Background Cemento-osseous dysplasia is a benign fibro-osseous lesion of the tooth-bearing region of the jaws with a periodontal ligament origin. It appears predominantly in Black and Asian middle-aged females. Its importance is that it could mimic a periapical lesion in the early, translucent stage. Case presentation In this report a rare case of familial cemento-osseous dysplasia is presented: a 50-years old Caucasian woman with labial paraesthesia and radiological translucency around the roots of the mandibular incisors and the first molar teeth. The lesion around the first molar was diagnosed as periapical granuloma and a root canal treatment was carried out. The diagnosis of florid cemento-osseous dysplasia and the treatment plan based on two- and three-dimensional radiographic examinations were certified histologically after surgical removal of the lesion. We screened the family members - including the patient’s mother, daughter and son - and identified a periapical version of cemento-osseous dysplasia in the daughter. Our case highlights the difficulties of differential diagnosis of cemento-osseous dysplasia and other periapical pathologies. The inconsistencies in the present classification of cemento-osseous dysplasia are also discussed with a proposal for a different classification based on new aspects that would be very helpful in setting up a correct treatment plan. Conclusion Differentiation of endodontic and non-endodontic origin of radiolucency and distinguishing it from anatomical landmarks by appropriate clinical evaluation and using vitality testing can give an opportunity to prevent unnecessary endodontic treatment. The current categories of cemento-osseous dysplasia classification do not cover the early stage of a hereditary florid form of cemento-osseous dysplasia. Instead of anatomical location of the lesion, clinical and genetic features may be recommended as parameters of cemento-osseous dysplasia classification.
Our results suggest that CBCT images provide reliable volumetric information that can be depended on for artificial organ construction, and which may aid the guidance of the operator prior to or during the intervention.
BackgroundPravastatin, a known inducer of endothelial nitric-oxide synthase (eNOS) was demonstrated in human placenta, however the exact mechanism of it’s action is not fully understood. Since placental NO (nitric oxide) synthesis is of primary importance in the regulation of placental blood flow, we aimed to clarify the effects of pravastatin on healthy (n = 6) and preeclamptic (n = 6) placentas (Caucasian participants).MethodsThe eNOS activity of human placental microsomes was determined by the conversion rate of C14 L-arginine into C14 L-citrulline with or without pravastatin and Geldanamycin. Phosphorylation of eNOS (Ser1177) was investigated by Western blot. Microsomal arginine uptake was measured by a rapid filtration method.ResultsPravastatin significantly increased total eNOS activity in healthy (28%, p<0.05) and preeclamptic placentas (32%, p<0.05) using 1 mM Ca2+ promoting the dissociation of a eNOS from it’s inhibitor caveolin. Pravastatin and Geldanamycin (Hsp90 inhibitor) cotreatment increased microsomal eNOS activity. Pravastatin treatment had no significant effects on Ser1177 phosphorylation of eNOS in either healthy or preeclamptic placentas. Pravastatin induced arginine uptake of placental microsomes in both healthy (38%, p < 0.05) and preeclamptic pregnancies (34%, p < 0.05).ConclusionsThis study provides a novel mechanism of pravastatin action on placental NO metabolism. Pravastatin induces the placental microsomal arginine uptake leading to the rapid activation of eNOS independently of Ser1177 phosphorylation. These new findings may contribute to better understanding of preeclampsia and may also have a clinical relevance.
Cytochrome c is a member of the respiratory chain of the mitochondria. Non-membrane-bound (free) cytochrome c can be reduced by gluthatione as well as ascorbic acid. We investigated the effect of pH, Ca2+, Mg2+ and anionic phospholipids on the reduction of cytochrome c by glutathione.The reduction of cytochrome c by thiols was measured using photometry. Mitochondrial oxygen consumption was detected by use of oxygen electrode. Glutathione does not reduce cytochrome c at pH = 7.0 in the absence of Ca2+ and Mg2+. The reduction of cytochrome c by glutathione is inhibited by anionic lipids, especially cardiolipin. The typical conditions of apoptosis—elevated pH, Ca2+ level and Mg2+—increases the reduction of cytochrome c. Glutathione (5 mM) causes increased mitochondrial O2 consumption at pH = 8.0, in the presence of ADP either 1 mM Mg2+ or 1 mM Ca2+. Our results suggest that membrane bound cyt c does not oxidize glutathione. Free (not membrane bound) cytochrome c can oxidize glutathione. In mitochondria, O2 is depleted only in the presence of ADP, so the O2 depletion observed in the presence of glutathione can be related to the respiratory chain. Decreased glutathione levels play a role in apoptosis. Therefore, membrane unbound cyt c can contribute to apoptosis by oxidation of glutathione.
