Objective: To critically examine the existing Brazilian and International scientific literature regarding the influence of short lingual frenulum over growth and development of the stomatognathic system, as well as how it impacts the achievement of the shape-function balance.Data sources: An electronic literature search was conducted in databases, including MEDLINE/PubMed, Google Scholar, LILACS, SciELO, and ScienceDirect, using the key words “lingual frenum” and “development”, as well as their equivalents in Brazilian Portuguese. The literature search yielded 51 papers published between January 1997 and the present date; 14 articles of clinical trials were selected for meeting the inclusion criteria and were read in full.Data synthesis: The integrated literature review supported the proposition that some malocclusions are closely related to the presence of ankyloglossia and, although very few clinical trials on this topic have been published so far, there is a consensus among authors concerning the negative effects of functional imbalances over the stomatognathic system’s proper growth and development. Half of the studies found state that surgical interventions for releasing the lingual frenum are both safe and effective, concerning improvement in breastfeeding scores. Moreover, 4 out of the 14 studies included in this integrated review, report a negative influence of ankyloglossia over the orofacial muscular system.Conclusions: There is a consensus among the authors concerning the negative effects of lingual frenulum’s anatomic and functional alterations over craniofacial growth and development. The opinion about the early surgical intervention, however, is not unanimous.
ResumoObjetivos: a proposta deste estudo prospectivo, transversal e multicêntrico foi correlacionar as alterações morfológicas das vértebras cervicais C2, C3 e C4, com a idade cronológica e comparar com a idade esqueletal dos ossos da mão e punho; e verificar se existem diferenças no padrão de crescimento ósseo entre os gêneros masculino e feminino. Metodologia: a amostra foi constituída por 140 pacientes, 74 do gênero feminino e 66 do masculino. A avaliação das alterações das vértebras cervicais foi feita pelo método de Hassel e Farman modificado por Baccetti, Franchi e McNamara para 5 estágios que se correlacionam com o crescimento puberal em telerradiografias cranianas em norma lateral. As radiografias da mão e punho avaliaram os eventos de ossificação por meio do método de Greulich e Pyle. Resultados: os resultados demonstraram correlação dos estágios de maturação das vértebras cervicais com a idade cronológica; e correlação positiva e moderada, estatisticamente significante entre os dois métodos com r = 0,6326 (IC = 95%) e p < 0,0001. Conclusões: desta forma, pôde-se concluir que a idade aumenta proporcionalmente em relação aos estágios de maturação, sendo que a maioria dos pacientes da amostra encontrou-se no estágio II, considerado bom para o início da terapêutica ortodôntico/ortopédica. O nível de correlação entre os métodos demonstrou que a avaliação dos estágios de maturação das vértebras cervicais é um método adicional útil e confiável na determinação do estágio de crescimento facial nas crianças em crescimento puberal. O gênero feminino apresentou estágios de crescimento mais precoces que o gênero masculino, atingindo o início e o final do surto de crescimento puberal em idade cronológica mais inferior.Palavras-chave: Maturação óssea. O presente trabalho teve como objetivo avaliar se a mineralização dos segundos molares inferiores permanentes podem ser usados como parâmetro para classificar a idade biológica do indivíduo. A amostra foi constituída por 129 radiografias panorâmicas, sendo 71 indivíduos do sexo feminino e 58 indivíduos do sexo masculino, na faixa etária de 7 anos à 12 anos e 1 mês. Para a análise da mineralização dental foi utilizada a tabela proposta por Nolla (1960) com as radiografias sobre o negatoscópio de marca Softline. Os resultados da análise foram documentados numa planilha do programa Microsoft Excel 2010 contendo o nome completo, data de nascimento, data da tomada radiográfica, idade em anos e meses, número do prontuário, estágio de Nolla (1960) lado direito e lado esquerdo. Foi realizada a análise estatística e podemos concluir que em nossa amostra não foi encontrado dimorfismo sexual, que a mineralização dentária ocorre de forma similar do lado direito e esquerdo, e que a mineralização dos segundos molares inferiores permanentes podem ser usadas como parâmetro para estimar a idade biológica e cronológica de um indivíduo.
