Background. Goal-directed fluid therapy reduces morbidity and mortality in various clinical settings. Respiratory variations in photoplethysmography are proposed as a noninvasive alternative to predict fluid responsiveness during mechanical ventilation. This paper aims to critically evaluate current data on the ability of photoplethysmography to predict fluid responsiveness. Method. Primary searches were performed in PubMed, Medline, and Embase on November 10, 2011. Results. 14 papers evaluating photoplethysmography and fluid responsiveness were found. Nine studies calculated areas under the receiver operating characteristic curves for ΔPOP (>0.85 in four, 0.75–0.85 in one, and <0.75 in four studies) and seven for PVI (values ranging from 0.54 to 0.98). Correlations between ΔPOP/PVI and ΔPP/other dynamic variables vary substantially. Conclusion. Although photoplethysmography is a promising technique, predictive values and correlations with other hemodynamic variables indicating fluid responsiveness vary substantially. Presently, it is not documented that photoplethysmography is adequately valid and reliable to be included in clinical practice for evaluation of fluid responsiveness.

Det er et ansvar vi må ønske å ha.Noen fagfelt -som anestesi og traumatologi -er preget av tidskritiske ja/nei-beslutninger som må tas hurtig og på sviktende informasjonsgrunnlag. I andre fagfelt, for eksempel rehabilitering, er usikkerheten knyttet til komplekse og langvarige beslutningsprosesser over uker og måneder.Tradisjonelt har vi vaert flinke til å vektlegge oppdatert faglig kunnskap om diagnostikk og behandling. Når vi opplever feil og avvik, er løsningen ofte å søke mer kunnskap og/eller utarbeide nye retningslinjer og prosedyrer. Dette er viktige og riktige forutsetninger for god medisinsk praksis, men bare en del av løsningen.Her vil jeg belyse temaet fra en annen vinkel: Hvordan kan innsikt i beslutningsteori gjøre oss til bedre beslutningstagere? Ingen oppgitt interessekonflikter.Min kronikk om hva kompleksitetsteori kan laere oss om sykdom og organisering har vakt stor interesse, og jeg takker Carl-Fredrik Bassøe for hans engasjement. Han innvender at den har begrenset verdi fordi «kaosteori primaert gjelder åpne systemer, mens biologiske prosesser foregår i lukkende kammer». Jeg mener at hans kommentar bygger på to helt sentrale misforståelser: 1) kompleksitetsteori er ikke det samme som kaosteori, og 2) biologiske systemer er ikke lukkede.For det første er jeg uenig i at organsystemer er lukkede. Selv om molekylaere prosesser foregår i adskilte celler og langs ulike metabolske stier, interagerer organsystemene våre på måter vi stadig lar oss forbløffe over (1). For det andre -og dette er den viktigste misforståelsen -er det helt fundamentale forskjeller på kaosteori og kompleksitetsteori (selv om begge misvisende ofte faller innunder begrepet complexity science).Kaosteori beskriver deterministiske systemer der usikkerhet og endringer oppstår på grunn av små endringer i initialverdier. En minimal endring i initialverdier gir store endringer i utfall. Denne effekten kalles ofte «sommerfugleffekten» og har gitt opphav til den etterhvert misforståtte idéen om at en sommerfugl som flakser med vingene et sted, gir tropisk storm et annet sted. Kompleksitetsteori -derimot -beskriver ikke-deterministiske systemer der usikkerhet oppstår på grunn av emergens, altså egenskaper det ikke er mulig å forutse selv om vi (hypotetisk) kjente initialverdiene med uendelig grad av nøyaktighet. Kaosteori beskriver altså systemer der usikkerheten skyldes praktiske vanskeligheter med å måle systemets initialverdier, mens kompleksitetsteori beskriver systemer der usikkerheten skyldes systemets iboende usikkerhet som oppstår på grunn av emergens. I dette ligger vesensforskjellen mellom kaotiske og komplekse systemer.Bassøe har rett i at kunnskap om initialbetingelsene ofte er utilgjengelige for klinikere og administratorer. Men nettopp dette underbygger viktigheten av å bevege seg bort fra et reduksjonistisk tankesett -der en av forutsetningene er at vi forsøker kontrollere initialbetingelsene -og heller forsøksvis beskriver sammenhenger basert på de verktøyene kompleksitetsteori gir oss (2). Kaosteori har gitt viktig innsikt på mange områder -for eksempel viktigheten av et systems sensitivitet for initialverdier. Men kaosteori er likevel kun et verktøy for å forstå deterministiske systemer. For å forstå komplekse, ikkedeterministiske systemer -som menneskekroppen eller store organisasjoner -har innsikt fra kompleksitetsteori enormt potensiale.
