Bei Durchsicht des Schrifttums fiber die BlutgerinnungsstSrungen zeigt sich, dab die Blutgerinnungstheorie yon MOI~AWlTZ (1904) wesentliche _Anderungen erfahren hat. Zur Zeit sind bereits schr viele Komponenten bekannt, welche ffir die normale Gerinnung unbedingt notwelidig sind. Es ist wohl nicht fibertrieben, wenn man die Zahl der unerl~-lichen Faktoren bzw. Substanzen der normal verlaufenden Blutgerinnung mit etwa 20--28 veranschlagt. Das Prothrombin, dessen Begriff und Bestimmung mit QuicKs Namen eng verknfipft ist, nimmt im Mechanismus der Blutgerinnung sozusagen eine Zentralstellung ein. Es bildet sich in der Leber in Gegenwart yon K-Vitamin. Seine Menge schwankt im Blur innerhalb ziem]ich waiter Grenzen; solange nicht bis ungef~hr 80% davon verlorengehen, ist die normale Gerinnung gesichert.Nun erhebt sich die Frage: Durch welche Erscheinungen wird ein Absinken des Blutprothrombinspiegels herbeigeffihrt?Bekanntlich ist bei Neugeborenen eine ausgesprochene Senkung des l~rothrombinspiegels zu beobachten, die in den ersten 3 Tagen zunimmt, sodann allm~hlich zur Norm ansteigt. Die Ursache dieser Erscheinung ist sehon bekannt.Eine durch K-Vitaminmangel veranlal~te Verminderung des Prothrombingeh~ltes kann zufolge verschiedener Faktoren eintreten: bei mangelhafter bzw. unzul~nglicher Ern~hrung, bei Erkrankungen des Magen-Darmkanals, wenn neben norm~]er K-Vitaminaufnahme eine l~esorptionsstSrung vorliegt, so u. a. in t0~llen yon Colitis mueosa und ulcerosa, AmSbendysenterie, chronischer Diarrhoe, Sprue, SpeiserShrenund Pylorusstenose, schlecht funktion'erender Gastrostomie, hohem Dfinndarmverschlul3, hoher Ileostomie, ferner bei mehrfachen Strikturen, Gallenfisteln und ]angdauerndem Erbrechen.Auch Lebersch~digungen (Cirrhose, chronische Hepatitis) kSnnen einen Abfall des Prothrombinspiegels herbeiffihren. AuBerdem pflegt bei Obturationsileus in der 2.--3. Erkrankungswoehe der Prothrombingeh~lt abzunehmen, ferner Gallenstein, Gallenblasen-und Pankreaskrebs, GallengangsverschluB als Folge yon Lymphknotendruck.
