Cette étude propose de montrer le bénéfice potentiel du recours au matériel projectif dans la relation de l'enfant atteint d'un cancer avec le psychologue. À travers l'exposé d'une situation clinique, les auteurs présentent l'intérêt des outils de médiation proposés par le clinicien ou l'enfant dans la compréhension du fonctionnement psychique du patient. Ils analysent aussi l'impact de la prise en charge psychologique sur la dynamique psychoaffective et s'interrogent enfin sur la place et le rôle du psychologue dans un service d'onco-pédiatrie.Abstract: This study proposes to show the potential benefit of recourse to projective material in the relationship between a child suffering from cancer and a psychologist.Through the presentation of a clinical situation, the authors show how tools for mediation proposed by the clinician or the child can be useful in understanding the patient's psychic functioning. They also analyze the impact of psychological follow-up on the psycho-affective dynamic and finally question the place and the role of the psychologist in a pediatric oncology department.
Ré sumé : Les services d'oncologie sont tous concerné s par la difficulté de continuer à faire alliance avec le patient au moment où la position du malade n'est plus claire en regard des choix qui lui sont proposé s, au moment où on se demande à qui appartient vraiment le choix thé rapeutique qui vient d'ê tre pris. C'est invariablement le sens que donne le patient à sa maladie et à ses traitements qui vient soutenir son dé sir de se battre ou de ne plus se battre. The patient's relationship with his disease and its treatment invariably influences his will to fight or not. Differentiating between what the patient is consciously thinking and what his subconscious is saying is an important and essential area of theorisation with respect to treating cancer patients. É thique mé dicale face aux demandes des patients en oncologieConcernant les demandes d'entré e en é tude thé rapeutique par des patients en é chec thé rapeutique, les critè res d'entré e dans ces protocoles dé terminent grandement, pour le mé decin, la ré ponse apporté e à ces demandes.La problé matique é thique et dé ontologique se situe davantage face aux demandes de refus.Quelle que soit l'é tendue de la ré flexion mé dicale qui fait suite à un refus thé rapeutique, il convient de rappeler d'emblé e le postulat suivant : on ne peut pas contraindre un patient à recevoir un traitement.Au moment de l'offre thé rapeutique, le mé decin connaît les effets secondaires du traitement alors que le malade, lui, les dé couvre. Ces derniers peuvent constituer la cause de son refus. Le mé decin doit alors se situer dans une position de respect, mais un respect ouvert vers un besoin ulté rieur de complé -ment d'information et de discussion, voire à un é ventuel retour sur la dé cision.Respecter un refus de traitement engage une obligation d'accompagnement. En pratique, le refus d'un traitement spé cifique de la part d'un patient enclenche d'autres devoirs que ce traitement lui-mê me de la part du mé decin. En particulier, l'orientation vers les soins de confort, les prises en charge symptomatiques et le maintien d'un dialogue de confiance.Les soins de support, donné s par les autres professionnels, doivent eux aussi pouvoir ê tre poursuivis au-delà du refus de traitement, comme c'est le cas pour la prise en charge psychologique à condition toutefois qu'elle ait un sens pour le patient [12].Rappelons encore que la loi du 4 mars 2002 relative aux « d...
