Background:The use of the anterior nucleus of thalamus (ANT) as a target for treatment of pharmacoresistant epilepsy is based on its crucial role in seizure propagation. We describe results of chronic bilateral ANT stimulation and bilateral ANT lesions in 31 patients with refractory epilepsy.Methods:ANT DBS was performed in 12 patients (group I) and bilateral stereotactic radiofrequency lesions of ANT were performed in 19 patients (group II). Targeting was based on stereotactic atlas information with correction of the final coordinates according to the location of anatomical landmarks and intraoperative microelectrode recording data.Results:Both groups were similar in age, gender, seizures frequency, and duration of disease. The median x, y, and z coordinates of ANT were found to be 2.9, 5, and 11 mm anterior, lateral, and superior to the mid-commissural point, respectively. Mean seizures reduction reached 80.3% in group of patients with ANT DBS with two nonresponders and 91.2% in group of patients with lesions. Five patients from group I and three patients from group II became seizure-free. The morbidity rate was low in both groups.Conclusions:Stereotactic anterior thalamotomy and chronic ANT stimulation are both effective for seizure control in epilepsy originated from frontal and temporal lobes. ANT lesions and stimulation were more effective for secondary-generalized seizures compared to simple partial seizures.
Background:The use of awake craniotomy for surgical treatment of epilepsy was applied in surgery of convexital tumors, arteriovenous malformations, some superficial aneurysms, and stereotactic neurosurgery. The aim of this study was to show the advantages of awake craniotomy without sedation, accompanied by intraoperative neurophysiological monitoring in patients with symptomatic epilepsy.Methods:This article describes the results of surgical treatment in 41 patients with various pathologies; 31 among them suffered from epilepsy.Results:Most frequently, the pathological foci were located in frontal and parietal lobes nearby eloquent brain areas. Irrespective of damage location, simple partial and complex partial seizures were seen almost with the same frequency. Intraoperative mapping of eloquent cortical areas and subcortical tracts without sedation resulted in total resection of pathological area in 75% of cases with low rate of permanent neurological deficit (two patients). Minor perioperative complications, including the decrease in blood pressure in six patients and intraoperative convulsions in two patients, were handled and did not led to operation termination or anesthesia conversion. Excellent seizures control (Engel 1) was achieved in 80% of patients with available catamnesis.Conclusion:Thus, the proposed method allows eliminating the complications associated with sedation and provides radical resection of pathological epileptogenic foci with low complication rate.

Выявление важной роли передних таламических ядер anterior Thalamic nuclei (aTn) в генерализации судо-рог привело к повышению интереса к хирургическим вмешательствам в этой области у пациентов с фармакорезистентной эпилепсией. Моделирование на животных показало высокую эффективность деструкций aTn в предупреждении развития и снижении частоты приступов. однако двусторонняя радиочастотная деструкция aTn у человека до сих пор не была описанa. Цель исследования -выполнить и оценить результаты двусторонней радиочастотной передней таламотомии у пациентов с фармакорезистентной эпилепсией. Материал и методы. Нами впервые была выполнена двусторонняя стереотаксическая радиочастотная термокоагуляция aTn 13 пациентам, длительно страдавшим фармакорезистентной эпилепсией. перед операцией определяли длительность заболевания, возраст начала приступов, локализацию источников патологической активности, тип приступов и морфоло-гические повреждения, а также оценивали проводимую фармакотерапию. Все вмешательства были осуществлены под местной анестезией и сопровождались интраоперационной микроэлектродной регистрацией нейрональной активности и записью электроэнцефалограммы (ЭЭГ). Результаты. оперированы 7 мужчин и 6 женщин в возрасте от 22 до 48 лет. у всех пациентов эпилептогенные очаги лока-лизовались в лобных и/или височных долях мозга. Мрт выявила эпилептогенные структурные аномалии у 3 пациентов. послеоперационных осложнений не было. при дальнейшем обследовании 5 пациентов отметили отсутствие приступов, у 6 -снижение частоты приступов составило более 70%, у 1 -50% и у 1 пациента не последовало ответа на проведенное вмешательство. Достигнутый эффект выражался не только в снижении частоты и тяжести приступов, но и в уменьшении доз принимаемых противосудорожных препаратов. ЭЭГ также подтвердила значительное улучшение у подавляющего большинства пациентов. Заключение. Наш опыт показывает, что двусторонняя радиочастотная передняя таламотомия является безопасным и эффективным методом контроля судорожных приступов у человека. Дальнейшие исследования позволят уточнить крите-рии отбора пациентов для хирургического лечения на основании клинических и ЭЭГ-данных. Background. Эпилепсия-наиболее частое клиническое проявление кавернозных мальформаций (КМ) медиобазальных отделов височной доли (МБОВД). Хирургическое удаление КМ служит методом лечения ассоциированной эпилепсии и варьирует от иссечения КМ до резекции височной доли. Цель исследования-определение оптимальной хирургической тактики для лечения эпилепсии у пациентов с КМ МБОВД. Материал и методы. Проведен ретроспективный анализ исходов хирургического лечения 11 пациентов с эпилепсией и КМ МБОВД. Всем пациентам выполнена видеоэлектроэнцефалография, а также МРТ и КТ головы до и после операции. Предоперационная имплантация электродов в овальные отверстия проведена в 9 случаях. Все операции сопровождались интраоперационной электрокортикографией (ЭКоГ). Результаты. КМ локализовались в передней трети МБОВД (7), в передней и средней третях (1), в средней трети (2), в средней и задней третях (1). В 8 случаях предоперационный мониторинг выявил зону инициации приступа. Этим пациентам выполнена каверномэктомия с дополнительной резекцией гемосидеринового кольца с использованием птерионального доступа (4 случая) и супрацеребеллярного транстенториального подхода (СТП) (4). В 3 случаях распространение патологической активности на латеральный височный неокортекс потребовало каверномэктомии с дополнительной передней височной лобэктомией. Контроль приступов в катамнезе был отличным у 7 пациентов (класс 1 ILAE) и хорошим-у 4 (класс 2 ILAE). Выводы. Хирургическое лечение у пациентов с резистентной эпилепсией при КМ МБОВД должно проводиться с учетом данных неинвазивного и инвазивного дооперационного исследования. При локализации зоны инициации приступа в МБОВД птериональный доступ и СТП могут быть использованы с одинаковой эффективностью для контроля приступов. Латеральное распространение патологической активности требует выполнения каверномэктомии с дополнительной передней височной лобэктомией.
