SUMMARY The aetiology of the so‐called fat embolism is discussed, and the treatment of five cases widi this syndrome is described. In our opinion, fat embolism is the result of a traumatic stress‐induced intravascular coagulation and disturbances in fat metabolism. A clinical programme is given for early prophylactic treatment, including adequate colloid infusions, high caloric supply, sedatives and “sympathico‐lytics,” and heparin. More than 20 severe traumatic cases with considerable risk of fat embolism were treated according to this programme. Most of them did not show any severe symptoms of fat embolism. In the five cases in which clinical symptoms of fat embolism appeared, the initial prophylactic treatment was not quite in accordance with our present treatment programme. After further intensive respiratory and circulatory therapy four of the five patients survived. In die fifth case, therapy started late; the pulmonary changes were too severe to allow adequate ventilation, and die patient died of respiratory insufficiency. The use of corticosteroids in two patients resulted in clinical and radiographic improvement. ZUSAMMENFASSUNG Die Ätiologie der sogenannten Fettembolie wir ddiskutiert und die Behand‐ung von 5 Fällen mit diesem Syndrom beschrieben. Nach unserer Auffassung ist die Fettembolie das Resultat einer durch den traumatischen Stress bedingten intravaskulären Koagulation und von Störungen im Fettstoffwechsel. Für die frühprophylaktische Behandlung wird ein klinisches Programm erstellt, das ausreichende Kolloidinfusionen, hohe Kalorienzufuhr, Sedativa, Sympatikolytika und Heparin beinhaltet. Mehr als 20 Unfallpatienten mit beträchtlichem Fettembolierisiko wurden, diesem Programm entsprechend, behandelt. Die meisten von diesen zeigten keine schweren Symptome von Fettembolie. Bei den 5 Fällen, bei welchen die klinischen Symptome der Fettembolie aufschienen, war die anfängliche prophylaktische Behandlung nicht ganz im Einklang mit unserem derzeitigen Therapieplan. Nach weiterer intensiver Behandlung von Atmung und Kreislauf überlebten 4 dieser 5 Patienten. Beim fünften Fall begann die Behandlung zu spät; die pulmonalen Veränderungen waren zu schwer um eine adäquate Beatmung zu ermöglichen. Der Patient erlag seiner Ateminsuffzienz. Die Anwendung von Corticosteroiden bei zwei Patienten führte zu klinischer und röntgenologischer Besserung.
Cardiac output was determined and arterial and pulmonary artery pressures were recorded during a control period, after spinal anaesthesia (Th I—III) and after administration of 1 mg dihydroergotomine (DHE) i. v. in 10 healthy males.After spinal anaesthesia, arterial and pulmonary artery pressures decreased, whereas heart rate and cardiac output remained unchanged. After administration of DHE, arterial pressure increased to control level, while pulmonary artery pressure increased significantly above that level. Cardiac output and heart rate remained mainly unchanged.Since stroke volume did not increase after DHE administration, the restitution of arterial pressure must be due to an effect on resistance vessels, although it has been shown that DHE has more pronounced effect on capacity than resistance vessels.However, the increase in pulmonary artery pressure might indicate that in this study also, the effect of DHE was more pronounced on capacity vessels, but in a situation with normal filling pressures this does not affect the cardiac output to a significant degree.ZUSAMMENFASSUNGCardiac Output, Arterien‐und Pulmonalarterien‐druck wurden während einer Kontrollperiode nach Spinalanaesthesie (Th I‐III) und nach Verabrei‐chung von 1 mg Dihydroergotamin (DHE) i. v. bei 10 gesunden Männern registriert.Nach Spinalanaesthesie waren Arterien‐und Pulmonalarteriendruck vermindert, während Herz‐frequenz und Cardiac Output unverändert blieben. Nach Verabreichung von DHE stieg der Arterien‐druck auf das Kontrollniveau an, während der Pulmonalarteriendruck dieses Niveau signifikant überstieg. Cardiac Output und Herzfrequenz blieben im wesentlichen unverändert.Da das Schlagvolumen nach Verabreichung von DHE nicht ansteigt, muß die Wiederherstellung des Arterienausgangsdruckes durch die »Resistancegefäße« bedingt sein, obwohl gezeigt wurde, daß DHE mehr Wirkung auf die Kapazität als auf die »Resistancegefäße« hat.Jedenfalls könnte die Erhöhung des Pulmonalar‐teriendruckes bei dieser Unte suchung darauf hinweisen, daß die Wirkung von DHE auf die Kapazitätsgefäße zwar stärker war, aber daß dies bei normalem Fülldruck den Cardiac Output nicht nennenswert beeinflußt.
