EA at SP6 can immediately relieve menstrual pain and minimize the influence of pain on daily life compared with GB39 and an adjacent non-meridian point. The data preliminarily show the specificity of SP6 for the immediate pain relief of primary dysmenorrhea.
PurposeXEN gel stents are used for the treatment of open-angle glaucoma (OAG), including primary and secondary glaucoma that are uncontrolled by previous medical therapy and cases with previous failed surgery. Our aim was to systematically review of the clinical data of currently published ab-interno XEN gel stents with an emphasis on intraocular pressure (IOP), antiglaucoma medication outcomes, and safety profiles.MethodsWe analyzed all of the publications (MEDLINE, EMBASE, Cochrane Library) on the ab-interno XEN gel stent to evaluate the reduction in IOP and antiglaucoma medications following the procedure. The primary outcomes measured for the meta-analysis were reduction in IOP and anti-glaucoma medications. The secondary outcome were adverse events. For each study, we used a random effects analysis model to calculate the mean difference and 95% confidence intervals for the continuous results (reduction in IOP and antiglaucoma medications) using the inverse variance statistical method.ResultsFive hundred twenty-seven articles were checked and 56 studies were found to be relevant with a total of 4,410 eyes. There was a significant reduction in IOP as well as in the number of medications required in patients treated with ab-interno XEN implant either alone or combined with cataract surgery. This new treatment for various types of glaucoma reduced the IOP by 35% to a final average close to 15 mmHg. This reduction was accompanied by a decrease in the number of antiglaucoma medications in all the studies, approximately 2 classes of medication at the price of more needlings. The overall complete success rate was 21.0–70.8% after 2 years using strict criteria originally designed to record success rate in filtration surgery. The incidence of complications vision-threatening was low at <1%.ConclusionsXEN gel stent was effective and safe for primary and secondary OAG. Further studies should be performed to investigate the impact of ethnicity on the success and failure rate after XEN implantation.
PurposeCrohn's disease (CD) has been known to cause both abdominal pain alongside functional and structural alterations in the central nervous system (CNS) in affected patients. This study seeks to determine the alternations of metabolites in the bilateral anterior cingulate cortex (ACC) of CD patients with abdominal pain by using proton magnetic resonance spectroscopy (1H-MRS) to further explore the neural mechanism.MethodsSixteen CD patients with abdominal pain and 13 CD patients without abdominal pain, were recruited alongside 20 healthy controls (HCs) for this study. Clinical evaluations, including the 0–10 Visual Analogue Scale (VAS) of pain, Hospital Anxiety and Depression Scale (HADS) and Crohn's Disease Activity Index (CDAI), were evaluated prior to MR scanning. This study selected the bilateral ACC as the region of interest (ROI). The metabolites of the bilateral ACC were quantitatively analyzed by LCModel and Gannet. A independent sample t-test and one-way analysis of variance (ANOVA) were performed for statistical analysis. Spearman correlation analyses were performed to examine the relationship between the metabolite levels and clinical evaluations.ResultsThe results indicated that CD patients with abdominal pain exhibited significantly higher levels of Glutamate (Glu)/(creatine + phosphocreatine, total creatine, tCr) over CD patients without abdominal pain, and HCs (p = 0.003, 0.009, respectively) in the bilateral ACC. The level of (Glutamate + Glutamine, Glx)/tCr of pain CD group was higher than non-pain CD group (p = 0.022). Moreover, within the pain CD group, Glu/tCr and Glx/tCr levels correlated strongly with the VAS scores of pain (ρ = 0.86, 0.59 respectively, p < 0.05). Meanwhile, the results indicates that CD patients with abdominal pain have significantly lower levels of γ-aminobutyric acid plus (GABA+)/tCr (p = 0.002) than HCs. To some extent, CDAI demonstrated a trend of negative correlation with GABA+/tCr levels (p = 0.088, ρ = −0.60).ConclusionThe neural mechanism of CD patients with abdominal pain in pain processing is tightly associated with neurochemical metabolites. An imbalance in Glu and GABA may play a key role in abdominal pain processing for patients with CD. This mechanism of pain may associate with the intestinal microbiota on the brain-gut axis.
The present study aimed to investigate the value of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) combined with quantitative analysis of diffusion weighted imaging (DWI) for the diagnosis of prostate cancer (PCa). A total of 81 patients with prostatic diseases, including PCa (n=44) and benign prostatic hyperplasia (BPH, n=37), were imaged with T1 weighted imaging (T1WI), T2 weighted imaging (T2WI), DWI and DCE-MRI. The blood vessel permeability parameters volume transfer rate constant (K trans ), back flow rate constant (K ep ), extravascular extracellular space volume fraction (V e ), plasma volume fraction (V p ) and apparent diffusion coefficient (ADC) were measured, and compared between the two groups. The efficiency of these tools for the diagnosis of PCa was analyzed by receiver operating characteristic curve analysis. The efficiency of ADC combined with blood vessel permeability parameters in the diagnosis of PCa was analyzed by logistic regression. The correlation between these parameters and the Gleason score was evaluated by Spearman correlation analysis in the PCa group. The results demonstrated that, compared with the BPH group, K trans , K ep , V e and V p were higher, and ADC was lower in the PCa group (P<0.05). The combination of K ep and ADC offered the highest diagnosis efficiency [area under the curve (AUC=0.939)]. However, the combination of three parameters did not significantly improve the diagnostic efficiency. A subtle improvement in diagnostic efficiency was observed when four parameters (K trans + K ep + V e + ADC) were combined (AUC=0.940), which was significantly higher than with one parameter. The ADC value of the PCa group was negatively correlated with the primary Gleason pattern, secondary Gleason pattern and the total Gleason score in PCa (r=−0.665, −0.456 and −0.714, respectively; P<0.001). The V p in the PCa group was slightly negatively correlated with the primary Gleason pattern of PCa (r=−0.385; P<0.05); however, no significant correlation was found with secondary Gleason pattern and the total Gleason score. The present study revealed that the combination of DCE-MRI quantitative analysis and DWI was efficient for PCa diagnosis. This may be because DCE-MRI and DWI can noninvasively detect water motility in tumor tissues and alterations in permeability during tumor neovascularization. The present study demonstrated that K ep and ADC values may be used as predictive parameters for PCa diagnosis, which may help differentiate benign from malignant prostate lesions.
Glioma is the most common primary malignant brain tumor in adults. It accounts for about 75% of such tumors and occurs more commonly in men. The incidence rate has been increasing in the past 30 years. Moreover, the 5-year overall survival rate of glioma patients is < 35%. Different locations, grades, and molecular characteristics of gliomas can lead to different behavioral deficits and prognosis, which are closely related to patients' quality of life and associated with neuroplasticity. Some advanced magnetic resonance imaging (MRI) technologies can explore the neuroplasticity of structural, topological, biochemical metabolism, and related mechanisms, which may contribute to the improvement of prognosis and function in glioma patients. In this review, we summarized the studies conducted on structural and topological plasticity of glioma patients through different MRI technologies and discussed future research directions. Previous studies have found that glioma itself and related functional impairments can lead to structural and topological plasticity using multimodal MRI. However, neuroplasticity caused by highly heterogeneous gliomas is not fully understood, and should be further explored through multimodal MRI. In addition, the individualized prediction of functional prognosis of glioma patients from the functional level based on machine learning (ML) is promising. These approaches and the introduction of ML can further shed light on the neuroplasticity and related mechanism of the brain, which will be helpful for management of glioma patients.
