Genetic aspects of familial isolated pituitary adenomas (FIPA) are expanding rapidly in the last few years. Around 20% of these FIPA families have been identified to have mutations within the aryl hydrocarbon receptor interacting protein (AIP) gene. The number of identified AIP-positive FIPA families is increasing all over the world.
низкотравматичные переломы и стероидная ми-опатия остаются существенной причиной инва-лидности у пациентов с гиперкортицизмом [1]. впервые переломы тел позвонков и мышечная слабость вследствие гиперкортицизма были опи-саны в 1932 году харвеем кушингом, как одно из клинических проявлений гормонально-активной базофильной аденомы гипофиза [2]. после внедрения глю-кокортикоидных гормонов (гк) в широкую врачебную прак-тику, глюкокортикоидный остеопороз (гко) стал наиболее частой причиной вторичного остеопороза [3].глюкокортикоиды оказывают множественные негатив-ные эффекты на костную ткань. влияние гк может реали-зоваться через системные эффекты (снижение половых сте-роидов, гормона роста, инсулиноподобного фактора роста 1 (иРф-1), ускорение катаболизма витамина D, а вследствие этого отрицательный кальциевый баланс, вплоть до раз-вития вторичного гиперпаратиреоза; снижение мышечной силы, а также непосредственного влияния на остеобластоге-нез и остеокластогенез [1,[3][4][5][6][7][8]. при экзогенном гиперкор-тицизме, определенный негативный вклад может вносить заболевание, по поводу которого гк были назначены, а так-же имеет значение доза, тип гк, продолжительность лече-ния и приверженность пациента к лечению [9][10][11].гко характеризуется выраженным подавлением мар-кера костеобразования -остеокальцина [12][13][14][15], который может быть использован как диагностический тест для эг [16, 17]; значительным увеличением риска переломов в со-четании с умеренным снижением минеральной плотности кости (мпк) [18][19][20][21]. ввиду редкости эндогенного гипер-кортицизма, ранее опубликованные работы по распростра-ненности низкотравматичных переломов в этой категории пациентов включали не более 100 больных [20,21]. Выводы: Пациенты с ЭГ имеют высокий риск низкотравматичных переломов, при этом тяжесть гиперкорти-цизма, выраженная в уровне кортизола в сыворотке крови в вечернее время является наиболее значимым предиктором переломов. Пациенты с ЭГ, осложненным низкотравматичными переломами, испытывают больше боли и вследствие этого имеют больше функциональных ограничений. Следовательно, для больных с высоким уровнем кортизола в вечер-нее время в крови оправданно раннее профилактическое лечение остеопороза. Granins are glycoproteins presented in granules of neuroendocrine cells, they regulate secretory pathway that is responsible for controlled delivery of peptides, hormones and neurotransmitters, and growth factors, organization of the granular matrix, and the mechanisms of administration after hormone secretion The highest accuracy has been observed in neuroendocrine tumors characterized by an intense secretory activity, but its specificity and sensitivity remain very high also in nonfunctioning tumors. The granins application as the markers of nonfunctioning pituitary adenomas would allow the early disease detection, assessment the secretory capacity of tumor, and differential diagnosis with autoimmune forms of pituitary lesions. Furthermore, granins can be used as markers of related metabolic disorders, especially of cardiovascular diseases.
The review of literature details the issues of genetics, the specific features of inheritance, the clinical picture and treatment of pseudohypoparathyroidisms (PHPT). In practice, clinicians more frequently deal with type 1 PHPT and the diagnosis of this type creates no significant problems. However, despite the low prevalence of the other types of PHPT - 1b, 1c, and 2, the diseases may run with noticeably clinical symptoms and present a significant problem in the context of diagnosis and treatment. This review may be of concern to both clinicians and geneticists who are interested in this problem.
