INTRODUÇÃO:O SUS é um sistema em permanente evolução e estruturação, o qual busca se afastar de uma visão "medicalizante" e "hospitalocêntrica", tendo como foco (além do tratamento) a prevenção e a reabilitação das doenças e seus agravos. Esta postura (que afirma um compromisso em defesa da vida e da garantia de acesso à saúde de qualidade para todos) exige a prática contínua da ferramenta tecnológica e relacional do acolhimento, a qual enquanto diretriz da PNH, (re) organiza a assistência em saúde promovendo humanização, protagonismo do usuário e qualidade do cuidado prestado. Ademais, em consonância ao acolhimento, o dispositivo de classificação de risco viabiliza a humanização ao promover a equidade, ofertando atendimento aos usuários segundo o seu risco/vulnerabilidade, e desta maneira aborda o processo de adoecimento do indivíduo em suas múltiplas dimensões (social, biológica e subjetivas). OBJETIVOS: Aperfeiçoamento das ações de acolhimento e classificação de risco (ACCR) e construção de protocolo de ACCR para a rede municipal da atenção primária de um município do interior da Bahia. RELATO DE EXPERIÊNCIA: O projeto de extensão da Universidade Estadual do Sudoeste da Bahia: "Acolhe o risco, humaniza a atenção" por meio de ações de educação permanente em saúde (ratificando e qualificando a assistência em ACCR), práticas de diagnóstico situacional e proposição de estratégicas para a melhoria da assistência em saúde do setor de ACCR da atenção primária, bem como (e em conjunto com as equipes dos setores ACCR desses serviços) têm qualificado à assistência da rede de urgência/emergência no município do interior da Bahia onde elaborou-se um protocolo piloto em acolhimento e classificação de risco inédito para a atenção primária em saúde. DISCUSSÃO: O projeto "Acolhe o risco, humaniza a atenção" ancora-se na PHN e em seus dispositivos relacionais (acolhimento) e de reorganização do atendimento (avaliação de risco), entendendo ser este o caminho para um fazer saúde mais humanizado, para todos, com protagonismo do usuário e, principalmente, equânime. CONCLUSÃO: Dessa maneira, esta experiência (que têm se mostrado exitosa) ratifica a importância dessas duas ferramentas tecnológicas (acolhimento e classificação de risco) na área da saúde coletiva.
