SamenvattingIn dit onderzoek worden twee methoden om spierkracht te testen vergeleken:1. de gebruikelijke manuele-weerstandstest en 2. een methode die gebruik maakt van een band om weerstand te geven, waarbij met behulp van een gemodificeerde bloeddrukmeter de isometrische kracht van de elleboogflexoren wordt gemeten.Het gebruik van een gemodificeerde bloeddrukmeter bij isometrische spierkrachttests is eerder beschreven.Bij ons onderzoek plaatsten wij de bloeddrukmeter tussen de onderarm van de proefpersoon en de weerstand, dat wil zeggen de hand van de tester bij methode 1 of de band bij methode 2. Het protocol werd zo opgezet, dat elk van de tien gezonde vrouwelijke proefpersonen die aan het onderzoek meededen, bij elke testmethode en elk van de twee testers drie maximale isometrische contracties uitvoerden. De uitkomsten van de manuele-weerstandstest toonden, vergeleken met de bandfixatiemethode, consequent een grotere kracht (p < 0.01). Omdat de excentrische spiercontractie een significant grotere kracht levert dan hetzij isometrische, hetzij concentrische contracties, kan deze uitkomst worden verklaard uit het opwekken van een rekprikkel tijdens de manuele-weerstandstest.De twee methoden waren beide zeer betrouwbaar. Een tester was zeer betrouwbaar bij beide methoden, terwijl de andere tester zeer betrouwbaar was bij de uitvoering van methode 1 en matig betrouwbaar bij methode 2. Noch voor de testers, noch voor de proeven werden significante verschillen waargenomen (p >0.05). Wij concludeerden dat bij het klinisch onderzoek alleen gelijksoortige tests mogen worden vergeleken en dat objectieve methoden zijn te verkiezen boven semikwantitatieve methoden als de manuele-weerstandstests. Verder onderzoek is nodig om de rol van spierverlenging bij de gebruikelijke manuele-weerstandstest uit te diepen.
InleidingHet manueel testen van spieren is in de fysiotherapie vele tientallen jaren de hoeksteen geweest van het spierkrachtonderzoek. De spierkrachtschaal varieert van o tot 5:0 is ge´e´n spieractiviteit, 1 is palpabele activiteit, 3 geeft aan dat de spier een beweging kan uitvoeren als de zwaartekracht is uitgeschakeld en 5 heeft betrekking op normale spierkracht. De waarden 2 en 4 worden gebruikt om aan te geven dat de spierkracht valt tussen 1 en 3, respectievelijk 3 en 5. Auteurs meldden dat deze schaal vele beperkingen had. Bijvoorbeeld: de schaal is een semikwantitatieve ordinale schaal, waarbij de graden hoger dan 3 alineair zijn en al meer dan dertig jaar geleden pleitte Baesley (1956) voor meer objectieve methoden om spieren te testen.
