ZusammenfassungFragestellung: Die Säuglingssterblichkeit ist ein Parameter zur Beurteilung der Qualität der medizinischen Betreuung. Die Gründe für die Säuglingssterblichkeit sind ungenügend charakterisiert. Obduktionsstudien bei nahezu vollständiger Obduktionsrate (OR) liefern zu dieser Fragestellung wichtige Ergebnisse. Material und Methodik: Retrospektiv wurden die Obduktionsprotokolle von 454 konsekutiven Lebendgeburten, die bis zur Vollendung des 1. Lebensjahres verstarben, zur Erfassung der mittel-oder unmittelbar zum Tode führenden Grunderkrankung analysiert. In diesem Zeitraum wurden die Verstorbenen unter Verwendung eines standardisierten Protokolls obduziert. Die Fälle wurden nach der Frühsterblichkeit (£ 7. Lebenstag; LT), Spät-(8. ± 28. LT) und Nachsterblichkeit (> 28. LT) geordnet. Kinder mit einem Geburtsgewicht von < 2500 g galten als untergewichtig und Geborene bis zur vollendeten 36. SSW als präterm. Die in der ehemaligen DDR gesetzlich geregelte Obduktionspflicht von Kindern bis zur Vollendung des 16. Lebensjahres ermöglichte eine nahezu vollständige Obduktionsrate aller am Perinatalzentrum der Universität Leipzig verstorbenen Kinder. Ergebnisse: Unter den Frühsterbefällen (n = 201) überwogen die unreifeassoziierten Erkrankungen (47,3 %), gefolgt von den Fehlbildungen (FB) mit 18,9%. Bei den Spätsterbefällen (n = 91) standen mit 41,8% die FB im Vordergrund, gefolgt von den unreifebedingten Erkrankungen (34,1 %) und den Infektionen (20,9 %). ¾hnliches galt für die Nachsterbefälle (n = 162) mit 60,5 % FB, AbstractObjective: The perinatal mortality rate is a parameter of the quality of medical care. The causes of perinatal deaths are not well established because autopsy is often not performed. We analyzed causes of perinatal mortality based on autopsy reports at a perinatal center with a consistent autopsy policy. Methods: We reviewed the autopsy records of 454 live-borne infants who died before 1 year of age. Standardized autopsy was performed for all children as required by law in the former East Germany. Infants weighting < 2500 g were classified as low birth weight, those born before the end of 36 weeks gestation were classified as premature. Results: Among the 201 infants who died within 7 days of birth the most common cause of death was complications of immaturity (respiratory distress syndrome, intracranial hemorrhage; 47.3%), followed by congenital malformations and infectious diseases (both 18.9 %). Among 91 infants who died up to 28 days after birth, the most common cause of death was congenital malformations (41.8 %) followed by complications of prematurity (34.1%) and infectious diseases (20.9%). The most common causes of death in the 162 infants older than 29 days were malformation (60.5 %), infections (29.6 %), and prematurity (6.2 %). Malformations most commonly involved the cardiovascular system (36.1%). Neonatal death (up to 28 days after birth; n = 292) was mainly caused by complications of immaturity (43.1 %) followed by malformation (26%), and infections (19.5 %).
Fragestellung: In Deutschland beginnt die gesetzliche Vorsorge des Zervixkarzinoms ab dem 20. Lebensjahr. Ziel dieser Untersuchung war, herauszufinden, ob dies ausreichend ist oder ob der Screeningbeginn anhand anderer Kriterien vorgezogen werden sollte. Material und Methodik: Zwischen 7/2004 und 2/ 2009 wurden Überweisungsgrund, Alter und Kohabitarche aller Patientinnen, die sich bis zum 20. Lebensjahr in unserer Dysplasiesprechstunde vorstellten, erhoben. Ergebnisse: Insgesamt wurden 117 Patientinnen im Alter zwischen 12 und 20 Jahren (durchschnittlich 18,3 Jahre) rekrutiert. Die mittlere Kohabitarche wurde mit 14,9 Jahren angegeben, also 41 Monate vor Erstvorstellung bei uns. Ein Viertel der Frauen (28/117; 24 %) wurde wegen Vulvakondylomen überwiesen, die meisten Frauen (81/117; 69 %) wegen eines auffälligen Zervixbefundes. Von letzteren wiesen wir bei 56 Patientinnen histologisch eine LSIL (low grade squamous intraepithelial lesion), bei 6 Patientinnen eine HSIL (high grade squamous intraepithelial lesion) nach. Fünf Patientinnen erhielten bei einer ektozervikalen CIN II eine Lasertherapie, eine Konisation wurde bei CIN III durchgeführt. Bei den verbliebenen Patientinnen (n = 19) wurde keine Pathologie nachgewiesen. Schlussfolgerungen: Diese Daten verdeutlichen, dass 3 Jahre nach Kohabitarche die ersten zytologischen Auffälligkeiten auftreten. Nur wenige Patientinnen zeigen zu diesem Zeitpunkt jedoch bereits eine therapiebedürftige HSIL. Die Ergebnisse legen nahe, dass der Beginn des zytologischen Screenings beim Zervixkarzinom nicht vom Alter der Patientin, sondern von der Kohabitarche abhängig gemacht werden sollte. In der Praxis könnte dies durch eine Kopplung an die Kontrazeptionsverschreibung erreicht werden, empfehlenswert wäre die 1. zytologische Kontrolle 3 JahAbstract ! Purpose:The German screening program for cervical cancer starts at age 20. The goal of this study was to examine whether this is sufficient or whether screening should already be performed earlier, based on other criteria. Materials and Methods: From 7/2004-2/2009 we evaluated the reason for referral, the age and cohabitarche of all patients younger than 20 years referred to our Dysplasia Clinic. Results: 117 patients between 12-20 years (mean 18.3 years) were included. The mean age of cohabitarche was 14.9 years, 41 months prior to initial referral to our department. Twenty-four percent (28/117) were referred for vulvar condylomata. Most women (81/117; 69 %) were referred because of suspicious cervical findings. In the latter group histological examination showed LSIL (low grade squamous intraepithelial lesion) in 56 patients and HSIL (high grade squamous intraepithelial lesion) in 6 patients. Five patients received laser therapy for ectocervical CIN II, one patient had conization due to CIN III. The remaining patients (n = 19) showed no pathologic findings.