Bevezetés: Az állkapocsízületi károsodás nehezen felismerhető népbetegség, mely a folyamatosan fennálló fej-és arcfájdalom, illetve a rágófunkció zavara miatt a betegek mindennapi életét nehezíti meg. Terápiája sokrétű, több társszakma együttműködését igényli. A kezelési sorrend első lépcsője a konzervatív terápia; ennek hatástalansága esetén minimálinvazív beavatkozáshoz, azaz intraarticularis terápiához folyamodunk, melyben egyre nagyobb hangsúlyt kap a hialuronsav alkalmazása. Célkitűzés: Kutatásunk célja volt összehasonlítani az intraarticularis hialuronsavnak és a kortikoszteroidinjekciónak az állkapocsízületre gyakorolt hatását. Vizsgáltuk ezenfelül a hialuronsavas kezelés esetén a molekulasúly és az adagolás gyakoriságának kérdését. Módszer: 37, intraarticularisan, szteroiddal vagy hialuronsavval kezelt ízület gyógyítási eredményeit hasonlítottuk össze. Preoperatívan meghatároztuk a klinikai és a radiológiai (MR-felvétel) adatok alapján az ízületi károsodást jól mutató Wilkes-stádiumot, a szájnyitás mértékét és a betegek szubjektív fájdalomintenzitását reprezentáló Visual Analogue Scale-t, majd ugyanezen vizsgálatokat 6 hónap múlva megismételtük. Kortikoszteroidot egy alkalommal adtunk, míg hialuronsavat heti egyszer, egymást követő 3 alkalommal, alacsony (6-10 × 10 5 dalton) vagy magas molekulasúlyú (24-36 × 10 5 dalton) anyagot alkalmazva.Eredmények: A szteroidinjekció után a betegek állapota átmeneti posztoperatív javulást mutatott, később viszont a tünetek recidiváltak. A hialuronsavas kezelés hatására mindhárom vizsgált paraméter szignifikánsan javult a preoperatív értékekhez képest (p wilkes <0,0001; p szájnyitás = 0,0002; p VAS <0,0001); a recidíva nem volt jelentős (T = 2,05). A háromszori adagolás szignifikáns fájdalomredukciót eredményezett az első, illetve a második injekció hatásával összehasonlítva (T 3.-1. = 20,37; T 3.-2. = 9,57). Következtetés: A két anyagot összehasonlítva megállapíthatjuk a hialuronsav szignifikánsan nagyobb eredményességét, illetve a háromszori alkalmazás hatásosságát. A nagyobb molekulasúlyú változat csak a szájnyitás mértékének javításában volt hatékonyabb, de az eltérés nem volt szignifikáns. A hialuronsav minden Wilkes-stádiumban hatékonynak bizonyult, a szteroid viszont a magasabb stádiumban tartós hatást nem fejtett ki. Statins are used to treat hypercholesterolemia, with several pleiotropic effects. Alongside their positive effects (for example, decreasing blood pressure), they can also bring about negative effects/symptoms (such as myopathy). Their main mechanism of action is inducing apoptosis, the key step being the release of cytochrome c from the mitochondria. This can be facilitated by oxidative stress, through which glutathione is oxidized. In this research, glutathione was used as a respiratory substrate to measure the mitochondrial oxygen consumption of rat liver with an O2 electrode. The reduction of cytochrome c was monitored photometrically. Hydrophilic (pravastatin) and lipophilic (simvastatin) statins were used for the measurements. Pravastatin reduces the reduction of cytochrome c and the oxygen consumption of the mitochondria, while simvastatin, on the other hand, increases the reduction of cytochrome c and the mitochondrial oxygen consumption. The results make it seem probable that statins influence the mitochondrial oxygen consumption through cytochrome c. Simvastatin could enhance the oxidizing capacity of free cytochrome c, thereby increasing oxidative stress and thus facilitating apoptosis. The observed effects could further the understanding of the mechanism of action of statins and thereby aid in constructing optimal statin therapy for every patient.
Absztrakt: Bevezetés: A biszfoszfonátok által okozott állcsontnekrózis (BRONJ) az életminőséget jelentősen befolyásoló megbetegedés, mely mind általános, mind sebészeti vonatkozásaiban indokolja a korai diagnózis felállítását, illetve a kialakulás kockázatának felmérését. Célkitűzés: A BRONJ prognózisának becslése nem megoldott, számos radiológiai lehetőség közül a legkedvezőbbnek a fogászatban használatos képalkotó eljárások tűnnek, melyek közül a legígéretesebb lehet a ’cone-beam computed tomography’ (CBCT). Az oralisan alkalmazott biszfoszfonátterápia következtében kialakuló BRONJ kockázatbecslését nem hangsúlyozzák a vizsgálatok, a nagyobb kockázatot jelentő intravénás alkalmazásra fókuszálnak. Módszer: Méréseinket – az eddig publikált vizsgálatokkal ellentétben – preoperatív CBCT-felvételeken végeztük, ezáltal a kockázatbecslés lehetőségét közvetlenül vizsgáltuk. Vizsgálatainkat CBCT-felvételek értékelésével végeztük; reprezentatív területként a foramen mentale középvonalában elhelyezkedő frontális metszeteket választottuk ki, és több ponton mértünk denzitást, a corticalis csontállomány vastagságát, valamint a foramen mentale átmérőjét is megmértük. Az első vizsgálati csoportban olyan osteoporosisos betegeket vizsgáltunk, akiknél oralis biszfoszfonátterápiát követően alakult ki BRONJ. A második csoportban olyan betegeket néztünk, akik osteoporosis miatt oralis biszfoszfonátterápiában részesültek, és a szájsebészeti beavatkozást követően nem alakult ki BRONJ. A kontrollcsoportban olyan betegek voltak, akik nem kaptak a BRONJ etiológiájában szereplő gyógyszert. Eredmények: Eredményeink alapján elmondható, hogy a preoperatív CBCT-felvételeket értékelve nem tapasztalható jelentős különbség a BRONJ-elváltozástól szenvedők és az egészségesek csontdenzitási értékei között. Következtetés: Ezek alapján kijelenthető, hogy a sugárterheléssel járó CBCT-felvétel az osteoporosis miatt oralis biszfoszfonátot szedők esetében a BRONJ prognosztikai becslésére nem alkalmazható. Mindez fontos ahhoz, hogy a prognózisbecsléshez a pácienst érő sugárdózist ne emeljük szükségtelenül az ALARA (as low as reasonably achievable)-elv értelmében. Orv Hetil. 2020; 161(21): 867–872.