SUMMARY In three different test series the local effect of Octapressin® Sandoz in a concentration of 5 I.U./30 ml Xylocaine® Astra was investigated. In a double‐blind series consisting of 157 out‐patients, the haemostatic effect and the postoperative bleeding were assessed. In a second series comprising 31 patients who had been operated on under general anaesthesia for cleft lip and palate, Xylocaine‐Octapressin was used locally, and the vasoconstrictor effect assessed. In a third series, Xylocaine‐adrenaline and Xylocaine‐Octapressin were injected subcutaneously, the skin graft taken, and the bleeding rated. Good or very good vasoconstriction was obtained in 80 per cent in the cases where adrenaline was used, whereas with Octapressin the corresponding figure was 65 per cent. Postoperative bleeding occurred in four cases of 70 after Xylocaine‐adrenaline, and there was slight bleeding in one case among 87 after Xylocaine‐Octapressin. Most of the operations in this series were started immediately. In a cleft lip and palate series, the operations were begun 10‐15 minutes after the local injection. Satisfactory or better effect was obtained in 77 per cent of the cases. It could also be shown at the skin donor sites in a third series, where, in the adrenaline‐injected areas, onset of the vasoconstrictor effect was more rapid (within 5 minutes) and somewhat stronger than when Octapressin was injected (5‐15 minutes). Our conclusion is that as a vasoconstrictor in local anaesdiesia, Octapressin has an effect which is equal to or somewhat weaker than that of adrenaline. For its vasoconstrictive effect, Octapressin requires a longer onset time than adrenaline. In view of the local side effects of adrenaline (local cyanosis, lower O2 tension), and its general side effects, we would recommend the use of Octapressin as a vasoconstrictor in local anaesthesia. ZUSAMMENFASSUNG In 3 verschiedenen Testreihen wurde die lokale Wirkung von Octapressin, Sandoz, bei einer Konzentration von 5 I.E. auf 30 ml Xylocaine, Astra, untersucht. Bei 157 ambulanten Patienten wurde mit Hilfe des Doppelblindversuches die haemostatische Wirkung und die postoperative Blutung bewertet. Bei einer zweiten Serie von 31 Patienten, die unter Allgemeinnarkose wegen Lippen‐ und Gaumenspalten operiert wurden, wurde Xylocain‐Octapressin lokal angewendet und der vasokonstriktorische Effekt bewertet. Schliesslich wurden in einer dritten Serie Xylocain‐Adrenalin, bzw. Xylocain‐Octapressin subcutan injiziert, ein Hauttransplantat entnommen und die Blutung ausgewertet. Gute oder sehr gute Vasokonstriktion wurden in 80% der Fälle erzielt, bei denen Adrenalin angewandt wurde, während die korrespondierende Zahl bei Octapressin 65% betrug. Postoperative Blutung wurde bei 4 von 70 Fällen nach Xylocain‐Adrenalin und leichte Blutung nur in einem Fall von 87 nach Xylocain‐Octapressin beobachtet. SUMMARYIn five cats the blood flows in the skeletal muscles and the skin were simultaneously recorded with a drop‐counting technique during exposures to halothane or ether. During both halothane and ether exposures, very similar qualitative response patterns were found, viz. arterial hypotension, increased or moderately decreased muscle vascular resistance, markedly decreased skin vascular resistance, decrease in heart rate and a more or less unchanged or slightly increased central venous pressure. The combination of the changes in peripheral resistances in the two tissue regions investigated is not considered to contribute greatly to the rather pronounced arterial hypotension, for which other causative factors are suggested, e.g. a reduction in cardiac output.Even though depression anywhere in the sympathetic pathway may explain a skin vasodilatation, it is suggested that the divergence of the vascular responses in the two investigated tissue regions is caused mainly by a differentiation within the bulbar vasomotor centre when released from baroreceptor inhibition owing to the blood‐pressure fall leading to a reflexly induced increase in vasoconstrictive discharge to the skeletal muscles but not to the dilated cutaneous vascular bed.The quantitative differences between the responses to halothane and to ether, i.e. a less pronounced hypotension and a smaller decrease in skin vascular resistance and heart rate, as well as a seemingly more pronounced increase in muscle vascular resistance during the influence of ether, are, it is suggested, caused by the sympathicomimetic properties of this anaesthetic.ZUSAMMENFASSUNGBei 5 Katzen wurde während Halothan‐ oder Äthernarkose die Blutdurchströmung im Skelettmuskel und in der Haut mit einer Tropfenzählmethode simultan registriert. Die qualitativen Reaktionsmuster waren bei beiden Narkoseagentien sehr ähnlich, das heisst arterielle Hypotension, erhöhter oder schwach verminderter Gefässwiderstand im Muskel, deutlich verminderter Gefässwiderstand in der Haut, Abfall der Herzfrequenz und ein mehr oder weniger unveränderter oder schwach erhöhter zentraler Venendruck. Die Kombination der Veränderungen im peripheren Widerstand dieser zwei untersuchten Gewebsregionen kann wohl kaum besonders zu dem ziemlich ausgeprägten arteriellen Blutdruckabfall beitragen, fur den andere ursächliche Faktoren herangezogen werden müssen, z. B. eine Verminderung des Herzminutenvolumens.Obwohl eine Depression an irgendeiner Stelle der sympathischen Leitungsbahnen die Gefässerweiterung in der Haut erklären könnte, wird doch angenommen, dass die Divergenz der vaskularen Reaktionen in den beiden untersuchten Gewebsregionen hauptsächlich durch eine Unterschiedlichkeit im Bereich des bulbären Vasomotorenzentrums bedingt ist, wenn dieses von der Barorezeptorenhemmung infolge des Blutdruckabfalls abgeschaltet ist, was zu einer reflexbedingten Erhöhung der vasokonstriktiven Reize zur Skelettmusku‐latur, aber nicht zu dem erweiterten Gefässbett der Haut führt.Was die quantitativen Unterschiede zwischen der Reaktion auf Halothan und auf Äther betrifft, das heisst die schwächer ausgeprägte Hypotension und der geringere Abfall des Gefässwiderstandes in der Haut, sowie der Herzfrequenz, als auch die anscheinend ausgeprägtere Erhöhung des Gefässwiderstandes im Muskel unter dem Einfluss von Äther, wird angenommen, dass diese durch die sympathiko‐mimetischen Eigenschaften dieses Narkotikums bedingt sind.
