Introduction. Lower back pain (LBP) is almost a problem of civilizations. Quite often, it is a consequence of many years of disturbed distribution of tension within the human body caused by local conditions (injuries, hernias, stenoses, spondylolisthesis, cancer, etc.), global factors (postural defects, structural integration disorders, lifestyle, type of activity, etc.), or systemic diseases (connective tissue, inflammation, tumours, abdominal aneurysm, and kidney diseases, including urolithiasis, endometriosis, and prostatitis). Therefore, LBP rehabilitation requires the use of integrated therapeutic methods, combining the competences of interdisciplinary teams, both in the process of diagnosis and treatment. Aim of the Study. Given the above, the authors of the article conducted meta-analysis of the literature in terms of integrated therapeutic methods, indicating the techniques focused on a holistic approach to the patient. The aim of the article is to provide the reader with comprehensive knowledge about treating LBP using noninterventional methods. Material and Methods. An extensive search for the materials was conducted online using PubMed, the Cochrane database, and Embase. The most common noninterventional methods have been described, as well as the most relevantly updated and previously referenced treatment of LBP. The authors also proposed noninvasive (measurable) diagnostic procedures for the functional assessment of the musculoskeletal system, including initial, systematic, and cross-sectional control. All figures and images have been prepared by the authors and are their property. Results. This review article goes beyond combining a detailed description of each procedure with full references, as well as a comprehensive discussion of this very complex and troublesome problem. Conclusions. Lower back pain is a serious health problem, and this review article will help educate physicians and physiotherapists dealing with LBP in the options of evidence-based treatment. Ultimately, the article introduces and postulates the need to systematize therapeutic procedures in LBP therapy, with a long-term perspective.
Background. Considering the enormous risk of fractures in the course of osteoporosis in the area of the feet, an important aspect of prophylaxis is periodic and, in special cases, ongoing monitoring of defects and deformations as well as pressure distribution. The purpose of this article is to indicate the role of the examination of posture and pressure distribution during standing, postural balance, and gait, in the prevention of fatigue fractures in the course of osteoporosis, based on the literature review and examples of patients. Methods. The manuscript consists of two parts; it has a review-analytical character. The first part reviews the literature. The data were obtained using the MEDLINE (PubMed), as well as Cochrane and Embase databases. The database review was carried out focusing mainly on English-language publications, while taking into account the topicality of scientific and research works in the area of osteoporosis. The problem of multiaspects in the area of bone density was pointed out. Considering the above, in the second part, the authors analyzed 11 exemplary patients with osteoporosis, referring to the assessment of foot and lower limb defects using traditional posturological methods and including pedobarography to diagnostic procedures that are used in the assessment of pressure distribution, standing and moving, and an attempt to balance. Results. Analysis of the research and scientific literature proved the lack of unambiguous diagnostic procedures of the locomotor system recommended for the prevention of fatigue fractures in the course of osteoporosis. The main diagnostic recommendations are imaging tests (most often X-ray), which are recommended in the case of specific clinical symptoms. The analysis of exemplary patients with osteoporosis showed numerous disorders in the distribution of pressure in the plantar part of the feet, which are related, among other things, with their individual defects and lower limbs. Conclusions. Detailed posture diagnostics and gait estimation, along with the analysis of pressure distribution within the feet are a very important aspect of the prevention of structural degradation and fatigue fractures within the feet. An important postulate for further research and scientific work is the elaboration of the procedures that will serve the preventive diagnostics of the locomotor system, aimed at early detection of threats of fatigue fractures.
Increased pressure on the heel apophysis is often implicated as a cause of paediatric heel pain. However, there are few reports on the causes of the increased pressure and its origin. Therefore, the aim of this study was to analyse the distribution of pressure on the feet in children with heel pain. The study included 33 paediatric patients with non-traumatic heel pain, i.e., 24 boys (73%) and 9 girls (27%), aged on average 11.2 years (±3 years). Pedobarographic diagnostics proved a decrease in the pressure on the heels in relation to the ground and the transfer of the projection of the centre of gravity to the forefoot. While standing, the average contribution of the pressure on the heel was 0.52, SD = 0.14 in children with normal and reduced weight. In overweight children, the average pressure on the heel was higher (0.60, SD = 0.08), but the small number of children with this characteristic (n = 4) did not allow conclusions to be drawn in this area. Heel underload was also demonstrated during gait. However, the assessment of this aspect requires additional observational analyses in the field of propulsion and gait phases. The reduced pressure on the heel promotes apophysis traction, causing intracanal compression. Studies have shown that the causes of apophysis traction may be postural defects (in particular, forward inclination of body posture) and overpronation of the foot, or defects in the metatarsal area.
Although it enables a quick evaluation, medical diagnostics of the human myofascial-skeletal system is not always used to its full capabilities. This often hampers the objective assessment of a dysfunction and limits treatment options. Dysfunctions of the stomatognathic system, of different aetiology, are becoming more prevalent. Many scientific studies are in to relationships between organs and systems of the human body that can cause dysfunctions of the temporomandibular disorders. Studies investigating correlations between a myofascial-skeletal disorder and temporomandibular disorders are to be found in the literature. To achieve a good understanding of aetiology of these dysfunctions, a holistic view of a patient is advisable, which takes into consideration the fact that the temporomandibular joint is an integral part of the whole body. This study is a review of the literature, supported by own examples on investigating the relationship between myofascial-skeletal disorders and a temporomandibular disorder, which, in the light of current medical knowledge, is attributed to the phenomenon of biotensegrity. A conclusion has been put forward on the basis of 136 scientific reports that a dependency exists between the said dysfunctions.
