Резюме. Обзор посвящён роли протеолитического разрушения эластических волокон, входящих в состав различных тканей и органов, под действием фермента эластазы. Этот фермент является регулятором воспаления, причем в разных ситуациях он может выступать и как провоспалительный, и как противовоспалительный агент. Дисбаланс в регулировании активности эластазы играет важную роль в патогенезе муковисцидоза, острого респираторного дистресс-синдрома, бронхоэктатической болезни, хронической обструктивной болезни легких, сахарного диабета 2 типа, атеросклероза, артериальной гипертензии. В обзоре приведены данные многочисленных исследований, доказывающих важность определения эластазной активности, концентрации эластин-производных пептидов (ЭПП) и антител к ним для прогноза развития заболеваний сердечно-сосудистой системы. Уровень эластазной активности, о котором косвенно свидетельствует количество эластин-ПРП, а также антител к ним, может стать в перспективе стандартным клинико-лабораторным показателем, так как, исходя из результатов многочисленных исследований, он отражает степень нарушений эластических волокон различных органов. Уровень эластазной активности является одним из показателей, который наравне с рутинными лабораторными (ХС-ЛПНП, холестерин липопротеин высокой плотности (ХС-ЛПВП) и др.) и клиническими (АД, ИМТ, возраст и др.) данными, может быть полезным для комплексной характеристики тяжести состояния пациента на доклиническом этапе.
