Reabilitar maxilares severamente atróficos é um desafio para os profissionais cirurgiões dentistas, uma opção é a reconstrução com enxertos ósseos para posterior protocolo com implantes osseointegrados e próteses implantossuportadas. Contudo, a necessidade de áreas doadoras, ou mesmo de banco de ossos, vários momentos cirúrgicos e longo tempo de tratamento tem sido um empecilho. Nesse sentido, recurso terapêutico que tem ganhado notoriedade e espaço são os implantes zigomáticos (ZIs). Esse, por vez, dispensa a necessidade de enxertias e possibilitam carga imediata. O objetivo desta revisão de literatura atual foi realizar uma abordagem acerca da reabilitação com ancoragem zigomática, compreender as vantagens e desvantagens desse tipo de implante, indicações, contra indicações, complicações e as técnicas cirúrgicas mais empregadas na terapia. Foram selecionados artigos científicos, livros e teses publicados entre (2015-2022) em inglês e português, consultados nas bases de dados: SciELO, MEDLINE, Cochrane Library, PubMed e Google Scholar. Os Zis, quando associado a uma correta indicação, tem mostrado excelentes resultados na intervenção de maxila com severa reabsorção de osso, apresentando índices de sucesso bastante elevados.
