A CPSS score of 3 reliably identifies LVO in AIS patients. EMS providers may be able to use the CPSS, a simple, widely adopted prehospital stroke assessment tool, with a cut-off score to screen for patients with suspected LVO.
Zusammenfassung Die neurologische Fr?hrehabilitation nach Schlaganfall ist ein wesentlicher Faktor, um den Behinderungsgrad langfristig zu senken. Je fr?her die rehabilitative Behandlung von Schlaganfallpatienten einsetzt, desto besser ist das funktionelle Outcome. Mit einem eigens entwickelten Erhebungsinstrument wurden Daten von 333 Schlaganfallpatienten in 3 Hamburger Fr?hrehabilitationseinrichtungen ?ber 20 Monate gesammelt und deskriptiv analysiert. Es ergaben sich unterschiedliche R?ckbildungstendenzen der verschiedenen Symptome, einen nicht unerheblichen Anteil an Komplikationen und eine relativ hohe Rate an weiterf?hrenden station?ren rehabilitativen Ma?nahmen. Die entwickelten Qualit?tsindikatoren decken die relevanten Aspekte der Rehabilitation mit wenigen Kennzahlen ab. Aus den erhobenen Daten l?sst sich ein Qualit?tsmanagement-System entwickeln, um die Behandlungsqualit?t weiter zu verbessern.
Zusammenfassung Aus Daten des Registers zur neurologischen Fr?hrehabilitation nach Schlaganfall der Asklepios Kliniken Hamburg wurde eine Analyse der Pr?diktoren f?r ung?nstiges Outcome durchgef?hrt. Alle F?lle (DRG: I61, 60, 63 und 64) und deren rehabilitive Behandlung erf?llten die Kriterien der OPS 8-552. Die deskriptive Statistik (Median [1. Quartil, 3. Quartil]) und ein Regressionsmodell f?r die Vorhersage eines Fr?hrehabilitation Barthel-Index (FRBI)<30 Punkte am Ende der Behandlung wurden erstellt. Von 7/2012 bis 06/2015 konnten Datens?tze von 818 Schlaganfall-Patienten (medianes Alter 72 Jahre [61, 78], 43% weiblich, h?morrhagische [28%] und isch?mische [72%] Schlaganf?lle) analysiert werden. Der mediane FRBI bei der Aufnahme lag bei ? 40 [?75,0] Punkte. Die mediane Aufenthaltsdauer betrug 34 [21, 48] Tage. Die Pr?diktoren f?r Tod oder das Erreichen von weniger als 30 Punkten im FRBI am Ende der Behandlung waren h?heres Alter, weibliches Geschlecht, Vorhofflimmern, Langzeitpflege vor Schlaganfall, neuropsychologische Defizite, epileptische Anf?lle, Delir oder Psychose und Pneumonie oder Tracheobronchitis. Aus diesen Ergebnissen wurde ein Prognose-Score entwickelt, der im individuellen Fall die Wahrscheinlichkeit f?r Tod oder einen FRBI<30 Punkte am Ende der Behandlung pr?diziert (Prognose-Score in der Schlaganfall-Fr?hrehabilitation=PSSF). Eine Optimierung der modifizierbaren Pr?diktoren k?nnte zu einer Verbesserung des Behandlungsergebnisses f?hren. Dies muss im Rahmen einer prospektiven Untersuchung ?berpr?ft werden.
Over the past few years, the number of people who have avoided animal products has been rising steadily. A plant-based diet is associated with a healthier lifestyle and has positive effects on various diseases. More and more healthy active people and performance-orientated athletes are giving up animal products for various reasons, such as for an improved performance or faster regeneration. However, the data in this context are limited. This study aimed to obtain initial findings on the influence of a diet change to veganism on the performance of strength-trained individuals. For this study, a total of 15 omnivorous individuals were recruited. They documented their dietary food intakes over 16 weeks. Every four weeks, the strength performance was tested via a leg press and bench press. In the first 8 weeks, the participants maintained their omnivorous diet, followed by 8 weeks of a vegan dietary phase. In total, 10 subjects participated successfully, and their data were part of the statistical analyses. There was no difference in the absolute and relative strength performance for the leg and bench press after changing to a vegan diet. For the total calorie intake and carbohydrates, only a small treatment effect, but no time effect, was observed. However, for the protein intake, a time and group effect were detected. In addition, the relative protein intake decreased significantly and was lower than the current recommendations for athletes. The results demonstrate that a change to a vegan diet has no beneficial nor negative effect on the strength performance when the total calorie intake and carbohydrate content are covered in the first 8 weeks.
