BACKGROUND: Studies have suggested an association between the presence of acanthosis nigricans (AN) and the development of diabetes. OBJECTIVE: To investigate the association between AN and insulin resistance (IR) in overweight children and adolescents receiving care at the Center for Childhood Obesity, Campina Grande, PB. METHODS: This cross-sectional study was conducted between April 2009 and April 2010 including 194 individuals of 2 to 18 years of age receiving care within the Brazilian national health network. The presence of acanthosis nigricans was verified and anthropometric measurements were taken. The following tests were performed: insulin, triglycerides, HDL-cholesterol, glucose and homeostasis model of assessment -insulin resistance (HOMA-IR). Statistical analyses were performed using the SPSS software program, version 17.0. RESULTS: There was a greater prevalence of females (66%), brown-skinned individuals (63.4%), adolescents (61.3%) and severely obese individuals (66.5%). Acanthosis nigricans was identified in 58.2% and IR in 42.7% of the participants. Acanthosis nigricans was associated with being non-white (p = 0.003), with being an adolescent (p = 0.003) and with IR (p = 0.001). Non-white individuals, adolescents and those with insulin resistance were 5.4, 2.47 and 2.66 times more likely to have acanthosis nigricans, respectively. CONCLUSION: The results of this study indicate a need to train healthcare professionals to identify acanthosis nigricans, since this condition is associated with IR. Identifying acanthosis nigricans in childhood permits the safe and timely treatment of cardiometabolic disorders through careful monitoring and appropriate treatment. OBJETIVO: Verificar a associação entre Acantose Nigricans e Resistência Insulínica (RI) em crianças e adolescentes com excesso de peso, atendidos no Centro de Obesidade Infantil, Campina Grande-PB. MÉTODOS: Estudo transversal realizado entre abril/2009 a abril/2010, com amostra de 194 pessoas entre 2 e 18 anos, usuários do Sistema Único de Saúde. Na avaliação, foi observada a presença de AN e verificadas as medidas antropométricas. Foram realizados os exames: insulina, triglicerídeos, HDL-colesterol, glicose e HOMA-IR. As análises estatísticas foram realizadas no SPSS, 17.0. RESULTADOS: Houve maior prevalência do sexo feminino (66%), pardos (63,4%), adolescentes (61,3%) e obesos graves (66,5%). Foi identificada AN em 58,2% e RI em 42,7%. A Acantose Nigricans esteve associada à cor não-branca (p=0,003), adolescentes (p=0,003) e RI (p=0,001). Os não-brancos apresentaram chance de 5,4 vezes maior de terem Acantose Nigricans, os adolescentes, de 2,47 e os com Resistência Insulínica, de 2,66. PURPOSE: to identify changes in the food intake patterns among overweight children and teenagers, treated at a reference medical centre. METHOD: the method used is that of a cohort study, between April 2010 and April 2011. A total of 109 children and teenagers, either obese or overweight, took part in the study. The population was divided into two subgroups depending on the permanence period (more than 6 months, and less than 6 months off the treatment). The chi-square test and logistic regression were carried out. RESULTS: the group which had been longer off the treatment tended to consume more soft drinks, pasta and fried foods, and less fruit and vegetables. The group with less time showed an improvement, with a reduction of consumption of soft drinks and other goodies. There was confirmation of an increased risk for consumption of soft drinks, pasta and goodies in general, as also detachment from the treatment in adolescence. CONCLUSIONS: The group with a longer period of monitoring has had a positive change in food intake frequency. The main contribution made by this study is that of showing that multiprofissional treatment, including some nursing care, is efficient in progressively changing the food intake of children and adolescents who are overweight.