Zmiany w obrębie zespołu stopy cukrzycowej (ZSC), powstałe w wyniku wadliwej dystrybucji nacisków, w literaturze badawczo-naukowej dotyczą zmian neuropatycznych oraz ich następstw, tj. wad, uszkodzeń strukturalnych czy zwyrodnień. Istnieje wiele dowodów dotyczących przyczynowości wzmożonego nacisku na powstanie ran w obrębie części podeszwowej stóp. Brak jest jednak badań, które wskazywałyby na jednoznaczny związek wzmożonej dystrybucji nacisku z niedokrwieniem. Mając na uwadze patofizjologiczne podstawy chorób krążenia w przebiegu cukrzycy, wydaje się, że nacisk może wywoływać niedokrwienie kończyn dolnych. Najczęściej niedokrwienie dotyczy bowiem dystalnych części ciała, w szczególności kończyn dolnych, począwszy od podudzia, na co wpływ ma przede wszystkim czynnik grawitacyjny. Schorzenie rozwija się najczęściej u pacjentów z cukrzycą, u których w przebiegu długotrwałej hiperglikemii dochodzi do szeregu zmian, m.in. w układzie krążenia. Nie można jednak pominąć faktu, iż ciasnota powięziowa, nierzadko powstająca w przebiegu wad, deformacji lub innych problemów narządu ruchu, może być istotnym czynnikiem determinującym zaburzenia krążenia. Tym samym wzmożony nacisk, nierzadko współistniejący z wadliwą dystrybucją nacisku, może być ważną determinantą kompresji strukturalnej. Ten istotny czynnik u osób chorych na cukrzycę może być czynnikiem współistniejącym, a nawet determinującym powstanie zmian w stopie pacjenta z cukrzycą. Cel Celem niniejszej pracy badawczo-naukowej jest analiza przypadków ZSC ze zdiagnozowanymi zmianami niedokrwiennymi o zróżnicowanym ich umiejscowieniu. Materiał i metody Analizie poddano 8 pacjentów ze zdiagnozowanym niedokrwiennym ZSC. Badanie dystrybucji nacisków oraz pomiary antropometrii realizowano za pomocą: pedobarografii (podczas stania, podczas chodu, ocena równowagi; pedobarograf E.P.S./R2, oprogramowanie BIOMECH Studio), goniometrii elektronicznej (czujnik biokinematyczny WIVA® Science, oprogramowanie BIOMECH Studio), skanera podologicznego (PodoScan 2D, oprogramowanie BIOMECH Studio), badania podoskopowego. Wyniki i wnioski Analiza studiów przypadków wykazała, iż wzmożony nacisk może być przyczyną lokalnych stanów niedokrwienia, jak w opisanych przypadkach, np. niedokrwienia palców czy łożyska paznokcia, i jest on powiązany m.in. z wadami lub dysfunkcjami stóp oraz zaburzeniami chodu; w przypadkach deformacji palców, paznokci, przodostopia prace badawczo-naukowe powinny być kontynuowane w sposób randomizowany. Analiza studiów przypadków, których niedokrwienie zlokalizowane było powyżej stawu skokowo-goleniowego i obejmowało stopę, wykazała współistnienie wzmożonej dystrybucji nacisków oraz zmian przeciążeniowych, które również związane były bezpośrednio z wadami i dysfunkcjami stóp. Nie można stwierdzić, że niedokrwienie jest bezpośrednią przyczyną powstawania zmian przeciążeniowych (lub na odwrót). Ze względu na fakt współistnienia wad oraz wpływu niedokrwienia na degradację struktur, tym bardziej w przypadku niedokrwienia w przebiegu ZSC, istotne jest diagnozowanie stóp, w szczególności badanie nacisku podczas stania oraz podczas chodu; w badaniach uwzględnić należy pedobarografię. Analizowane przypadki pacjentów wykazały częste występowanie zaburzeń chodu, tj. w 3 studiach przypadków badanie nie było możliwe do realizacji, natomiast w 2 wnioskowano na podstawie wyników częściowych, ze względu na zaburzone wzorce chodu, zmniejszony rytm, wadliwe przetaczanie stóp. U wszystkich analizowanych pacjentów ze zmianami niedokrwiennymi w ZSC stwierdzono znaczne zaburzenia równowagi, które istotnie wpływają na lokomocję, a w konsekwencji na kondycję. Analizowane studia przypadków wykazały znaczne ograniczenie mechanizmów ochronnych, a w przypadku pacjentów z zaburzeniami chodu związki dystrybucji nacisków ze zmianami przeciążeniowymi uwidoczniły się już podczas stania. Zważywszy na powyższe obserwacje, w przebiegu niedokrwienia bardzo istotnym aspektem jest włączenie rehabilitacji oraz zaopatrzenia odciążającego, z uwzględnieniem możliwej korekcji wad oraz poprawy równowagi i wzorców chodu.
