Se presenta el caso de una enfermedad rara, multisistémica intersticial, que tiene trofismo pulmonar. La condición asociada a CET es menos frecuente, pero los datos del examen físico y los hallazgos en las imágenes del tórax son cruciales en el abordaje diagnóstico. MATERIAL Y MEDOTOS Realización de historia clínica, consentimiento informado, toma de fotografías y descripción del caso clínico RESULTADOS Mujer de 49 años de edad, con antecedente de Hipertensión Arterial, epilepsia y exposición a humo de leña por 40 años, quien en enero de 2018, tuvo cuadro clínico de disnea de esfuerzo y tos seca de algunos meses, acentuado antes de su ingreso hospitalario. En la TC de tórax se observó enfermedad bullosa, quistes múltiples, nódulos pulmonares y neumotórax derecho. Se le realizo biopsia pulmonar y toracostomia cerrada derecha. El reporte histopatológico fue compatible con linfangioleiomiomatosis pulmonar. En julio del mismo año, se le realizo un TC de tórax de control ante la persistencia de síntomas respiratorios demostrando similares hallazgos pulmonares, sin recurrencia del neumotórax. En su examen físico se observaron máculas hipocrómicas de forma poligonal, "en confeti", distribuidas en la región anterior de sus piernas, adicionalmente, múltiples lesiones fibromatoides en cara y manos, en relación a esclerosis tuberosa, considerándose asociación LAM-CET CONCLUSIONES Complejo esclerosis tuberosa-Linfangioleiomiomatosis, una visión más allá del pulmón
El pulmón del crack es un síndrome potencialmente letal relacionado con el consumo de cocaína o sus derivados. La identificación de la exposición puede no se fácil, ya que algunos no proporcionan información confiable, ocasionando la instauración de tratamientos innecesarios. Finalmente, en los casos agudos, el manejo de soporte bajo vigilancia clínica termina siendo la intervención apropiada y con muy buenos resultados, tal y como se pudo corroborar en nuestra paciente. RESULTADOS El pulmón de crack, es un síndrome pulmonar agudo, que se presenta después de la inhalación de crack-cocaína y se considera una complicación rara y potencialmente fatal. En nuestro paciente, las manifestaciones clínicas, paraclínicas, imagenológicas y la evidencia de hemosiderofagos en la citología del lavado bronco alveolar, fue importante para establecer el diagnostico del pulmón de crack. CONCLUSIONES Hemorragia alveolar por consumo de cocaína.