ABSTRACT. The male participation in the process of breastfeeding needs to be encouraged as it can contribute significantly to the prevention of early weaning. In this sense, this research aimed to identify a parent's knowledge about breastfeeding. This is an exploratory and descriptive study with a qualitative approach, performed in the Family Health Strategy in Bom Jesus, Rio Grande do Norte State, Brazil. Data were collected through semi-structured interview with the guiding question: what is your understanding of breastfeeding? The population was 15 men with aged over 18 years old and preserved mental faculties, who lived with his wife and the child under one year of age and performed development and growth consultations in the Family Health Strategy. The interviews were conducted in place previously defined parent. The information was organized as content analysis Bardin, giving rise to two categories and analyzed according to the symbolic interacionism. It was observed that parents of participating antenatal clinics have more knowledge about it and recognize the benefits of breastfeeding in this way can encourage their wives in order to exclusive breastfeeding.Keywords: breastfeeding, father, nursing, primary health care.Conhecimento do homem sobre aleitamento materno RESUMO. Participação do homem no processo da amamentação pode colaborar de forma significativa para a prevenção do desmame precoce. Esta pesquisa teve o objetivo de identificar o conhecimento do pai sobre o aleitamento materno. Trata-se de um estudo exploratório e descritivo, com abordagem qualitativa, realizado em uma Unidade de Saúde da Família, em um município do interior do Rio Grande do Norte, Brasil. Os dados foram obtidos por meio de entrevista semiestruturada, com a seguinte questão norteadora: qual o seu entendimento sobre a amamentação? A população foi composta por 15 pais, com idade acima dos 18 anos e faculdades mentais preservadas; que residiam com a esposa e o filho menor de um ano de idade; e que realizavam a consulta de puericultura na Unidade Básica de Saúde. As entrevistas foram realizadas em local previamente definido pelo genitor. As informações foram organizadas segundo Bardin, dando origem a duas categorias e analisadas de acordo com o Interacionismo Simbólico. Observou-se que a maioria dos entrevistados tem conhecimento sobre o assunto e reconhece os benefícios da amamentação, desta forma conseguem incentivar suas esposas com vistas ao aleitamento materno exclusivo.Palavras-chaves: lactente, pai, enfermagem, atenção primária à saúde.
OBJECTIVE: to analyze the association between the presence of Acantose nigricans and metabolic changes in overweight adolescents, so as to ascertain the relevance of the identification of this marker in the nursing consultation. METHOD: a cross-sectional study undertaken between April 2009 and April 2010 with 118 adolescents who were service users of the Center for Child Obesity in Campina Grande in the Brazilian State of Paraíba (PB). The presence of Acantose nigricans, and the subjects' anthropometric measurements, were investigated. The following exams were made: insulin, triglycerides, HDL-Cholesterol, Glucose and the homeostatic model of assessment (HOMA-IR). RESULTS: there was association between the presence of Acantose nigricans and participants with insulin resistance (p=0.008), metabolic syndrome (p=0.031), elevated triglycerides (p=0.045) and altered HDL (p=0.002). CONCLUSIONS: the suggestion is supported that the detection/identification of Acantose nigricans may be used in the nursing consultation as a tool for identifying overweight adolescents with greater risk of metabolic changes.
The objective of this study was to reflect on the causes of obesity beyond physical, genetic and biological factors. It is based on the understanding of the human condition and its associated economic and social relationships, derived from Hannah Arendt's Theory of The Human Condition and relevant literature. The satiety-need cycle and the technological advances of modern society cause consumerism, a sedentary lifestyle that affects children´s health and nutritional education, and an increase of body fat in people. The causes of obesity surpass the direct results of excessive caloric intake and indicate the need to consider the person's social and economic context. Health professionals need to understand the existing relationship of the self with the socioeconomic, cultural, and political factors that determine obesity in children and adolescents.
Este estudo objetiva relatar a experiência de acadêmicos, mestrandos e professores de enfermagem no Centro de Obesidade Infantil quanto ao processo de acompanhamento e tratamento de crianças com sobrepeso e obesidade, entre agosto de 2008 a janeiro de 2012. O Centro conta com equipe multiprofissional para o acompanhamento de 349 crianças; essas têm a oportunidade de serem acompanhadas pelos profissionais, os quais interagem na discussão sobre o estado clínico e avaliam a sua evolução. A vivência no Centro propiciou perceber a real possibilidade de reunir uma equipe multidisciplinar focada em um mesmo objetivo com vistas a realizar um trabalho contínuo que possibilite meios para o alcance de resultados satisfatórios à saúde de crianças obesas ou com sobrepeso.
Este estudo objetivou avaliar a consulta de enfermagem aos pacientes com tuberculose. Estudo descritivo, quantitativo, realizado em município do nordeste brasileiro, com 60 enfermeiros da Atenção Primária à Saúde, mediante aplicação de questionário baseado no protocolo da consulta de enfermagem ao paciente com tuberculose do Ministério da Saúde. Elementos da coleta de dados eram mais abordados, ocorrendo, na anamnese e exame físico, a supervalorização das queixas (100% sempre investigavam) e da mensuração do peso do paciente (96,8% realizavam). Os diagnósticos de enfermagem, planejamento, implementação e avaliação do plano terapêutico não eram realizados completamente e aspectos socioculturais eram subestimados. Essa fragmentação pode contribuir como deficiência na operacionalização do Plano de Controle da Tuberculose. A Consulta de Enfermagem, se ampliada científica e tecnologicamente pode favorecer o desenvolvimento dos programas de saúde e a minimização de entraves que permeiam o controle da Tuberculose.
