Background. La contaminazione di emocolture da CoNS, in particolare S.epidermidis, rappresenta un inconveniente della fase di prelievo. La segnalazione dei probabili contaminanti cutanei (falsi positivi) potrebbe ridurre l'uso improprio degli antibiotici glicopeptidi. Obiettivi. Quantificare la riduzione del numero di positivi dopo applicazione di un algoritmo lavorativo per individuare come contaminanti certi isolati di CoNS da emocoltura; valutare la riduzione del consumo di glicopeptidi. Metodi. Le emocolture sono state lavorate mediante Bactec 9240. L'algoritmo, applicato da giugno 2002, prevede che 1) le singole emocolture positive per CoNS siano refertate come Contaminate; 2) che le emocolture positive da VP e CVC per CoNS ( t crescita +120') siano lavorate solo se prelevate contemporaneamente; in caso contrario, sono identificate come Contaminate; 3) che i CoNS positivi, da isolati multipli, siano validati solo se l'identificazione e antibiotipo sono uguali o l'antibiotipo è diverso per non più di due antibiotici; tale procedura si applica anche per gli isolati di CoNS da multiple emocolture VP positive. Il programma epidemiologico (Italab c/s) calcola come negative le colture in cui i CoNS sono associati alla sigla Contaminante. La valutazione dei campioni ha riguardato il periodo [2001][2002][2003]. I consumi dei glicopeptidi (teicoplanina e vancomicina) sono stati espressi in DDD. Risultati. Le emocolture positive, sul totale, sono diminuite da 26% (2001) a 19% (2002; 2003). La quota di emocolture contaminate, sulle positive, è passata da 0 (2001) a 15% (2002), fino a 19% (2003). Gli isolamenti da S.epidermidis da emocolture sono significativamente diminuti (-44%) dal 2° semestre 2002. L'analisi temporale 2001-2003 degli isolati MRSE e del consumo di glicopeptidi mostra una tendenza alla progressiva riduzione. Si è osservata una significativa correlazione tra i due fenomeni (R2=0.826; P=0.016). Conclusioni. L'applicazione di un semplice algoritmo di lavoro sembra aver contribuito alla diminuzione degli isolamenti significativi di CoNs, in particolare di MRSE, con una parallela diminuzione del consumo dei glicopeptidi.