ResumoO objetivo deste estudo foi avaliar as alterações dentárias na maxila e as possíveis alterações verticais decorrentes da utilização do Pendex como método de tratamento ortodôntico para distalização dos molares superiores. Uma amostra por conveniência de 10 pacientes de ambos os sexos (9,3 -28,8 anos; média: 15,3 anos), com necessidade de tratamento ortodôntico, foi selecionada para participação desse estudo na Faculdade de Odontologia da Universidade Paulista -FOUNIP. Duas radiografias em norma lateral foram obtidas de cada paciente, uma ao início do tratamento (T1) e outra após a distalização dos molares dos primeiros molares superiores (T2). Os cefalogramas foram traçados e mensurados pelo mesmo operador e os dados de T2 e T1 foram comparados entre si, para cada paciente. As seguintes medidas cefalométricas foram avaliadas: distância de PTV ao 1 o molar superior (A6-PTV), ao 1 o pré molar superior (A4-PTV) e ao incisivo central superior (A1-PTV), inclinação do 1 o molar superior (A6.PP), inclinação do 1 o pré molar superior (A4.PP) e a inclinação do incisivo central superior (A1.PP) em relação ao plano palatino (PP), ângulo interincisivo (A1.B1); altura facial inferior (AFI); eixo facial (EF); altura facial total (AFT). Os dados obtidos foram submetidos ao teste "t" para amostras pareadas (5%). Os valores médios e desvios-padrão obtidos para cada medida (T2-T1) foram: A6-PTV= -4,9±6,5; A4-PTV=1,2±4,1; A1-PTV= -1,4±7,3; A6.PP=9,4±7,7; A4.PP=-3,6±6,7; A1.PP=9,4±7,7; A1.B1=-4,9±13,7; AFI=1,7±2,1; EF=-1,0±2,4; AFT=1,0±2,7. Os resultados mostraram significância estatística para: A6-PTV (p=0,041); A6.PP (p=0,004) e AFI (p=0,031) (p>0,05). Pôde-se concluir que Pendex mostrou-se eficiente na correção da Classe II e que seus efeitos foram limitados às estruturas dentoalveolares.Descritores: Má Oclusão de Angle Classe II; Ortodontia; Cefalometria Craniana. AbstractThe objective of this study was to evaluate the dental changes in the maxilla and the possible facial vertical alterations of patients after using a Pendex appliance for maxillary molar distalization during their orthodontic treatment. Objecti ve: To compare diff erent verti cal measurements in Class II subjects with anterior open bite between the neutroverted and retroverted facial types and to identi fy which bone structure (maxilla and/or mandibula) is more related to open bite malocclusion based on the reference values of Rickett s and Schwarz analyses. Method: Based on the orthodonti c documentati on fi les of the Paulista University (UNIP) Specializati on Course (São Paulo, SP, Brazil), 300 lateral teleradiographs were selected from subjects of both genders aged 8 to 13 years, who presented decreased overbite (≤ 1mm), which was the initi al inclusion criterion. The teleradiographs were selected according to Schwarz's Class II patt ern and according to Rickett s's facial type, as follows (n=15): Group 1: neutroverted and Group 2: retroverted. The cephalograms were drawn from the teleradiographs. For each group, the following cephalometric variables were measured: denti ti on height, maxillary height, total facial height, palatal plane inclinati on (palatal plane/Frankfurt plane), mandibular plane angle, mandibular arch and cranial basal angle (BaNa)/ palatal plane. The data were stati sti cally analyzed by the Student's t-test (5%). Results: There were stati sti cally signifi cant diff erences between the groups (p<0.05) for the variables denti ti on height, total facial height, mandibular plane angle and mandibular arch between the groups. The groups had stati sti cally similar results (p>0.05) only for the variables relati ve to the palatal plane. Conclusion: Both the maxilla (represented by the palatal plane), due to its anti clockwise rotati on, and the mandible, due to its clockwise rotati on, are involved in the occurrence of malocclusion and the retroverted facial type presents more severe cephalometric characteristi cs of open bite. There is correlation between body growth and maturation of cervical vertebrae in rats. Despite the continuous development of concavities, it was not possible to clearly identify the 5/6 stages as in studies of cervical vertebrae maturation in humans.
