bipedestada erguida. En la presente tesis, los efectos de la mala alineación del encaje de la prótesis transtibial sobre parámetros biomecánicos se midieron en términos de ángulos de las extremidades inferiores, distribución de fuerza sobre los pies y ubicación del COP cuando el sujeto se encuentra en posición bipedestada estática. Adicionalmente, se crearon modelos predictivos estocásticos, basados en conocimiento, de la alineación estática de prótesis. Las perturbaciones de angulación del encaje en el plano sagital se introdujeron desde un punto base denominado alineación, establecido por un protesista experimentado. Se utilizaron goniómetros digitales para medir los rangos articulares de las extremidades inferiores y la variación en la ubicación angular del encaje tanto en flexión como en extensión (2°, 4° y 6°). Un sistema de medición de fuerza plantar permitió detectar y almacenar los cambios en la distribución de fuerza sobre los pies y la ubicación del COP. En la investigación participaron siete sujetos amputados transtibiales usuarios de prótesis. Se midieron siete condiciones de alineación del encaje en cada uno de 6 sujetos, al sujeto 7 se le ubicaron seis condiciones, para un total de 144 pruebas. Se registraron en archivos de computador el ángulo del encaje, los ángulos de los rangos articulares, el peso soportado por cada pie y el COP. Los datos de los registros permitieron generar una base de datos de alineación estática donde se involucra ubicación angular en el plano sagital del encaje de la prótesis, rangos articulares de los miembros inferiores, distribución de peso bajo cada pie y la ubicación del COP. Luego se desarrolló un análisis estadístico de las mediciones, en primera instancia un análisis gráfico realizado con diagramas de caja y en segunda instancia la prueba estadística de Levene, ambas pruebas mostraron la existencia de variabilidad intra e inter-sujetos, por esto se planteó una metodología para la obtención del modelo de la alineación estática. El análisis de los datos y la construcción del modelo se hicieron tomando como base la teoría de la información. La prueba de Kolmogorov-Smirnov (KS) aplicada a los datos de entrenamiento mostró que el desempeño de los modelos fue superior al 94%, en los datos de prueba el rendimiento fue superior al 93% y en la validación al 84%. La otra prueba de rendimiento aplicada fue la divergencia de Kullback-Leibler (KL), usada como medida cuantitativa de discriminación entre dos poblaciones, allí la longitud de la distancia es 0.36 bits en la variable Tobillo Amputado, para todos los demás casos es 0 bits. El resultado arrojado por estas dos pruebas evidencia que los datos se ajustan correctamente al modelo desarrollado.
This paper describes the importance of developing learning tools, such as virtual laboratories in different areas of science with emphasis in chemistry, through the development of a virtual laboratory that allows students to access to an environment of experiential learning. It has two main stages which are: a virtual world (software) where the student is guided by an integrated environment with the features necessary in order to perform the practices and at the same time, has a capture stage (hardware) that is responsible for making the process of experimentation as real as possible, offering the instrument's sensation or machine's sensation during the experiments. Throughout the document the steps for the completion of the virtual laboratory are seen with the results so far.
In Colombia, amputations are not only due to cardiovascular or traumatic causes. According to the Presidential Program for Action against Antipersonnel Mines, amputations inflicted by this type of mines increased between 1990 and 2020 due to the predominance of the armed conflict (mainly in the lower members). The most used method for the amputee is the prosthesis, however, the prosthetic adaptation requires processes and procedures that go according to each individual, existing one generalized, the alignment of the prosthesis. Alignment is the key element of an optimal prosthetic function. The relationship between the prosthesis, the patient, and the components of the prosthesis, critically affects the functional performance and comfort. A new static alignment method for transtibial prostheses is suggested using computational models. An application was made in MATLAB®environment, making use of Neural Networks of Generalized Regression (GRNN), which allows predicting biomechanical data from the alignment of the prosthesis socket. A graphical user interface was developed to show the functioning of the algorithm, this was validated with recorded data of amputees, which allowed to evaluate the repeatability and accuracy of the results obtained. This application seeks to build a support system for the prosthetics technician to improve the rehabilitation process of a person using transtibial prosthesis.
