Introducción: El compromiso laboral es un estado mental positivo, satisfactorio y relacionado con el trabajo; caracterizado por vigor, dedicación y absorción; está relacionado con una mejor atención al paciente y menos errores médicos debido a la inexperiencia y/o falta de tiempo. Por otro lado, el estrés es un estado de cansancio mental, multifactorial, depende de la percepción de situaciones específicas y de la habilidad para sobrellevarlas. Los médicos residentes están sujetos a múltiples estresores perjudiciales a su desempeño.Objetivo: Describir el nivel de compromiso laboral y estrés percibido entre los residentes en la Ciudad de México y la probable relación entre estas variables.Método: Estudio cuantitativo, exploratorio, no experimental, descriptivo y de corte transversal. Se evaluaron un total de 248 residentes médicos de la Ciudad de México, utilizando las escalas UWES-15 y PSS. Análisis estadístico descriptivo y test de Chi cuadrado de Pearson.Resultados: La media de compromiso fue de 4.5 (± 1.2), y de las dimensiones: vigor (4.3, DE ± 1.4), dedicación (4.6, DE ± 1.3) y absorción (4.4, DE ± 1.3). El total de los residentes tuvo un promedio de 38.1 (DE ± 7.6) de estrés percibido. De acuerdo al análisis de asociación entre compromiso y estrés percibido, se encontró que existe una correlación moderada lineal negativa estadísticamente significativa (Spearman r = –0.443, p = 0.000).Conclusiones: Este estudio es el primero en examinar el nivel de compromiso laboral y de estrés percibido entre los médicos residentes de la Ciudad de México. Estas variables son comunes en la formación médica de los residentes, y se demostró que existe correlación negativa entre estas variables. Mejorar el compromiso laboral es importante tanto para los médicos jóvenes como para los pacientes, ya que estos últimos recibirán una atención de mejor calidad.
La dermatoscopía es un método auxiliar para el diagnóstico de las neoformaciones cutáneas, principalmente las melanocíticas, que disminuye el número de biopsias innecesarias. En la actualidad también se utiliza para la evaluación clínica de enfermedades inflamatorias, tricopatías y trastornos ungueales. Las principales barreras para implementar su uso en la práctica clínica son la falta de acceso a programas de capacitación formal y el costo del equipo para los sistemas de salud públicos. El objetivo de esta revisión fue describir y analizar los diferentes métodos de enseñanza-aprendizaje implementados en los programas de formación de médicos generales, residentes y dermatólogos.
Caso para diagnóstico CASO CLÍNICOSe trata de un paciente del sexo masculino, de 14 años de edad, originario y residente de la Ciudad de México, quien acudió por una dermatosis localizada a cabeza, de la que afectaba cara y de ésta el lado derecho del mentón, constituida por una neoformación oval, eritematoviolácea, de bordes bien delimitados, superficie con fina escama blanquecina seca, con costras sanguíneas en uno de sus extremos, de consistencia firme. De evolución crónica y asintomática (Figura 1).Al interrogatorio refiere haber iniciado cuatro años atrás con «un granito» en la cara, el cual traumatizó. La lesión fue creciendo lentamente hasta adquirir la morfología actual. Se aplicó un ungüento con sulfato de neomicina y retinol, sin mejoría. Se realiza biopsia escisional de la lesión. El estudio histopatológico con tinción de rutina muestra una lesión hemisférica en cuya epidermis predomina la acantosis irregular. La dermis superficial forma una delgada banda de colágeno normal, por debajo de la cual se observan numerosos agregados de células redondas que hacia la profundidad exhiben un patrón fusiforme (Figuras 2 y 3).Con los datos clínicos e histológicos proporcionados, ¿cuál es su diagnóstico?
