Juvenile osteochondrosis, which can be complicated by a herniated disc, is a common disease in the pediatric population. Currently, this nosology is considered as one of the forms of manifestation of connective tissue dysplasia syndrome. A clinical study was conducted to identify dysplastic processes among 34 adolescents operated on for a herniated disc. Based on the characteristic phenotypic features, it was revealed that 15.36 % of the subjects had grade 1 connective tissue dysplasia, 74.47% — grade 2, and 10.17% — grade 3. The diagnosis was supplemented by a biochemical blood test for C-terminal telopeptides of type I collagen Beta-Cross Laps. In 18 patients (52,94%), the biochemical blood test exceeded the reference values, indicating consistency in the ongoing systemic catabolic reactions associated with the prevalent destruction of the collagen fibers of the tissues of the body.
Introduction. In pediatric practice, herniated discs that require surgical treatment are quite rare (0.5% of all surgical interventions for degenerative spinal diseases). At the same time, there is a tendency to the increase in the number of diagnosed intervertebral disc extrusions in adolescents. Despite of good effectiveness of the standard microdiscectomy, new techniques for removing a herniated disc are being developed to improve results in the postoperative period. Material and methods. The paper presents outcomes of surgical treatment of 59 patients with herniated discs, including 22 children. A comparative characteristics of two techniques was made: traditional microdiscectomy where a hernial component is removed and advanced microdiscectomy where an additional foraminotomy is made under the control of endoscopic technique. The discussed modified surgical treatment was patented (patent for invention No. 2687020 dated 06.05.2019). The authors have compared the effectiveness of these techniques at immediate and long-term periods in all operated patients. Rezults. The number of excellent and good results for the proposed method was higher than 95%, than for the traditional method - 88,9% . Conclusion. The proposed technique improves outcomes of surgical treatment of a herniated disc by eliminating compression of the spinal nerve root in the initially narrow foraminal opening and by expanding the latter under the control of endoscopy with a minimal risk of postoperative complications.
У больных с дисплазией соединительной ткани в связи с высоким риском развития у них послеоперационных осложнений изучалась взаимосвязь качественных и количественных параметров ряда биохимических показателей сыворотки крови. Для предоперационного выявления дисплазии соединительной ткани было использовано определение уровня щелочной фосфатазы, сиаловых кислот, фибриногена, магния эритроцитов и уровня С-терминального концевого телопептида коллагена 1-го типа в сыворотке крови пациента, при этом все исследования проводились одномоментно и однократно в ходе обычного предоперационного обследования. Авторами был выявлен феномен разнонаправленного изменения стабильных клинико-биохимических показателей крови. Данный феномен характерен только для выраженного диспластического процесса в организме. Предложенная методика может быть внесена в стандартное предоперационное обследование пациента по программе обязательного медицинского страхования, так как эти биохимические показатели могут применяться анестезиологами для оценки предоперационного риска. Для предотвращения технических ошибок исключается использование численных диагностических критериев-оценивается направленность изменений пяти показателей в сравнении со среднепопуляционными значениями для данной возрастной группы, полученными на аналогичном лабораторном оборудовании. Исследование проводится с использованием готовых заводских лабораторных тест-систем, что снижает риск ошибочно завышенного или ошибочно заниженного результата анализа.
