O índice de dose em tomografia computadoriza (CTDI) é empregado como indicador da dose de radiação recebida pelos pacientes submetidos a exames nessa modalidade de diagnóstico por imagem. Recentemente, organizações internacionais têm recomendado a adequação desse índice considerando o biótipo do paciente (estimativa de dose especificada pelo tamanho – SSDE) para fins de gerenciamento das doses. Este trabalho teve como objetivo a determinação desse novo indicador de dose baseado no tamanho do paciente (SSDE), calculando a espessura antero-posterior (AP) do paciente por meio de parâmetro geométrico (altura da mesa) extraído do cabeçalho DICOM dos arquivos de imagem do exame. Foram selecionados 101 exames de CT de abdome de pacientes adultos realizados em um equipamento Philips modelo Ingenuity CT com 64 canais, instalado no setor de radiologia de um hospital especializado de grande porte. Além do parâmetro de altura da mesa, foram extraídos do cabeçalho DICOM: data de nascimento e peso do paciente, tensão (kVp) e corrente (mA) aplicados no tubo, valor de CTDIvole localização do corte tomográfico. Os dados foram coletados por programa dedicado, desenvolvido em linguagem C. A espessura AP do paciente também foi determinada diretamente a partir da imagem correspondente a um corte tomográfico na região do abdome. Os resultados de SSDE encontrados indicam que 98% dos valores de CTDI estão subestimados. Considerando o tamanho dos pacientes analisados, a dose associada a esse indicador aumentou em ate 60%. Não houve diferença estatisticamente significante entre o valor estimado pela altura da mesa e pela espessura AP medida diretamente na imagem. Os resultados demonstram que os atuais valores de CTDI estão subestimados, sugerindo que a implementação prática dos valores de SSDE pode ser efetuada utilizando o valor da altura da mesa para uma estimativa mais realista da dose recebida pelos pacientes.
O indicador de exposição (EI) é um parâmetro numérico que fornece uma estimativa da quantidade de radiação que alcançou o detector em sistemas de imagem digitais. Este indicador na radiografia computadorizada (CR) pode auxiliar os técnicos na busca da condição ótima na relação entre dose e qualidade de imagem. Um indicador de exposição alvo (EIT) deve ser definido pela instituição para cada sistema radiográfico digital e para cada tipo de exame/projeção. O desvio do indicador de exposição praticado (EI) em relação ao valor esperado (EIT), para um exame radiológico específico, pode ser avaliado por um índice de desvio (DI). O TG-116 da AAPM estabeleceu limites e ações relacionados a valores de DI. Em 2018, o TG-232 da AAPM realizou um estudo sobre a implementação prática das recomendações do TG-116, propondo novos limites e ações, agora baseados no desvio padrão (DP) da distribuição de DI. Neste sentido, o objetivo desse trabalho foi analisar as distribuições de DI dos principais exames praticados em um setor de radiologia convencional de um hospital especializado, levando em conta os aspectos descritos no TG-232 da AAPM. No total, foram analisados 975 dados de EI e DI para tórax (PA, AP e LAT) e uretrocistografia (abdome AP), de um sistema de radiografia computadorizada (CR). O resultado das distribuições de DI dos exames mostrou que os valores de DI mais frequentes estão distantes do recomendado (DI=0), embora as imagens tenham sido consideradas clinicamente aceitáveis. Isso pode sugerir que os valores do indicador de exposição-alvo (EIT) não foram definidos dequadamente ou que os parâmetros de exposição utilizados pelos técnicos/tecnólogos devem ser revistos. Além disso, os limites de ação para o DI definidos em ±1 e ±2DP, como recomendado pelo TG-232, fornecem uma melhor interpretação das ações que devem ser implementadas no setor.
Com o aumento da expectativa de vida o número dos casos de demência tem aumentando significativamente, sendo assim, cada vez mais se faz necessário os diagnósticos dessas patologias. Por esse motivo, é importante estar atento aos exames de imagens e quando os solicitar, uma vez que, são grandes auxiliares do diagnóstico.
