Recibido (nueva versión): 23 de diciembre de 2019 / Aceptado: 9 de marzo de 2020 Resumen Los azoles son fármacos que inhiben la enzima 14α-esteroldemetilasa, impidiendo la unión de ergosterol; esto altera la estructura y función de la pared celular fúngica. Especialmente el grupo de los triazoles: fluconazol, itraconazol, voriconazol, posaconazol e isavuconazol, son una alternativa farmacológica para el tratamiento de la enfermedad fúngica invasora causada por Aspergillus spp, Candida spp, Cryptococcus spp, patógenos emergentes como los Mucorales, y de micosis endémicas como las ocasionadas por Histoplasma spp y Coccidioides spp. Los efectos adversos de los triazoles son menos frecuentes comparados con los ocasionados por anfotericina B, un antifúngico de uso común para estas micosis. Los principales efectos adversos de los triazoles son hepáticos, gastrointestinales y cardiovasculares como la prolongación del intervalo QT. Las interacciones farmacológicas son usuales y se presentan con moléculas que usan sustratos del citocromo CYP3A4, lo que incluye anti-retrovirales, anti-tuberculosos e inmunomoduladores. En este trabajo se revisan la historia, características farmacológicas y los ensayos clínicos que evidencian su eficacia clínica en los diferentes escenarios clínicos. Objetivo: determinar los perfiles de susceptibilidad a los principales agentes antimicrobianos utilizados en el manejo de infección de vías urinarias adquirida por gestantes en la comunidad, y caracterizarlos molecularmente para confirmar la existencia de resistencia bacteriana en este grupo poblacional. Materiales y métodos: Estudio de corte transversal, descriptivo, en el que se incluyeron gestantes con infección urinaria adquirida en la comunidad que requirieron hospitalización. Estas hacían parte de un estudio realizado en población general. Se analizaron los resultados microbiológicos de los urocultivos. Se identificaron los aislamientos de Escherichia coli, Klebsiella spp. y Proteus mirabilis durante 12 meses en 9 hospitales de Colombia, y se determinó su perfil de susceptibilidad por microdilución en caldo y pruebas de difusión por gradiente; se caracterizó la presencia de betalactamasas de espectro extendido, con métodos microbiológicos y moleculares. Se presentan las características sociodemográficas y clínicas de estas pacientes. Resultados: se recogieron 74 aislamientos (64 de E. coli, 7 de Klebsiella spp. y 3 de P. mirabilis) en 73 pacientes. En 58 % de las pacientes se reportó uso previo de antibióticos. La resistencia a ampicilina/ sulbactam, cefazolina y ceftriaxona fue de 15,6, 17,2 y 4,7 %, respectivamente. Tres aislamientos,

CASE REPORT | Abstract |Acute aortic dissection is a serious cardiovascular event and the most common acute disease of the great vessels. According to the anatomical distribution of the compromised aorta, the Stanford Group classifies it into type A and type B. Its prognosis depends on its early identification and treatment, as the mortality rate in type A increases rapidly with each hour of delay of diagnosis.Clinical manifestations of aortic dissection may be varied, which makes its early diagnosis difficult. Regarding its diagnosis, genital pain is one of the rarest symptoms. In this paper, the case of a patient who initially attended a health care institution due to acute bilateral testicular pain and was eventually diagnosed with acute aortic dissection is presented. | Resumen |La disección aórtica aguda es un evento cardiovascular catastrófico que corresponde a la más común de las enfermedades agudas de los grandes vasos. Según la distribución anatómica de la aorta comprometida, el grupo de Stanford la clasifica en dos tipos: A y B. Su pronóstico depende de la identificación y manejo tempranos, siendo la tasa de mortalidad rápidamente creciente en el tipo A con cada hora que se retrasa el diagnóstico.Las manifestaciones clínicas de la disección aórtica pueden ser múltiples, lo que dificulta su diagnóstico precoz. Dentro de las formas de presentación, una de las más infrecuentes es el dolor en los genitales. Se presenta el caso clínico de un paciente que consulta inicialmente por un dolor agudo testicular bilateral y que finalmente es diagnosticado con disección aórtica aguda.
