В когорте живых пациентов преобладают дети (74,7 %). Соотношение пациентов мужского и женского пола составляет 1:2,03. В регистре первичных иммунодефицитов СК верифицировано 27 нозологических форм. молекулярно-генетический диагноз подтвержден у 38,6 % больных. трансплантация гемопоэтических клеток осуществлена у 11,4 % больных. Препараты внутривенных иммуноглобулинов получают 20,2 % пациентов. Летальность в когорте больных ПиДС в регистре СК достигает 6,9 %. The prevalence of primary immunodeficiency in the ST is 3.1:100 000 population. The cohort of living patients is dominated by children (74.7 %). The ratio of male to female patients is 1:2.03. 27 nosological forms were verified in the register of primary immunodeficiency of the ST. Molecular genetic diagnosis was confirmed in 38.6 % of patients. Hematopoietic cell transplantation was performed in 11.4 % of patients. Intravenous immunoglobulin preparations are received by 20.2 % of patients. Mortality in the cohort of PID patients in the ST register reaches 6.9 %.
According to the data from Russian primary immunodeficiencies (PID) registry 71% of registered patients were treated with immunoglobulins (IG). Regular immunoglobulin substitutions were reported in 90% of patients with primary antibody deficiencies (PAD), 86% -with syndromic PID and 91% of patients with combined T and B cell defects. В 11,6 % случаев у пациентов с клиническими и патоморфологическими признаками врожденных иммунодефицитов установлено значительное снижение TREC и/или KREC до недетектируемых значений. Выявлено, что снижение уровня TREC наблюдается у недоношенных детей со сроками гестации менее 28 недель, а также в группе детей с врожденными пороками сердца и магистральных сосудов, желудочно-кишечного тракта, почек и мочевыделительных путей. Определение TREC и KREC является эффективным инструментом для выявления врожденных иммунодефицитов и других т-клеточных лимфопений, требующих профилактических мероприятий для предотвращения серьезных инфекций.
Ключевые слова: врожденные иммунодефициты, вторичные Т-клеточные лимфопении, TREC, KRECThe paper presents the results of retrospective determination of markers of T-cell (TREC) and B-cell (KREC) immunodeficiency in 49 children who died in the first year of life due to severe infections. In 11.6 % of cases, patients with clinical and pathological signs of congenital immunodeficiency showed a significant decrease in TREC and/or KREC to undetectable values. It was revealed that the decrease in the level of TREC is observed in premature infants with gestation less than 28 weeks, as well as in the group of children with congenital defects of the heart and main vessels, gastrointestinal tract, kidneys and urinary tract. A review of the data shows that the determination of TREC and KREC is an effective tool for the detection of congenital immunodeficiency and other T-cell lymphopenia requiring preventive measures to prevent serious infections.
The study analyzed 88 patients with congenital immunodeficiencies registered in the Stavropol Region (SR) during a 10year period (2009)(2010)(2011)(2012)(2013)(2014)(2015)(2016)(2017)(2018)(2019). The results established that the prevalence of primary immunodeficiencies (PID) in the SR was 3.1 per 100,000 population. Children affected by PID predominated (74.7 %) in the cohort of living patients. Twenty-seven nosological forms of PID were verified in the SR register. The most common was defective humoral component of the immune system (44.3 %), followed by combined immune disorders with syndromic manifestations (15.9 %) and phagocytosis defects (10.2 %). The molecular genetic diagnosis was confirmed in 38.6 % of patients. Hematopoietic cell transplantation was performed in 11.4 % of patients. Intravenous immunoglobulin medications were administered in 20.2 % of patients. Mortality in the PID cohort within the SR register was 6.9 %.
