В статье рассмотрены различные методы лечения ахалазии кардии -эндоскопическая баллонная дилатация, направ-ленная на расширение нижнего пищеводного сфинктера, введение ботулинического токсина, что способствует снижению тонуса кардии, малоинвазивные эндоскопические методики в сочетании с медикаментозной терапией, а также хирургиче-ское лечение больных с IV стадией заболевания, которое позволяет получить хороший и удовлетворительный клинический результат лечения у 98,2 % больных. В данной статье также представлен оптимальный алгоритм обследования больных с подозрением на ахалазию кардии. Для установления диагноза использовали рентгенологический метод исследования, эндоскопический метод исследования. Все используемые методы позволяют своевременно и правильно установить диа-гноз и стадию развития заболевания. При стадировании заболевания в России пользуются преимущественно классифи-кацией, предложенной Б.В. Петровским в 1962 г., основанной на рентгенологическом исследовании пищевода, в силу ее доступности и простоты интерпретации полученных результатов. Также в статье представлен совершенно новый в Россий-ской Федерации эндоскопический метод лечения ахалазии кардии -пероральная эндоскопическая миотомия (POEM, pero ralesophagealmyotomy). В нашей клинике данный вид эндоскопической операции выполняется с 2014 г. взрослым и впервые в России в 2016 г. проведен ребенку. Описан клинический пример ахалазии кардии у ребенка, методы диагностики, меди-каментозное лечение и лечение баллонной дилатацией, после чего у ребенка был рецидив заболевания. В дальнейшем ему выполнена пероральная эндоскопическая миотомия. Эта на сегодняшний день перспективная, эффективная, малоинвазив-ная эндоскопическая операция, в лечении ахалазии кардии как у взрослых, так и у детей.Ключевые слова: ахалазия кардии у ребенка; баллонная дилатация; пероральная эндоскопическая миотомия (ПОЭМ). The article deals with various methods of treatment of achalasiocardia, e. g., endoscopic balloon pneumatic dilation extending lower esophageal sphincter, injection of botulinic toxin that decreases cardia tone, low-invasive endoscopic methods in combination with medicinal therapy as well as surgical treatment of patients at the 4th stage of the disease that permits to reach good or satisfactory clinical result in 98,2% of patients. The present study also introduces an optimal algorithm for examination of patients with suspected achalasiocardia. X-ray diagnostics and endoscopic examination were used for verification of the diagnosis. All these methods ensure timely and correct diagnostics ascertaining also the stage of the disease development. Stage-vise diagnostics in Russia is mostly based on classification suggested in 1962 by B.V. Petrovsky. This classification is founded upon X-ray examination of the esophagus due to its simplicity and availability. The article also presents a completely novel approach for the treatment of achalasiocardia in Russia, i. e. peroral esophageal myotomy (POEM). Objective. To improve the outcomes of percutaneous endobiliary interventions through prediction of postoperative complications and searching for minimally invasive methods of their treatment. Material and methods. Percutaneous en dobiliary interventions have been performed in 2458 patients for the period from 2006 to 2018 including bile duct stenting in 1895 patients. Results. Early postoperative complications included hemobilia (2.1%), abdominal bleeding (0.2%), duodenal bleeding (0.1%), pleuritis (0.9%), abdominal bile leakage (1.5%) and peritonitis (0.8%), liver hematoma (0.7%), right-sided pleuritis (0.9%), pancreatitis (13.8%). In our opinion, these complications are serious and require immediate intensive care and invasive repair. Mortality rate was 8.4% among all patients with early complications and 0.9% among those after endobiliary interventions. Delayed complications of percutaneous transhepatic interventions are migration of stent or drainage tube, drain incrustation, malignant invasion of stent, recurrent cholangitis, cholangiogenic liver abscesses, abdominal abscesses, sepsis, fragmentation of drain or stent, portal vein thrombosis. The most severe late complications (sepsis and portal vein thrombosis) resulted mortality rate 50–60%. Conclusion. Combined dual (ante- and retrograde) biliary approach and control throughout minimally invasive surgery eliminate disadvantages of individual accesses, reduce invasiveness, postoperative morbidity and mortality.
