Поясничный спинальный стеноз-распространенное заболевание у пожилых лиц (старше 60 лет). К обычным хирургическим вариантам лечения поясничного стеноза относятся открытая декомпрессионная ламинэктомия, фораминотомия и спондилодез. В настоящее время золотым стандартом лечения поясничного стеноза, резистентного к консервативному лечению, является ламинэктомия с сохранением фасеток. Цель: на основании данных литературных источников провести оценку результатов исследований эффективности и преимущества UBE (Unilateral Biportal Endoscopy, унилатеральная бипортальная эндоскопическая хирургия) по сравнению с другими методами декомпрессии поясничного спинального стеноза. Результаты. Метаанализ проведенных исследований показал, что преимущество UBE заключается в возможности предотвращения атрофии параспинальных мышц благодаря уменьшению мышечной диссекции и ретракции по сравнению с открытой декомпрессией. Данная методика позволяет уменьшить травматизацию нервных структур, поскольку возможно тонкое и точное выполнение манипуляций благодаря значительно увеличенным изображениям. Благодаря широкому углу обзора через эндоскоп относительно простым является выполнение процедуры по декомпрессии контралатеральной стороны. Выводы. Методика UBE обладает рядом преимуществ по сравнению с другими малоинвазивными методиками: уменьшение повреждений заднего связочного аппарата и мягких тканей, полная декомпрессия поврежденных структур и меньший риск развития инфекции.
В аспекті ортопедо-травматологічної допомоги звертають на себе увагу омолодження контингенту з остеохондрозом грудного відділу хребта, який призводить до стійкого больового синдрому – торакалгій, що потребує диференціальної діагностики із захворюваннями серцево-судинної системи та вибору комплексного лікування. Мета. Удосконалитиреабілітаційну програму хворих з торакалгіями на фоні остеохондрозу грудного відділу хребта. Матеріали та методи. Було проаналізовано результати обстеження 50 пацієнтів. Обсяг обстеження передбачав загальні клінічні дані, рентгенографію грудного відділу хребта, магнітно-резонансну томографію, електрокардіографію, ехокардіографію, функціональні проби, анкетування. Результати. Запропонована удосконалена програма відновного лікуванняхворих, суть якої в персоніфікованому підходу до нього з урахуванням наявності ізольованої чи поєднаної з серцево-судинними захворюваннями патології, а також ступеня вираженості больового синдрому, як одного з основних критеріїв звернення пацієнтів за допомогою. Висновки. Використання удосконаленої програми реабілітації дозволило достовірно знизити больовий синдром у пацієнтів обох груп, результат продовжував набувати позитивної динамікичерез 1,5 міс. поспіль і залишався практично стабільним впродовж наступних 1,5 міс.
Одной из актуальных проблем современной ортопедии является лечение болевого синдрома, вызванного грыжей межпозвоночного диска. Актуальным методом лечения является эпидуральный адгезиолизис. Цель работы — оценить эффективность применения комбинированного медикаментозного лечения (эпидурального адгезиолизиса) и кинезотерапии у пациентов с грыжами межпозвоночных дисков поясничного отдела позвоночника. В исследовании приняли участие 54 пациента — 24 женщины и 30 мужчин в возрасте от 25 до 47 лет с клиническими и морфологическими проявлениями грыж межпозвоночных дисков поясничного отдела позвоночника. Применение кинезотерапии после проведенного эпидурального адгезиолизиса у пациентов с грыжами межпозвоночных дисков поясничного отдела позвоночника позволяет повысить и продлить эффективность процедуры.
Lumbar spinal stenosis is a disease in which degenerated discs, ligamentum flavum, facet joints, while aging, lead to a narrowing of the space around the neurovascular structures of the spine. This article presents a meta-analysis of literature data on epidemiology, causes, pathogenesis, diagnosis and various types of treatment of lumbar spinal stenosis.
Мета. Оцінити ефективність трансфорамінальної ендоскопічної мікродискектомії, виявити недоліки та переваги даного методу в порівнянні зі стандартними методиками, на основі отриманих даних визначити основні показання та протипоказання до даної процедури. Матеріали і методи. Проаналізовано 290 історій хвороб пацієнтів з грижами поперекового відділу хребта, які перебували на лікуванні в Обласному центрі ортопедії, травматології та вертебрології Рівненської обласної клінічної лікарні та відділенні хірургії хребта Інституту травматології та ортопедії НАМН України з квітня 2016 по квітень 2018 р. Серед пацієнтів було 156 жінок та 134 чоловіки віком від 20 до 84 років. Усім хворим в передопераційному періоді було проведено магнітно-резонансну (МРТ) або комп’ютерну (КТ) томографію поперекового відділу хребта, функціональну рентгенографію, загально-клінічні обстеження. Результати. В післяопераційному періоді відповідно до міжнародного опитувача якості життя Oswestry середній показник щодо досліджуваних хворих становив 21,1%, що відповідає доброму результату. Больовий синдром перед операцією за візуально-аналоговою шкалою був оцінений у 8,5 бала, а через 6 міс після операції - у 1,5 бала (переважала люмбалгія і майже не було корінцевого болю). Накопичений значний практичний досвід виконання ендоскопічної трансфорамінальної мікродискектомії засвідчив, що хірург може зіткнутися з певними технічними труднощами у разі краніальної або каудальної міграції секвестру, в ряді спостережень доступ на рівні L5 - S1 затруднений через високе розташування гребеня клубової кістки, але все-таки за наявності правильних показань ця методика дає змогу в короткі строки позбутися компресійного корінцевого синдрому.
