Os pilares protéticos muito utilizados são feitos de titânio, porém, a aparência acinzentada do pilar metálico através dos tecidos moles e a recessão gengival com exposição do metal resulta na insatisfação estética, desfavorecendo o sorriso harmônico para o paciente. As indicações encontradas na literatura, para o uso de pilares cerâmicos, estão na correção de pequenas alterações no posicionamento do implante, gengivas delgadas, onde há risco de transparência do titânio, substituição unitária em regiões estéticas, ou podendo ser uma alternativa em casos de dentes posteriores ou próteses parciais fixas. Em uma revisão de literatura, abutment de zircônia tem uma taxa de sobrevida de 5 anos de 99.1% com menos problemas técnicos, menos complicações biológicas e mais resultados estéticos em comparação ao abutments de titânio. Dadas as propriedades da zircônia, demonstrou-se que, os pilares personalizados de zircônia é um ótimo material, tanto em propriedades estéticas, quando a gengiva se torna delgada evitando expor a aparência acinzentada do titânio, como mecânicas, tornando o material mais resistente, promovendo previsibilidade ao tratamento e sempre utilizando a base metálica em titânio para a longevidade da peça. Tanto a zircônia como o dissilicato de lítio, obtiveram resultados significativos à resistência a fratura, podendo ser usado também como uma opção de tratamento para pilares cerâmicos personalizados.
Diversos materiais e técnicas têm sido utilizados para o tratamento de maxila atrófica e para uma reabilitação com próteses implantossuportadas é necessário a realização de procedimentos para criar volume ósseo, necessário para a instalação dos implantes ou, desviar desta técnica, utilizando outras táticas, onde não á necessidade de criar este volume ósseo, para a instalação dos implantes. Este estudo tem como objetivo, fazer uma revisão de literatura sobre os principais tipos de tratamento para maxila atrófica, incluindo a técnica All-on-Four, implantes zigomáticos e a necessidades enxertos e/ou biomateriais para a instalação dos implantes, assim discutir seus propósitos e comparando suas técnicas para a escolha do tratamento. Assim a escolha da técnica deve ser orientada por diversos critérios que associados a uma boa análise crítica da literatura disponível devem direcionar o clínico a resultados satisfatórios.
