Collagen corneal cross-linking (CXL) has proved to be effective in halting the progression of keratoconus and post-LASIK ectasia.Post-photorefractive keratectomy (PRK) ectasia, a rare PRK complication, has been reported in only a few cases, although PRK is the oldest form of laser refractive surgery. CXL for post-PRK ectasia has not been reported yet.Here is a case of a 22-year-old male who developed post-PRK ectasia more than 1 year after the procedure and was treated using CXL. A few months after CXL, an Intac (Addition Technology) was implanted due to contact lens intolerance.
Post-Lasik ectasia is the most feared complication following laser in situ keratomileusis (Lasik), occurring in majority of cases within 1 year of surgery. Furthermore, it is reported many years after surgery. In this case series, we report on late onset ectasia in 11 eyes of nine patients occurring at least 6 years after surgery. Causes and management plan are not discussed here. The aim is to emphasis the late occurrence of this complication.

SummaryA 63-year-old woman was referred to the eye clinic with a 2-year history of unresolving right eye discomfort, irritation and recurrent conjunctivitis, managed by her general practitioner, where frequent use of topical antibiotics and ocular lubricants provided little and transient relief. The right eye was blind following a complicated retinal detachment surgery 30 years ago. Examination revealed an extruding silicone sponge scleral buckle from a previous retinal detachment surgery in the superior conjunctival fornix, under the right upper lid. This was subsequently surgically removed and her symptoms resolved.
BACKGROUND
Collagen corneal cross-linking is an acceptable method for halting the progression of keratoconus and post-Lasik ectasia. The traditional method advocates the removal of epithelium for proper saturation of riboflavin within the stroma. The postoperative complications associated with epithelial removal have necessitated the search for a transepithelial method of riboflavin delivery. Controversies aroused with regard to the effectiveness of this approach. In this article, we review the up- to-date literature on this controversial subject.
